What the care home does well The service is well regarded by the people living in the home and much satisfaction has been expressed in service user surveys, confirming this observation. The home has made a big effort to redecorate and upgrade bedrooms in consultation with service users and in accordance with their preferences. A new kitchen is being installed in June 2010. Improvements have been made in documenting care plans and complaints, safeguarding training, health and safety training, and reporting on regulation 26 visits. There are good arrangements for health and personal care, including the safe administration of medicines. It is commendable that all care staff have achieved NVQ level 2 or above in Care. What the care home could do better: Care plans should be reviewed and evaluated at least once a month and documented to record this process. The quantities of medicines received for each service user should be confirmed on the MAR sheets and initialled by the staff concerned, to provide a clear audit trail. If there are any discrepancies, that information should also be entered onto the MAR sheet and the entry should be signed and witnessed by 2 staff (the person making the entry and a second member of staff witnessing the entry). The home is advised to check with the Pharmacist about best practice in this area. It is a twostorey building providing single and shared accommodation for up to 24 residents. There are 18 single bedrooms and 3 double bedrooms; 17 bedrooms have an en-suite Care Homes for Older People Page 2 of 8 1 2 0 8 2 0 0 9 Brief description of the care home facility, with a wash-basin and toilet. There are stairs as well as a passenger lift, giving people access to the first floor. Communal rooms are: a large lounge, conservatory and a dining room that has a seating area. Primrose Court has a small car park at the front of the building and the home is close to local shops and amenities. The fee, for residents at Primrose Court, is £465 weekly. Care Homes for Older People Page 3 of 8 What we found: People who use the service said that they were very satisfied with the homes arrangements for health and personal care. This was borne out in the sample of service user satisfaction surveys seen on the day. People felt they were treated with respect for their privacy and dignity. Suitable locks are supplied to bedroom doors in support of these principles. Also, the home has fitted electronic door closers to bedroom doors, which permit service users to have their doors open if they wish, knowing that in the event of a fire, the doors would close automatically. Bedrooms have been extensively redecorated, reflecting the preferences and choices of the occupants concerning colour, fabrics, wallpaper and furnishings. Each room is unique in this regard and service users said they were very pleased with the results. Care plans were examined and found to contain detailed information, including daily records, enabling staff to access full information guiding the care and support required. The home has made progress in this area. It was noted that the recording of monthly evaluations had slipped over the past 2 months. It is important to keep these records up to date to show that the home is actively reviewing and evaluating each plan of care. The minimum frequency should be once a month, but more often if required. Medicines records were well maintained and there are good arrangements in place for the safe administration of medicines. All staff responsible for medicines have completed accredited safe handling of medicine training (NCFE). There are good storage arrangements and the medicines room was noted to be well organised. The pre-printed Medicines Administration Records (MAR sheets) supplied by the Pharmacist are cross checked by the home with the medicines ordered and delivered. It is advisable to record confirmation of the accuracy of quantities received, by staff initials being placed on the MAR sheet. If there are any discrepancies, that information should also be entered onto the MAR sheet and the entry should be signed and witnessed by 2 staff (the person making the entry and a second member of staff witnessing the entry). The home is advised to check with the Pharmacist about best practice in this area. A full programme of social and recreational activities is in operation. People who use the service said they were satisfied with these arrangements and confirmed that they are regularly consulted about their likes and dislikes, plus any suggestions for improvement. It was noted that the home provides a Wii service, so that people can engage in a variety of activities using hand-held equipment and the large, flat-screen television. The home records complaints and actions taken to resolve them. It also has a record of thank you cards and compliments received. People who use the service said that the staff were very approachable and they felt able to discuss any concerns they might have. Staff have undergone training in adult protection since the last inspection. The home has been undergoing major redecoration of service users bedrooms, reflecting peoples individual choices and preferences. Also, a new kitchen is to be installed later this month. The home is creating a treatment room on the ground floor that will be available for a variety of examinations and treatments, saving people from having to use their bedrooms. There are good staffing arrangements with 3 care staff on duty throughout the day (8 am to 9 pm) and 2 at night (for 19 service users on the day of inspection). All care staff have achieved National Vocational Qualifications (NVQ) in care at level 2 or above, which is commendable. Also, each member of staff has an individual staff training record, helping to record any training needed or received. Catering and domestic staff are deployed in sufficient numbers for the needs of the home. Care Homes for Older People Page 4 of 8 There is a good management structure in place, including the registered manager and 2 deputies. There was evidence of good teamwork and a strong commitment to good outcomes for people using the service. Staff confirmed they were well supported and that their training needs were being well met. For example, refresher courses in moving and handling take place twice a year. Also, fire drills and training are being recorded in greater detail to ensure that all staff receive the required amounts. Regulation 26 reports are being completed each month as part of the homes quality assurance process. Also, service user satisfaction surveys are being completed, with the results being summarised and published on the notice board, so that people can provide and receive feedback on the quality of the service. The manager and deputies are due to commence NVQ level 4 in care this Autumn. Contracts of residency have been updated and include individuals room numbers as well as fees.
