What the care home does well The homes assessment procedures were very thorough and care plans had been structured to ensure staff employed by the home recognise the diverse needs of the people they support. Observation of practice and discussion with the staff members on duty confirmed the staff team had been provided with appropriate training to assist them in understanding and meeting the needs of the people in their care We spoke to people about their experiences living at the home and were told they liked living there and their needs were being met. One person said, "I have no complaints about my care and cannot praise the manager and her staff highly enough". Staffing levels were sufficient for the number of people living at the home. People we spoke to said the staff were available when needed. Routines observed throughout the day were relaxed and the staff members on duty appeared to have sufficient time to meet the needs of the people in their care. People spoken to said they enjoyed the quality and variety of food being provided and the activities being organised. One person said, ""Always enjoy my meals. You get plenty to eat and a choice is always available". Observation of care practices throughout the day confirmed the people they support are treated with respect and dignity. The home has 100% of staff who have achieved National Vocational Qualifications (NVQ) ensuring people are supported by competent and qualified staff. The home has been decorated and furnished to a good standard to ensure the comfort of people who live there. The people we spoke to were very happy with the standard of accommodation provided. What has improved since the last inspection? The home has continued to make improvements to the environment since the last inspection. A number of bedrooms have been redecorated and refurbished. The manager said the refurbishment of the home was ongoing and there were further plans for refurbishment throughout the home to ensure the present standards are maintained. The people we spoke told us they were happy with the improvements being made. The manager has completed the registration process with the Care Quality Commission confirming she is a fit person to run the home. What the care home could do better: The manager should ensure care plans are reviewed at least once a month and updated to reflect any changing needs. This will ensure the staff have up to date information about the people in their care and the appropriate level of care is being provided. Staff working at the home must have all relevant information in place including two written references before they commence their employment. This will confirm they have been recruited properly and provide protection for the people in their care. The quality rating for this service is 2 stars. This means the people who use this service experience good quality outcomes. This unannounced visit was undertaken as part of the homes key inspection. The visit commenced at 9.30 am and took place over 5 hours. Prior to the visit the manager of the home completed an Annual Quality Assurance Assessment form (AQAA) providing detailed information about the service they are providing. The AQAA is a self assessment and data set, which providers are asked to complete each year and is one of the main ways that we obtain information about how they are meeting outcomes for people in their care. Staff, care, maintenance and financial records were examined during the visit and a full tour of the premises was undertaken with the manager. We have made this judgement using a range of evidence, including a visit to this service. The admission and assessment procedures were clear to ensure the care needs of people were met. EVIDENCE: The care plan records of two people admitted to the home had full assessment information including their religious/cultural, relationship, social and healthcare needs. The assessment had identified the nutritional needs of the people and had risk assessment information advising staff members of the action to be taken to minimize identified risks and hazards whilst providing personal care. The staff members on duty confirmed they had access to this information and were fully aware of the care needs of the people living at the home. The people we spoke to during the visit confirmed they were happy with the care being provided and their needs were being met. One person said, “My daughter looked around the home and decided it was the right one for me. I am well looked after and happy with her decision”. This home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. We have made this judgement using a range of evidence, including a visit to this service. Promotion of health is taken seriously and health needs were met. EVIDENCE: Individual records are kept for each person with a plan of care setting out the action that is needed to be taken by support staff to ensure all aspects of health, personal and social care needs of the people living at the home were met. Significant events had been recorded and daily entries made setting out the care given. The care plans were structured and the staff members we spoke to said they found them easy to follow and felt they were providing the appropriate level of care. The two care plans we looked at hadn’t undergone a recent review of the care being provided. The manager was reminded care plans should be reviewed at least once a month and updated to reflect any changing needs and these should be actioned as required. Discussion with people living in the home and observation of practice throughout the day Rosehaven Residential Care Home DS0000069552.V377241.R01.S.doc Version 5.2 Page 12 confirmed people were receiving the appropriate level of care and they were happy their needs were being met. The manager told us the care plans would be reviewed and updated as a matter of priority. The care plans we looked at had identified and described the healthcare needs of people and how these should be met. Discussion with staff members on duty confirmed they were fully aware of the healthcare needs of people living at the home. One person who became poorly during the visit was dealt with promptly and professionally by staff members and received immediate medical assistance. The manager told us the person had an ongoing medical problem and was being closely monitored by the staff team for any signs of deterioration. The family of the person concerned was contacted by the manager and kept informed about developments. Entries made on care plans showed good communication between the home and healthcare professionals. Two healthcare professionals visiting the home said they were satisfied with the care being provided and had no concerns. We spoke to people about the care they were receiving and were told they were happy and their needs were being met. One person said, “I really cannot find fault with anything. The girls are very attentive and caring. We are well looked after”. Observation of practice and examination of care plan records confirmed staff were meeting the diverse needs of people with specific disabilities and specialised equipment was readily available to assist them with their duties. Discussion with staff confirmed they were aware of the needs of the people they were supporting and the level of care that needed to be provided. Medication practices observed were safe and good records had been maintained. The staff members responsible for the administration of medicines had received accredited training to ensure they had basic knowledge of how medicines are used and how to recognise and deal with problems in use. People spoken to said the staff team respected their privacy and they could spend time on their own if that was their wish. Staff members were observed knocking on bedroom doors and waiting to be called in before entering. