Kings Court is a care home which is registered to provide personal care and accommodation for up to 37 older people. The home does not provide for people who require nursing care but community based nursing staff can visit the home if this is required. The home is a private business, owned and managed by Southern Cross OPCO Ltd. Kings Court is a converted home located in the centre of the market town of Barnard Castle. All except one ofthe bedrooms in the home have their own toilet facilities and there is a passenger lift to help people get upstairs. There are a number of comfortable communal sitting areas and a large lounge. The bathrooms and communal toilets have equipment to help people and there are other aids provided for those people who need assistance for example with their mobility. There are public houses, shops, library, churches and other amenities close by, and regular bus services to nearby areas. There is easy level access into the home from the front entrance and car-parking facilities are provided. What the care home does well Kings Court provides a comfortable and homely environment for people to live. What the care home could do better: The manager must apply to the Care Quality Commission to become the registered manager. Training in Moving and Handling for staff is required. Care Homes for Older People Page 3 of 8 What we found: This inspection was a random unannounced inspection. It took place on 24 June 2010 and was completed in one day by one compliance inspector. At the time of the inspection there was a manager in post, however an application had not been made to the Care Quality Commission for the manager to become registered. The purpose of the inspection was to look at a range of matters that effect the care, welfare and wellbeing of people living at Kings Court. During the visit we looked at a number of care records of people living at Kings Court, medication administration records, staff recruitment records and staff training records. We also looked at some maintenance and servicing information along with quality assurance information. We had a look around the home, spoke with people living there, staff and the manager. The care files of three people at Kings Court were looked at. One of the care records was for a person who had been recently admitted to the home. The files contained assessments, which had been carried out prior to the people moving in to ensure their needs could be met. Three peoples care files were examined. The documentation in the files was easy to follow and all information regarding one particular area was kept together. For example one person had an identified need regarding diet and nutrition and instead of the assessment, MUST, fluid intake, nutritional intake, care plan and information from other health professionals being kept in different parts of the care file it was all held collectively. There were some good examples of person centred care plans. For example one person was unable to communicate verbally and in the care plan it stated that you needed to approach this lady from the front, calmly, with a relaxed smile. Sit on a chair next to her so you were on the same level. Speak in a clear slow manner. Observe for facial and body movements, non verbal communication. The monthly evaluations were detailed and informative. Daily progress reports gave a picture of how this lady spent her days. Medication systems were looked at. The medication administration records for prescribed medication were found to be complete with no gaps in the recording. When peoples prescribed creams or bottles are opened they were dated. The home does not use homely remedies. The home employs an activity co-ordinator and on the day of the inspection some people people were out with staff having coffee in one of the local cafes. Later on in the day a number of people were joining in with a game of bingo, a visit to look at trains had also been organised for the afternoon. Care Homes for Older People Page 4 of 8 There had not been any complaints made since the previous inspection. The home has a rota in place, which shows a skill mix of staff being on duty on each shift. The home has a policy and procedure in place for the recruitment of staff. Three staff files examined showed that the required information, references and CRB checks were in place prior to staff commencing work in the home. Since the previous inspection staff had undertaken training in Fire Safety, Focus on Food, Infection Control and First Aid. The manager said that all staff were currently undertaking training for Safeguarding People. This was via the Local Authority and work books and a DVD were being used. Staff spoken to confirmed this was the case. Moving and Handling training was required, the manager said this was to take place in July 2010. Staff have individual training records, however at the time of the inspection they were not up to date. The manager said she was in the process of updating them. At the time of the inspection there was not a registered manager at the home. A manager was responsible for two services, and was working half a day in Kings Court and half a day at the other service. This arrangement had been in place since October 2009. An application must be made to the Care Quality Commission for a registered manager to be in post for Kings Court Care Home. A quality audit had been carried out by a Quality Inspector within the organisation in April 2010 and there were some points for action by the manager. This was then followed up on the following Regulation 26 visit. There was evidence to show that Regulation 26 visits are carried out each month, the previous one being on 20 May 2010. Staff end people who use the service were spoken to and the environment and records were checked.
