What the care home does well Assessments of peoples needs are undertaken and include details of risks, to ensure people`s needs can be met. People receive care in a way that respects their privacy and dignity. A health care professional said " St Catherine` offer high quality support and emotional health, and provide a friendly service" A robust complaint`s procedure is in place to ensure that any concerns raised are investigated and dealt with thoroughly. Training for staff is provided to ensure that care is given by people who have relevant knowledge of how to give care safely.Audits on how the home runs are carried out by the manager and other staff, to ensure the quality of the service is monitored and any shortfalls identified are dealt with thoroughly. What has improved since the last inspection? Environmental improvements are being made, and more special equipment has been purchased to improve the care and facilities available to people who live there. Staff have received further training in relation to safeguarding people from abuse, this helps to ensure they are protected. What the care home could do better: People`s personal preferences relating to bathing should be provided. Date of last inspection Brief Description of the Service: St. Catherine’s Care Home is run by Wellburn Care Homes Limited. The home is located in the village of Shipton By Beningborough. There are local amenities and a bus link into York. The home provides care for up to 55 older people with general nursing and dementia needs. At present there are two distinct units, Harewood and Lomond. The Lomond unit provides accommodation for people with dementia. There is a car park provided in the grounds of the home. Information is provided in the service user guide and statement of purpose. Inspection reports are also available for people to read so that they have relevant information about what the home can offer them. Fees charged on the day of the site visit included people being funded at social services rates, up to £775.00 for a suite comprising of a bedroom, separate lounge area, bathroom and small patio. This is an overview of what the inspector found during the inspection. The quality rating for this service is 2 star. This means the people who use this service experience good quality outcomes.
The accumulated evidence used in this report has included: • A review of the information held on the home’s file since its last key inspection. • Information submitted by the registered provider in the Annual Quality Assurance Assessment. • Surveys received from seven people living at the home, four relatives, and one health care professional. • An unannounced visit to the home, which lasted five hours fifty minutes undertaken by one inspector, and one expert by experience who attended for four hours forty five minutes. This included a full tour of the premises. • Evidence was gained by direct observation during the site visit; which involved talking with people living at the home, visitors, the manager and operations manager and other members of staff. Inspection of records, including care profiles, medication administration records, staff files and some of the home’s policies and procedures. • What the service does well:

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. People are assessed prior to being accepted into the home so that their needs are known and can be met. We have made this judgement using a range of evidence, including a visit to this service. EVIDENCE: Pre-admission assessments are undertaken by experienced staff who find out what individual needs people have, including their social and mental health needs. Information from care manager and discharging hospitals is also gained to ensure the home knows peoples full needs. Admissions are not made to the home if their needs cannot be met. Information is available to people in the statement of purpose, service user guide and last inspection report. This ensures that people are well informed about what the home has to offer them so they can make an informed choice.

They can have lunch or stay for the day or a trial period. Staff are available to answer their questions and they can visit as many times as they like. Surveys received indicated that people felt they receive enough information about the home to make an informed choice. Intermediate care is not provided. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 7, 8, 9 and 10 People who use this service experience good quality outcomes in this area. People’s health care needs are met and medication policies and procedure’s protect people living in the home. We have made this judgement using a range of evidence, including a visit to this service. EVIDENCE: People who use the service have care plans and risk assessments in place these are reviewed monthly or as the persons needs change. People with special needs relating to their mobility, nutrition and mental health have individual care plans and risk assessments in place to ensure their health and safety is maintained. Health care professionals are asked for help and advice to enhance the care that people receive. The expert by experience talked with two people who said that they would like to have a bath or shower more often. Staff should find out what individual peoples bathing needs are and ensure these needs are met.

