Hollow Oak is a care home providing nursing care for up 27 older people some of whom may have dementia. Hollow Oak is well established family business and the current owner has been in charge for many years. The home is in the South Lakeland village of Haverthwaite, close to the market town of Ulverston and has good road access via the A590.Annual Service Review 22008The home is in a period house over three floors, which has been adapted to its current use and residents live on two of the floors. The floors used by residents have a passenger lift but two bedrooms in the older part of the home are reached by a short flight of stairs. The bedrooms in the home vary in size and layout and are individually decorated in keeping with the home`s period style and they retain many original features. The home has a large entrance area that is also used as a lounge and informal seating area, there is also large extended lounge that incorporates a new conservatory that overlooks and leads out onto the patio and the attractive garden areas. There is also a quieter lounge/dining room and another smaller conservatory for service users. Outside there are large, well kept gardens to the side and rear of the home and these are accessible to residents and have appropriate seating. There is a car park to the front of the building for visitors and staff. There are good views over the gardens and surrounding countryside from the communal rooms and many of the bedrooms. Fees payable vary depending upon the nursing needs and conditions of individuals and current information on these is available from the manager. There are additional charges for hairdressing and private chiropody. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? An Annual Service Review for Hollow Oak Nursing Home included the following: 1. We sent an Annual Quality Assurance Assessment (AQAA) form to the home and this was completed by the registered manager Angela King and the managing director Mr Hutchinson. The AQAA is a self assessment and a dataset that is completed annually by all providers of registered services. It is one of the ways we gather information from the providers of services about their service and how they believe they are meeting outcomes for the people living there and using their service. The AQAA also gives us statistical information about the individual service and trends and patterns in social care. 2.We sent out surveys to people living at Hollow Oak and to the staff who work there. We also sent out surveys to medical and nursing professionals who come into contact with the people living and working at Hollow Oak. 3. We looked at all the information we had about any changes that had been made in the running of the home and how any complaints or safeguarding issues had been handled. 4.We looked at what the Providers of the service have told us about things that happened in the service, these are called notifications and are a legal requirement. We also looked at how the providers have kept us informed about any changes in the organisation. 5.We looked at the previous key inspection and any telephone calls to speak with the manager and provider or visits we have made to the home since their last inspection. What has this told us about the service? The home has a suitably qualified and registered manager/matron in post, Angela King, who is a registered nurse. An AQAA was returned to us and it contained the general information we asked for and some examples to illustrate what the service has done in the last twelve months. The AQAA told us what the manager was doing to make sure that the views of people living at Hollow Oak were listened to and promoted in the way the home was run. The AQAA information told us they believe they do this do this by using a customer satisfaction enquiry given out annually. This is to ask people living there and their families if they are satisfied with their care and are happy living there. The manager told us in the AQAA that, Listening and reacting to patients and relatives has always given confidence in our care and continues to be paramount. Surveys received back from seven people living in the home indicated that staff always listened to them and acted upon what they said. Examples of changes made to improve the service and facilities for people living at Annual Service Review Page 4 of 7 Hollow Oak were stated in the AQAA. This included important areas such as the environment where considerable investment had been made in the building of four new single bedrooms and a new shower room and toilet. This provides more single bedrooms for people and more en suite facilities. The home has also created a new laundry to improve this facility. These things help provide a suitably homely environment for people and keep them safe and comfortable. The owner and manager told us in the AQAA that they strive to give a feeling of homeliness to the people who live there and to create the family of Hollow Oak. Feedback we got from the surveys we sent out to people living there indicated that they were satisfied with their care and made many positive comments about the service and the staff in their home. They indicated that staff were always available when they needed them and they felt they always got the medical care they needed. Comments from people living there included, The senior staff are very helpful, as are the carers and domestic staff. We get good care and attention. They show care and concern. The meals are excellent and the home is spotless. The home is always very clean, the staff are cheerful and helpful and they respond to the bell promptly. Survey responses from people living at Hollow Oak indicated that they always liked the meals provided in the home and that it was always clean and fresh. Other improvements the manager and owner felt they had made was in being able to take more people out to their private medical appointments, such as the dentist and optician using their own transport. The homes mini bus is also being used to take people out, both singly for shopping, and in groups for musical and theatre shows. The AQAA information told us that the service has a complaints procedure that is on prominent display in the home and that any problems that arise are dealt with immediately. The manager told us in the AQAA that they have not received any complaints about the service in the last year. We (CQC) have not received any complaints about this service either. Survey responses from people living there indicated that they knew about the process for making a formal complaint, should they have one, and that there was someone they could speak to informally if they were not happy. We did not receive any negative comments back in our surveys from people living in the home. One person did comment that they would like an exercise class as part of activities and be given more notice on activities. These are matters that the manager can address through consultation with people living in the home about what their recreational expectations are. The comments and surveys we received from health and nursing care professionals raised no concerns about the way peoples personal and health care needs are monitored, reviewed and met by the service. The responses indicated that advice is usually sought and acted upon to meet peoples social and health care needs and help people make choices in these areas. Comments indicated that the manager had Annual Service Review Page 5 of 7 responded appropriately when a concern had been raised over an aspect of care. Comments we received from nursing and medical professionals included: Hollow Oak provides a very homely and warm atmosphere. No members of nursing or care staff replied to our surveys. The AQAA information on staffing indicated a low rate of staff turnover and that there is a stable core staff group to provide continuity of care. The home has not used any agency staff. The AQAA information indicated staff have a development programme and that training is being done in the important areas of infection control, food hygiene and nutritional screening and NVQ training. This is to help ensure the staff are up to date and aware of best practice in these areas. The AQAA information also states that staff have had all the appropriate recruitment and security checks done before they start work. This is to help make sure that the right staff are recruited to work with vulnerable people. We looked at the information we had on the changes and improvements made since the last inspection. We looked at all of the information that we had gathered about Hollow Oak to form a judgement about the outcomes for people living there. The information in the AQAA was succinct and supported by the survey information we received. The AQAA also indicated how service developments and improvements in care and support were being managed. The responses of nursing and medical professionals coming into contact with the service were positive or constructive. The manager has notified us of accidents and events in the home required by regulation and the owner keeps us well informed on service development. On the basis of the information that is available to us for this review we judge that the service still continues to provide very good outcomes for the people living at Hollow Oak Nursing Home. What are we going to do as a result of this annual service review? The Care Quality Commission will continue to monitor information about this service and will carry out an inspection when required. We can visit and carry out an inspection of the service at any time if we have concerns about the quality of service being provided to people or the safety of the people living there.
