What the care home does well Mandale House achieves good and safe outcomes for people who use its service across all outcome areas. People who use the service and their relatives were overwhelmingly postive and complimentary in their comments about how the home has been performing and the responsiveness of staff and management to suggestions for improvement, such as staffing levels. Environmental standards have been improving also, including redecoration and refurbishment, plus tidying and landscaping the grounds. Very positive comments were received about the caring attitudes and practices of staff. It is also commendable that all care staff have achieved National Vocational Qualifications (NVQ) in care at level 2 or above. Other staff training including induction is comprehensive and appropriate for the needs of the home. The service is well managed and administered and there is a strong commitment to maintaining and improving positive outcomes for service users and their families wherever possible. What the care home could do better: The home is in the process of transferring service user plans of care onto new formats that will assist the development of care needs assessment, care planning and care evaluation processes. Care plans should be reviewed and evaluated at least monthly and the home is working towards that standard. Statutory reports of monthly visits required under regulation 26 (Care Homes Regulations 2001) are not always being completed. The home is reminded of its obligation to ensure that such visits are reported on each month. Date of last inspection Brief description of the care home Mandale House is a 57-bedded care home providing personal care for older people and older people with dementia. 55 of the bedrooms are single with ensuite facilities and Care Homes for Older People Page 2 of 8 1 9 0 8 2 0 0 9 Brief description of the care home there is one double room with ensuite facilities. The home operates two dedicated units, 30 beds within the upstairs unit for older people with dementia and 27 beds on the ground floor for older people who have personal care needs. Mandale House is situated in close proximity to a local park, other community facilities and a public house. Public transport is in easy reach and the home is on a main traffic route. At the last inspection, the fees for living at Mandale House ranged from £381 to £400 per week, depending on need and the local authority. Care Homes for Older People Page 3 of 8 What we found: The inspection took place on 14th June 2010 over 4 hours and included an examination of records and documents, plus discussions with people who use the service, staff and relatives. The inspection was also informed by the services Annual Quality Assurance Assessment (a self-assessment of its performance over the past year) and a number of surveys completed and returned by service users and/or their relatives. There are good arrangements for the provision of health and personal care at Mandale House. Comments received from people who use the service and/or their relatives confirmed that people experienced good outcomes concerning their health and personal care. Current plans of care are in the process of being transferred onto new formats, which will provide more focused and accessible information. Existing plans of care need to record that they are being evaluated monthly or more often as circumstances warrant. Currently, these written evaluations are occurring every two months, which is below the national minimum standard of once a month or more often if required. However, peoples dignity and privacy are being promoted by the care practices operating within the home. There are good arrangements for the safe administration of medicines, as confirmed by a PCT Pharmacy audit in April 2010. All staff responsible for administering medicines have completed accredited training (NCFE). A varied programme of social and recreational activities is provided. People who use the service and their relatives spoke highly of the programme, especially in December last year when staff and some relatives dressed up in Victorian costumes to provide entertainment for service users. A relaxed and supportive atmosphere is evident in the home, respecting and promoting individual choice. The home is planning to introduce a reminiscence room on the first floor, supplied with objects and artefacts from the last century, with which many service users will be familiar. Peoples rooms are personalised and there are short lifestories available in care plans, helping staff to get to know the individual service user as a unique person. There is an activities coordinator responsible for social and recreational activities and there is evidence of local community involvement. Good use of posters and pictures is made throughout the home and there is a tropical fish tank situated in a ground floor lounge. People said they enjoyed the catering and there was evidence of a good choice of menu operating on a 4 week cycle. Careful support is provided for people who require assistance with eating their meals, as observed on the first floor in the dining area. The kitchen has been awarded the maximum 5 stars by the environmental health authority for food hygiene. There are good arrangements for complaints and protection. People who use the service and their relatives said that staff were approachable and helpful, and they could discuss any concerns or complaints with them. Copies of the complaints procedure are readily available throughout the home. The home has successfully applied for deprivation of liberty safeguards in two cases recently, with the assistance of the Local Authority, in the best interests of the people concerned. The home has robust policies and procedures in place concerning safeguarding vulnerable adults and the signs and symptoms of abuse or neglect. Staff have undergone appropriate training. The home was found to be clean and well maintained. There is a rolling programme of decoration and refurbishment and improvements have been carried out to the grounds and garden area. A warm and friendly atmosphere has been created. Staffing levels and deployment were found to be appropriate for the needs of service users. These have been recently reviewed and increased. People who use the service and their relatives were complimentary in their comments about the caring practices of staff. Care Homes for Older People Page 4 of 8 Some also said that more staff would be helpful, but the home is keeping levels under review and has increased them recently. A full programme of staff training and induction is provided and it is commendable that all care staff members have achieved National Vocational Qualifications (NVQ) in care at level 2 or above. There are generally good arrangements for the management and administration of the home. Since the last inspection, the manager has achieved registration with CQC, and holds appropriate qualifications, including the Registered Managers Award at NVQ level 4. The home has a dedicated administrator and there was evidence from staff files and training programmes that the service is well organised and administered. One area where improvement is needed concerns the monthly regulation 26 reports which were not available for the past two months. The manager said that there were proposals for such reports to be completed by registered managers visiting other homes within the group under regulation 26. This seems to be a good idea, but in any event the regulation 26 visits must take place and be reported on each month. There was evidence of quality audits being carried out routinely, including health and safety matters. Summaries of service user satisfaction surveys were available on notice boards, providing useful feedback on what the home does well, plus identifying areas for improvement.