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. We have made this judgement using a range of evidence, including a visit to this service. Social activities and meals are both well managed, creative and provide daily variation and interest for people living in the home. EVIDENCE: We spoke to people about routines within the home and they told us they were flexible and they were able to make their own decisions about how to live their lives. One person said, “I prefer to stay in my room where I can watch television. The staff are very good and respect my wishes”. We spoke to people about the arrangements in place for receiving their visitors and were told they were happy with these. One person said, “My daughter visits most days and is always made welcome. She gets on very well with the staff and they always up date her on my care when she visits”. People spoken to confirmed they enjoyed the food provided by the home. The staff members responsible for the preparation of meals were able to confirm Rosehaven Residential Care Home DS0000069552.V377241.R01.S.doc Version 5.2 Page 14 they had information about people with special diets and personal preferences and these were being accommodated. One person said, “The food is lovely. You get plenty to eat and it is always well presented and appetising. Good home cooking and vegetables are always fresh and not over cooked. The cook comes around every day telling us the choices available and if you don’t like them they will try to accommodate what you want. I have put weight on since I moved into the home”. Meal times were served in a relaxed and unhurried manner. Staff members we observed were being very attentive to the needs of people. People we spoke to were very happy with the arrangements in place for social activities. These were varied and arranged individually and in groups. We have made this judgement using a range of evidence, including a visit to this service. Arrangements for complaints are handled well and taken seriously ensuring people feel listened to. EVIDENCE: The home has a detailed complaints procedure, which is made available to everyone on his or her admission. The people we spoke to knew how to make a complaint and felt these would be listened to and acted upon. One person said, “There is nothing to complain about I am perfectly happy. This is my seventh stay at the home and I always enjoy coming. The care is good, the food is lovely, we are kept entertained, the staff are kind and friendly and I have made some good friends. If I did need to complain about anything I am sure it would be dealt with appropriately”. At the time of this visit no complaints had been referred to the Care Quality Commission. The home has a procedure in place for dealing with allegations of abuse. Staff members have received training in the protection of vulnerable adults and showed a good understanding of the procedures to be followed in the event of any allegations or suspicion of abuse or neglect. At the time of this visit the Care Quality Commission had not received any safeguarding referrals about staff working at the home. We have made this judgement using a range of evidence, including a visit to this service. The home has a planned maintenance and renewal programme for the redecoration and refurbishment of the home to ensure people live in a comfortable, homely, clean and safe environment. EVIDENCE: The home has been well maintained and decorated for the comfort of the people living there. All furnishings were of a good standard and helped to provide a homely environment. People spoken to were very happy with the standard of accommodation being provided and said they felt at home. The manager said the refurbishment of the home was ongoing and there were further plans for refurbishment throughout the home to ensure the present standards are maintained. The people we spoke to were happy with the improvements being made. A tour of the building confirmed people had personalised their bedrooms with their own belongings. People spoken to were happy with their rooms and said they had the choice of spending time on their own or in the lounge area’s. Hot water temperatures throughout the home were checked and found to deliver water at a safe temperature in line with health and safety guidelines. It was observed during the visit the home was clean and hygienic ensuring a pleasant environment in which to live. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. This is what people staying in this care home experience: JUDGEMENT – we looked at outcomes for the following standard(s): People using the service experience good quality outcomes in this area. We have made this judgement using a range of evidence, including a visit to this service. Staffing levels are sufficient to meet the needs of people living at the home but recruitment procedures are not robust potentially placing people at risk. EVIDENCE: Staffing levels were sufficient for the number of people living at the home. The people we spoke to told us they were happy with the care they were receiving and were well treated by the staff. One person said, “I find the staff very caring and attentive. When I was taken ill during the night they responded as soon as I requested help. They were wonderful with me and I couldn’t have received better care anywhere”. The staff members we spoke to told us they were clear about their role and they work well as a team to ensure the individual and collective needs of the people in their care are met. Observation of practice during the visit confirmed people are treated with respect and dignity. Records show all staff members have access to a structured training and development programme ensuring the people in their care are being supported Rosehaven Residential Care Home DS0000069552.V377241.R01.S.doc Version 5.2 Page 20 by a well trained and competent staff team. In addition 100 of staff members have achieved National Vocational Qualifications (NVQ) ensuring people are in the safe hands of qualified and competent staff. We looked at the records of two staff members recently employed to work at the home and found that a Criminal Records Bureau (CRB) and Protection of Vulnerable Adults (POVA) check had been completed and documents required to confirm the identity of both staff members were in place before they commenced their employment. Both files seen only had one reference in place. The manager told us that both staff members had worked for her at a previous care home and as their last employer didn’t feel she could provide a reference when she was involved in their recruitment to the home. The manager told us she had been completely satisfied with the work and conduct of both staff members and would not have re-employed them if she had any doubts about their suitability. The manager was reminded that all relevant information must be in place before new staff members commence their employment at the home. These records are required to confirm their suitability to work with vulnerable people and reduce any potential risk of harm to the people in their care. The manager told us that should a situation arise again where she wishes to re-employ someone for whom she had been their last employer she would provide a reference and also seek a character reference from an independent person. We have made this judgement using a range of evidence, including a visit to this service. The home is well run and has policies and procedures in place to ensure the health and safety of staff and people living there are promoted and protected. EVIDENCE: The manager of the home has many years experience in caring for elderly people and has achieved a relevant care and management qualification. She is well supported by the owners of the home who visit regularly. Records seen confirmed the manager has access to training to ensure her knowledge and skills are regularly updated. We were told the manager and her staff were friendly, approachable helpful and professional. The home has effective quality assurance systems in place to monitor the level of service being provided for its residents. An annual quality assessment of standards is undertaken by a professionally recognised organisation who completes an audit of the care being provided and seek the views of the people who live at the home and their relatives. In addition the home has in place its own quality assurance systems in place to gather the views of people and keep them informed about events being organised by the home. People spoken to confirmed they are consulted about any changes taking place within the home and kept fully informed about forthcoming events being organised. Financial records were well maintained and up to date ensuring the interests of people are safeguarded.
