Sussex Grange is a privately owned care home registered to accommodate up to twenty-four persons in the category of older persons. It is a detached two storey building situated in the village of Selsey, West Sussex. It is within easy reach of Selsey village and all its amenities and facilities. The sea front is within a short walking distance. There are well-maintained gardens to the front, side and rear of the property. Annual Service Review Page 3 of 8 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Information we have about how the service has managed complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection and the results of any other visits that we have made to the service in the last 12 months. Relevant information from other organisations. What other people have told us about the service. What has this told us about the service? The home returned the annual quality assurance assessment (AQAA) when we asked for it. The AQAA provided us with clear and comprehensive information. It provided us with details of what the home does well, what they could do better and plans for improvement within the next twelve months. Prospective residents are given detailed information about Sussex Grange, and are welcome to visit the home, on several occasions if necessary to ensure that the home is suitable for them. Pre-admission assessments are always carried out to ensure that the staff have the knowledge and that the environment is suitable to the prospective residents needs. Ten questionnaires returned to CQC from residents at Sussex Grange all state that they were given good information about the home prior to deciding to move in and that they have all been given written information about the homes terms and conditions. Information gained at pre-admission assessment is used as a basis for each individual residents support plan. The support plans are reviewed on a monthly basis and rewritten every 3 to 6 months or more often if the health and care needs of the resident changes. Each resident is risk assessed according to their capabilities and lifestyle. The home has introduced a nutritional screening programme in the last twelve months, which has resulted in improved nutrition for some new residents. Where residents wish to continue with self administration of medication a standard assessment process is used to check their capability, and this is reviewed on a regular basis. From ten resident questionnaires returned to CQC seven residents said that they always received the care and support they need, three residents said that they usually received the care and support they need. Nine residents said they always receive the Annual Service Review Page 4 of 8 medical care they need, and one questionnaire was not completed on this question. The registered manager and the staff have good working relationships with general practitioners and other external health care professionals. All staff working in the home respect the residents rights to independence, privacy and dignity. Residents requests and wishes in regard to end of life care are recorded when they come to live in the home and are carried out should they become terminally ill. At Sussex Grange residents are offered a wide range of activities that they are able to choose to take part in, either in the community or within the home itself. The home publishes a monthly programme of activities to give residents advanced information on what is on offer in the way of activities. An additional facility for residents who wish to access the community is the Selsey Venture Bus, which is a charitable organisation and enables residents to visit the library, various clubs and to attend church services. From one resident s wish to grow his own salad produce more residents have joined in and the home now has its own gardening club that has won a special award. Residents are able to follow their choice of religion and are enabled by the home to do this. Relatives and friends are made welcome in the home at any time. All meals provided in the home offer choice, the use of fresh ingredients and are nutritious. Special diets, likes and dislikes are catered for. From ten resident questionnaires returned to CQC six residents said that there are usually activities they can take part in and four residents said that there are always activities they can take part in. Five residents said they always enjoy the meals in the home and five said they usually enjoy the meals in the home. The home has a simple complaints policy and procedure, that is included in the service user guide and displayed in the home. The AQAA states that there have been no complaints in the last twelve months. Both the registered manager and his staff listen to the residents comments and suggestions on a daily basis and this enables the home to meet the resident needs and reduce the likelihood of a formal complaint being made. The home has up to date policies and procedures including the West Sussex Multi Agency Guidelines on Protection of Vulnerable Adults. Other policies and procedures include, Use of Restrain, Dealing with Aggressive Behaviour and Whistle Blowing. Staff are encouraged to voice any concerns they may have. All staff have received safeguarding vulnerable adults training. There have been no safeguarding vulnerable adults referrals since the last key inspection. From ten resident questionnaires returned to CQC nine residents said that they know how to make a complaint and that they know of someone they can talk to informally if they are not happy, only one resident was not aware of how to make a complaint or if there was someone available they could talk to informally. Sussex Grange is a Tudor style house, but through thoughtful design has many modern features to ensure the comfort and safety of the residents. The home is kept clean and tidy at all times. The home comprises of a large lounge, dining room, sixteen single bedrooms and four double bedrooms which at the present time have single occupancy, there is a lift which provides access between the ground and first floor. All bedrooms have en suite facilities that include a bath. The home has appliances fitted that benefit the safety and well being of the residents. The well maintained gardens surround the home. During the next twelve months there will be an on-going maintenance programme and some older leaded windows will be replaced with double glazed units. The AQAA states that the home is kept clean and tidy at all times, but there is no evidence as to how the risk of cross infection is kept to a minimum. Ten residents Annual Service Review Page 5 of 8 questionnaires returned to CQC say that the home is always kept fresh and clean. At the present time the staffing levels in the home are above average enabling staff to spend quality time with the residents. The staff levels and skill mix on each shift are appropriate to meet the assessed needs of the residents. The home employs sufficient ancillary workers to carry out cooking, cleaning, laundry and maintenance. Seventy three percent of care staff have obtained their NVQ level 2 or above in Social Care. The home received two awards at the Care Training Consortium Accolades this year. Sussex Grange prides itself on the retention of good experienced staff. Recruitment procedures in the home are robust ensuring that all new staff are appropriately CRB and POVA checked and have two written references prior to taking up employment in the home and this helps to keep residents safe from abuse. All care staff have received and are kept updated on mandatory training as well as taking part in job related skills training. All new staff are expect to complete an initial induction and Skills for Care related induction. From ten resident questionnaires returned to CQC four residents said that there are always staff available when they need them, and six residents said their are usually staff available when they need them. The registered manager of the home Dr Tom Jameson has managed the home for more than six years, he has obtained his Registered Managers Award and continues to regularly attend training days and West Sussex Managers meetings as part of his continuing professional development. Staff and residents are happy, content and relaxed within the home. Several quality audit systems are in place and this includes resident, staff and management meetings, with minutes being recorded. Minutes of meetings show that actions are taken where comments are made. Regular resident surveys are sent out and the views, opinions and choices of the residents is set as high priority for providing a good quality of care in Sussex Grange. Sussex Grange has procedures in place to safeguard the financial interest of the residents. All staff receive regular supervision and appraisal this ensure that policies and procedures are understood and training needs are identified. The registered manager is fully aware of the need to maintain a healthy and safe environment within the home, to ensure that residents live and staff work in a safe and secure home. Fire and health and safety risk assessments are in place. Appliances used in the home have up to date maintenance certificates and regular checks are carried out and recorded for fire points, emergency lighting, hot water temperatures. Any incident or accident is recorded in the accident book, which is compliant with the Data Protection Act 1998. Ten resident questionnaires returned to CQC eight with additional comments as follows The home looks after my needs well. Everything is perfect and dandy. Never need to complain about anything all the staff are kind and helpful. I enjoy summer outings. Plenty of quizzes and entertainments and both Tome Jameson and his wife are very kind. For me a vegetarian, perhaps more variety of vegetables and cereals. The home provides a caring and welcoming environment. Communication between staff Annual Service Review Page 6 of 8 members and relatives is generally good but we feel there is room for improvement. I am well satisfied. It is first class and would take some beating. Excellent home very caring owners, and staff are absolutely first class. Happy with everything that the home does. What are we going to do as a result of this annual service review? There will be no change to the Inspection plan and we will do a Key Inspection by the 19th January 2011. However, we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