The manager holds care reviews with the resident and their family every 6 months. The reviews are documented and signed to ensure that all parties are fully informed and agree with the package of care the home is providing. New care plans being introduced focus on people’s strengths and how staff can support them to achieve their goals. Specialist equipment is available to ensure that peoples individual and special health care needs are met. The company have just bought five new airflow mattresses and will supply profiling beds for people if they are assessed as being required. People follow their chosen routines. Staff on Lomond unit help and support people with memory problems and dementia in a sensitive way, to promote their dignity. Medication systems inspected on both units protect people living in the home. At present no one is able to self medicate however the manager stated that people would be assessed and helped to self medicate if they were capable and safe to do so. A comment received on a relatives survey stated, “My relative has been very well looked after since their admission to St Catherine’s” Another commented “I am always updated with changes in medication”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12, 13, 14 and 15 People who use this service experience good quality outcomes in this area. People’s social needs are known by staff and are being met, a nutritious diet is provided. We have made this judgement using a range of evidence, including a visit to this service. EVIDENCE: Staff know each individual well and activities are advertised on a white board in Harewood. Activities are not advertised on Lomond, because people have different social needs and benefit more from staff talking with them about reminiscence or just being there to hold their hand or comfort them. A variety of activities are available including visits from “ Pat the dog”. Singer’s and entertainers visiting the home to perform. Film presentation’s, bingo and quizzes. Gentle exercises are also provided. A hairdresser attends the home twice a week as well as a chiropodist as required. The expert by experience talked with lots of people on both units and they indicated that they could join in with activities if they wished to or relax in the lounges or their bedrooms.
St Catherine`s Care Home DS0000070670.V353497.R01.S.doc Version 5.2 Page 13 On Lomond there is a Snoozelam room, which has a comfortable massage chair, music, fibre optic lights, larva lamps, even small lights in the carpet. This is to aid relaxation and sensory stimulation. Local schools are involved with the home. Children come and play in the homes garden and attend the Strawberry tea as well as visiting at Easter. Local clergy visit the home and carry out a service for residents monthly. They also visit as requested. Local village newsletters are provided at reception so people that can be involved in the community if they wish. Open visiting occurs. People can go out with their family and friends, this helps to promote their independence. The kitchen was inspected, the systems for preparing food are satisfactory and breakfast was enjoyed by people in their bedrooms or in the dining areas. One person said, “I have had a sausage sandwich for breakfast and the food is good”. Another comment was “I have a choice of food and enjoy the meals I have” Mealtimes are a social occasion. On Lomond people could eat where they wished. On both units people were helped and supported to eat where necessary by patient staff in an unhurried manner. On Lomond the expert by experience had lunch with the residents, and said people were not told before lunch arrived what the choice of food was. The menu was displayed on a white board in both units. The choices available should be discussed with people to let them think about what they might like to eat. This was discussed with management who said that menus were being printed to place on all tables so people are able to read what is available to them. The chef knew who required special diets and finger foods and high calorie snacks are available to people who require food supplements. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 16 and 18 People who use this service experience good quality outcomes in this area. Concerns and complaints raised are handled appropriately to ensure that people are protected from abuse. We have made this judgement using a range of evidence, including a visit to this service. EVIDENCE: The complaints procedure is displayed in the entrance hall and all bedrooms. Complaints are investigated and the person informed of the outcome. The manager operates an open door policy; anyone one can see her to make their views known at any time. People surveyed said they knew how to make a complaint and issues raised would be dealt with. A safeguarding policy is in place, there have been two safeguarding issues raised, management worked with the local authority to resolve the issues. Staff spoken with knew what to do if an allegation of abuse occurred. Ongoing training and information relating to the whistle blowing procedure has been given to all staff and issues are brought to management’s attention and acted upon appropriately. This ensures that people are being protected from abuse. Checks are undertaken to ensure new staff are suitable to work in the care industry. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19 and 26 People who use this service experience good quality outcomes in this area. People live in a home that is well maintained and infection control measures are in place. We have made this judgement using a range of evidence, including a visit to this service. EVIDENCE: The home is maintained and decorated with plans to make the environment more homely with soft furnishings and different lighting in certain areas. One upstairs bedroom is being made bigger and having an en suite bathroom created. Plans are in place to replace some chairs and to repaint and decorate corridors to give the home a new look, and aid people’s orientation within the home. All of the bedrooms have room names. On Lomond they may also benefit from having other indicators such as large room numbers or photographs of loved

One bedroom on Lomond has no nurse call in the bathroom, however the nurse call in the bedroom is detachable from the wall and can be taken into it. Bedroom doors are fitted with locks but are rarely used. A number of bedrooms had en suite facilities that allow wheelchair access. There are four double bedrooms which have screens between beds to ensure peoples privacy and dignity is maintained. There are two bedrooms on Harewood, which have living rooms and access onto their own private patio and en suite toilet facilities. Bathrooms are fitted with equipment to aid peoples independence where required. Reception is being moved so visitors can be met by the administration team or manager. A new doorbell is being fitted which will be compatible with the nurse call system to alert staff when visitors arrive at the new entrance. There are gardens to the front side and back of the home, and a central secure courtyard garden for Lomond unit with patio doors leading from the lounge so people can go outside if they wish. The grounds are suitable for disabled access. The laundry facilities inspected are adequate for the home and soiled linen is contained in red dissolvable bags. Industrial washers and dryers are in place, with sluice cycles. The laundry is a separate building in the grounds and a lone working policy is in place, and a walkie-talkie is available for staff so they do not feel isolated. Hand wash facilities are available throughout the home, alcohol gel is also available to clean hands with and these measures ensure infection control is in place. People surveyed said they were happy with the facilities the home had to offer. One comment was “ St Catherine’s is going through a process of refurbishment at this present time. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27, 28, 29 and 30 People who use this service experience good quality outcomes in this area. People are looked after by adequate numbers of well-trained staff. We have made this judgement using a range of evidence, including a visit to this service. EVIDENCE: Staff spoken with were friendly approachable and positive about the role they played in the home. Staffing level are adequate to ensure people get the care they require. Staff update their training to ensure their health and safety is protected and that of the people living in the home. Staff are recruited using thorough methods. However Criminal Record Bureau checks for 2 members of staff had not been received at the home and these staff were being supervised at all times. Management should ensure that this information is sought as soon as it is returned to the company who completes these for the home. Staff meet the needs of people living in the home, people spoken with by the expert by experience stated staff were very caring. Staff were seen to comfort people put their arms around them and reassure them. Agency staff are used when sickness or absence occurs, sometimes if this happens at short notice and agency staff cannot be found then there can be

There has been an increase in staffing levels since Wellburn took over to improve the quality of care being delivered. Staff who require extra help and support to be effective when recruited are given an individual support plan. Induction for new staff occurs and includes information on safeguarding individuals. The home has over 50 of care staff that have achieved a National Vocational Qualification at level 2 or 3. This helps to ensure that people are looked after by well-trained staff. Staff meetings are held to gain the views of people working in the home. Any issues raised are looked at by management and appropriate action taken. Staff spoken with were positive, saying they felt the company put the residents at the centre of what they do and they help and support staff. People live in a home that is well managed and their health and safety is protected. We have made this judgement using a range of evidence, including a visit to this service. EVIDENCE: The manager has been at the home for a long time, she is experienced and approachable. The company help and support her; the operations manager and directors visit the home regularly. Quality assurance procedures are in place. Audits are carried out, these look at all departments as well as asking the views of people living at the home. The management team deals with any issues raised.

The company also send out surveys to people living in the home to gain their views of the service. The home is continually being improved to ensure it is a safe and homely environment for people and their families to enjoy. The management team communicate well with the Commission for Social Care Inspection. Health and safety checks and regular maintenance is undertaken.
