What the care home does well Speke Care Home is on the whole clean and well furnished. Medication is well organised though there are some issues to be addressed. Care plans are detailed. In the absence of the registered manager and her deputy the home is being managed by an acting manager with the support of the owner and a registered manager from one of his other homes. All of the rotas were fully staffed. What the care home could do better: The unpleasant odour in one room needs to be eliminated. The conservatory furniture needs to be replaced and as it is used as a smoking room it should have an extractor fan fitted. This room is shaded from the sun by very rough and ready painting on the perspex roof. This does work but is very unattractive. Some minor improvements need to be made to the medication procedures. The very brief daily reports do not reflect thedetailed care plans and we discussed with the acting manager ways of improving this. Skin care issues should be addressed in a seperate element of the care plan if action is needed. Care Homes for Older People Page 3 of 9 What we found: We visited Speke Care Home to check the arrangements made for running the home whilst the manager and her deputy are suspended pending an enquiry. We also looked at progress on the requirements and recommendations made when we last inspected the home. We talked to the acting manager, a senior carer, the owners father and several people who live in the home. We also looked around the home and checked the administration of medication. We looked at the case files and care plans of two people who had been the subject of recent complaints. Medication is securely stored in a locked room. When we last inspected the home the room was very warm, so there was a risk that medicines would be stored at too high a temperature. The room was still too hot so the owners need to resolve the matter as a priority and inform us when the work has been done within the timescale set out. Medicines requiring refrigeration are kept in a dedicated fridge. We looked at the medication records for two people. On the whole it was properly recorded but there were one or two issues to be addressed. The home was on the whole clean and well maintained, though there was one area where there was a strong, unpleasant odour. The acting manager said that this was due to a carpet that could not be effectively cleaned; this needs to be replaced by a surface that better meets the needs of the person in the room, such as laminate. The conservatory is used as a smoking room but does not have an extractor fan. The furniture in this room is rather old and scruffy and needs to be replaced. The perspex roof has been painted to stop the room from getting too hot. This is very unattractive indeed and the owner should look at the possibility of a more homely solution, such as roof blinds. The furniture in the main lounge has recently been replaced and is very comfortable and attractive. A former shower room is being used as a store for broken wheelchairs but it is now full to overflowing and the chairs should be removed. The care plans we looked at were very detailed. Daily write ups were rather brief, however, and could be improved by linking them more closely to the care plan itself. Also, when skin care issues have been identified these should be an a separate care plan to make sure that staff are aware of the issues and carry out the required action.
