The Step Down Service (House 2, Slade House) is a four bedroomed house situated on The Slade site in Oxford. It provides a transitional placement for individuals who have a learning disability and have moved from secure in-patient environments. The service falls within the remit of the specialist health services at the Oxfordshire Learning Disability NHS Trust (OLDT). The house provides short-term accommodation and outreach support for up to four people who are assessed to subsequently move into the community within a timescale of two years. Clinicians and the management team, before acceptance into the service, formally assess individual`s needs. Service users identify their own needs through a `life planning` process/tool and are supported by staff to gain the skills required to achieve goals and eventually achieve independence in the community. The current range of fees for this service is unavailable due to funding being received through the National Health Service.

What the care home does well The home completes a clear and detailed assessment of need before approving that a service user moves into the service, to ensure that individual care needs can be met. Health and social care professionals are involved in the assessment to ensure legal requirements in relation to the Mental Health Act are met. The needs and personal goals of people who live in the service are recorded within support plans. People are appropriately supported to make decisions and to take acceptable risks within a risk assessment framework. People who live in the service are supported to make lifestyle choices that reflect their needs and wishes and are supported to develop life skills. The personal and healthcare needs of service users are supported appropriately. Service users can be confident that any complaints made will be listened to , taken seriously and acted upon. Service users are protected from abuse, neglect and harm through the service`s policies and procedures. There are appropriate levels of well trained staff in post to meet the needs of the service users. We gained the impression that there is a good rapport between service users and members of staff. Service users benefit from a well managed service, lead by an experienced and qualified manager. What the care home could do better: The service is to be extended and refurbished in the next twelve months. At the last two inspections it has been noted that window restrictors were either broken or has been disabled. Appropriate risk assessments were seen in relation to one service user. Following the inspection we received an action plan from the manager, who is to put into place risk assessments for all first floor windows currently without window restrictors and advice has been sought from the Fire Service, who do not endorse the use of windowrestrictors. A letter confirming this will be forwarded to the commission. Some good practice advice was given regarding the testing of hot water temperatures and the frequency of recording. Care Homes for Adults (18-65 years) Page 3 of 8 What we found: This random focused inspection visit took place on the 22nd June 2010, we the (commission) arrived at the service at 11:50 and we were in the service until 14:00. The focus of the inspection was to look at experiences and outcomes that affect service users. We spent time with one service user, the manager and two members of staff. We looked at and discussed plans for the refurbishment and development of the service, a sample of service user and staff records and records relating to health and safety. The last key inspection of this service was undertaken in August 2009, when the quality rating was judged to be good.
