Highfield House is a registered care home providing respite care service for adults with learning disabilities who live in the Eastleigh and Romsey District. The home is situated in the village of Bishopstoke within easy reach of Eastleigh town centre where there is a range of shopping and leisure facilities available. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys returned to us by people using the service and from other people with an interest in the service. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The results of the previous key inspection of the service on 08/11/07 and of the last annual service review on 07/11/08. What has this told us about the service? The homes annual quality assurance assessment (AQAA) was clear and gave us all the information we asked for. We looked at the information in the AQAA and our judgement is that the home is still providing a good service and that they know some of the further improvements they need to make. For example, they have identified that the service needs to continue to improve on pictorial signage and communication aids and to purchase specialised equipment for people who have complex needs. They have been putting in place pictorial all about me support plans to make these more accessible to the people they relate to. Care planning is being continually reviewed to take into account peoples changing needs. There have been further improvements made to the environment to make sure that it continues to meet the needs of people who come to stay. We sent out survey questionnaires and received comments from four of the people who use the service, who were supported by their relatives to complete the questionnaire. We also received comments from two social care professionals and three staff members. Both of the external professionals gave positive responses and indicated that peoples social and health care needs are always properly monitored, reviewed and met by the service. Both said that the service always seek advice and act on it to meet peoples social and health care needs and improve their well-being. Both social care professionals commented that the service is very person centred. They both mentioned that the service could do better at providing the same level of respite service for people with more complex needs. One said this is a staffing issue that is beyond the immediate control of the home. All of the staff members confirmed that their employer carried out checks and references before they started work in the home. Asked if their induction covered Annual Service Review Page 4 of 7 everything they needed to know to do the job when they started, two told us it did very well and one said it mostly did. All indicated that they are being given training which is relevant to their role, helps them understand and meet the needs of people who use the service, keeps them up to date with new ways of working and gives them enough knowledge about health care and medication. All three indicated that there are sometimes enough staff to meet the individual needs of all the people who use the service. Asked what does the home do well, staff members commented positively about the service, mentioning a comfortable homely environment and a well trained staff team. Asked what could the home do better, all the staff members suggested a higher ratio of staff members to service users, particularly with reference to providing a service to people with high support needs. The AQAA states that there are plans to recruit more casual staff. Staff have been purchased as and when needed to meet the specific needs of individuals, for example waking night staff. The AQAA identifies that increasingly people who use the service will have more complex needs. The service is monitoring the support it gives to people to ensure that it continues to meet their needs. People who use the service all said that they received enough information about the service before they started using it so they could decide if it was the right place for them. Two indicated that they always make decisions about what they do each day. Another said they usually do and one said they sometimes do. All said they can do what they want to do during the day, in the evening and at the weekend. All indicated that the staff and managers always treat them well. Two said that the staff and managers always listen and act on what they say and the other two said they usually do. Three of the four people who use the service indicated they know who to speak to if they are not happy. One said they know how to make a complaint, two said they did not and one did not give a response to the question. The AQAA told us that all new residents and their carers are provided with written information on how to make a complaint. This information is also provided in other formats, for example for people with a visual impairment. The registered manager has plans to undertake training on dealing with complaints and all staff are trained on the issues relating to safeguarding. The three staff members who completed our survey questionnaire indicated that they know what to do if someone has concerns about the home. The AQAA told us there have been no complaints or safeguarding referrals or investigations in the last twelve months. The home has kept us informed when there are incidents affecting the welfare of people who use the service. These notifications indicate that the service liaises with external social and health care agencies and has taken appropriate actions to ensure the wellbeing of people who use the service. Additional comments from people who use the service were that it is friendly, staff are caring and willing to do most things to support the clients in things they want to do. Another person wrote The staff are very good and do their best. What are we going to do as a result of this annual service review? However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
