What the care home does well Care plans were seen to have information on support staff should give to meet all identified needs. There were instructions and plans of care for staff regarding pain relief, wound care and how to support individuals mental well being. There is information for staff on when and how to offer homely remedies and when and how to record what they have done. Staff recruitment is carried out safely with all checks in place to protect individuals that use the service. We found that the service has taken action to meet the requirements. These changes must now be embedded into the practice of service delivery. What the care home could do better: The manager and staff must be aware of the procedures regarding safeguarding and when to make a referral. There is also a variation for one named person in the category of Physical Disablement (PD). The establishment is a detached property set in its own attractive grounds. Situated six miles from Chichester and two miles from West Wittering village. The registered provider is Intercare Orthopaedic Services Ltd. There is a new manager in post. The responsible person on behalf of the company is Dr Mayank Gupta. Fees at the time of the inspection are £550.00 - £650.00 a week. Care Homes for Older People Page 3 of 8 What we found: The purpose of the inspection was to assess how well the home is doing in meeting the key National Minimum Standards and Regulations and what action has been taken to meet the requirements made following the visit to the home on 11th May 2010. The visit was carried out on the 2nd August 2010 between the hours of 10:00 am and 11:30 am. Prior to the visit to the home we reviewed information received from the home since it was last visited in May 2010. We met with manager and deputy who explained what work they had carried out to fulfil the requirements and how those changes were becoming embedded in the practice of the home. We also spoke with two people that use the service. The previous requirements had been about care planing regarding medication and pain relief, plans of care regarding mental well being, what support staff should give to compliment treatment and care given by other health professionals such as district nurses and dressings for wound care, the administration of homely remedies and recruitment practices. We looked at two care plans and saw that either the individual or the persons representative had signed the plans of care. They had been reviewed monthly. We saw that there were plans of care for all identified needs for both individuals. There were instructions for staff regarding pain relief, when and how to manage the application of Butrans patches and when and how additional pain relief could be given. We saw that there were instructions for staff on what action they were to take to support peoples mental well being, how and when to record concerns and liaison with other professionals as needed. We saw information for staff on what action they should take regarding wound and pressure area care. This included action they could take whilst waiting for a district nurse to attend and how and when to contact support from the nursing team The manager explained the new procedure regarding homely remedies, we saw that the GP had signed a list of items for the home to use, when to use them, side effects and contraindications. The homely remedies are kept in a locked box on the wall in the managers office with a stock sheet which staff must sign to say what is being put in and taken out. There is also a list of staff who can administer these medications. We asked the manager who this was and he said the senior staff only. The manager told us that the home is changing medication supplier which he believes will prove safe and efficient. Whilst talking with the manager about staffing the manager mentioned that he was looking into an allegation about a member of staff. We asked when the safeguarding referral had been made and were told that he had not finished the investigation yet. We discussed the safeguarding process with the manager and asked that he make a referral and contact us about the incident formally. A requirement had been made following the last visit regarding employment checks for staff. The manager showed us two files for staff that had commenced employment since our last visit. We saw that the employment checks for staff were in place.
