What the care home does well Following our last key inspection of this service on 1st April 2010 we wrote: `we found that people living in the home were being well cared for and that staff understood their needs and were meeting these`. What the care home could do better: Because the provider has decided to close the home and has applied to cancel his registration, we will act proportionately in continuing to monitor the service until the date that the registration cancellation takes effect. The home is located on a busy main road in the heart of Bedford town within close proximity to the local hospital, and amenities including transport links. The
Care Homes for Older People Page 2 of 11 Brief description of the care home accommodation is set out over three floors - accessible via a staircase and a shaft lift. All bedrooms are single occupancy and communal facilities include a small lounge and lounge/diner on the ground floor. At the time of this inspection, two vacant rooms were being used to provide additional communal space - a quiet room and an activity room. At the time of this visit, work was still in progress to extend the home to create a further 7-8 bedrooms - all with ensuite facilities. Bedrooms in the older part of the home do not have ensuite facilities. A new enclosed garden/sitting area has been created to the side of the property. Information about the home is available within the Statement of Purpose (SoP) and Service User Guide (SUG). Both are available within the home along with a copy of the most recent inspection report for this service. The SUG states that the fees for the home at the current time are £457.06 per week upwards. A random inspection of Sandhurst Lodge was carried out on 19th May 2010. The purpose of the inspection was to assess compliance against an immediate requirement that we made at our last random inspection of this service on 29th April 2010. The immediate requirement was about there being little or no evidence of recruitment/vetting checks for family members of the provider who were working in the home, as carers. We did not check compliance against other requirements made in the last random inspection report on this occasion. Therefore where requirements are stated as being outstanding, they may not have been assessed during this inspection. We confirmed again with the manager that no new staff had been employed since our 1st December 2009 key inspection, when we had deemed there to be appropriate recruitment and vetting processes in place for staff working in the home, who were not family members of the registered provider. Therefore we only looked at records relating to two family members of the provider during this inspection. The manager told us at the time of the last random inspection that family members would not work at the home until all the required documentation was in place. We also received a letter from the provider to this effect on 30th April 2010. Rotas that were seen on this occasion however, showed us that one of the family members had been working in the home immediately after the last inspection, and another was planning to work in the home in the near future. As we reported in our 29th April 2010 report, there was evidence of some of the required recruitment and vetting documents and checks being in place for the family members, but there was also some that was still missing. This included a CRB (criminal record disclosure) check and a reference for one person, and where references had been obtained; no information to say in what capacity the referee had known the person they were providing a reference for. This was still the case at the time of this inspection. There was evidence that the manager was taking up new references for one person who did have a satisfactory CRB disclosure, and we were told that a CRB had been done for the other person although this could not be found. We spoke to someone on the phone during the inspection who told us that they were responsible for countersigning CRB disclosures for the home. They confirmed that a CRB had been done for the person whose CRB was missing, approximately 3-4 years ago. They also said that they could provide evidence of the disclosure reference number after the inspection. There was evidence that the person in question was not working in the home at the time of this inspection however. Prior to this inspection we received an email that told us that the registered provider had

During this inspection, the provider confirmed this and handed us his application for voluntary cancellation of the homes registration, with a proposed effective date for the cancellation to take place by 28th June 2010. The provider expressed his commitment to ensuring everyone living in the home was given enough time to find suitable alternative accommodation. We confirmed with Bedford Borough Council* after the inspection that they were aware of the situation and that appropriate steps were being taken to find the 5 people living in the home alternative accommodation. *Bedford Borough Council currently fund the 5 remaining people living at Sandhurst Lodge. No. Standard Regulation Requirement Timescale for action has applied to the CQC for registration, or that you have applied to have your provider registration cancelled. The concerns are that the home is being carried on by an unregistered provider. As well as this being an offence under the Care Standards Act 2000 and associated regulations, the insurance arrangements for the home (including legal liability cover) may be at risk. You must write to the CQC and confirm if/when an application to cancel your registration, and an application for a change of registered provider, has been submitted to the CQC. You must also provide confirmation that there are adequate insurance arrangements in place for the home given the current situation. This verification must be provided by the insurance company in question. To clarify who is carrying on the home and that there are clear lines of accountability in place. 4 34 25 There are concerns that the 19/04/2010 home may not be financially viable for the purpose of achieving the aims and objectives as set out in its Statement of Purpose.
