What the care home does well The home has clear records in place regarding the service users` weekly allowances and how they are paid and transacted. Service users are confident that they can access their money when they need it and are happy that they are encouraged to be independent with their finances. Service users are safely supported to deal with their finances independently and good procedures are followed if the home feels that more support is needed from outside agencies. Service users live in a safe and secure environment. What the care home could do better: Service users should have clear information about the fees charged by the home and the amount of money they are responsible for paying. This should be part of the contract that informs them of the home`s terms and conditions. This should be in a format that individual service users understand. So that service users have a secure facility to keep their money and other valuables safe a locked facility should be provided by the home. The registered provider must ensure that the electricty work needed in the home is carried out on the 19th January as planned and a copy of the certificate is forwarded to CQC. This is so service users live in an environment that is safe and that complies with legislation. The home is located on the edge of the small village of Trimdon Grange, close to shops, pubs and other amenities. There are local bus services which reach a number of larger towns in the area. There are thirteen single and two double bedrooms. There are no en-suite facilities but adequate bathing and toileting facilities are provided. There is a spacious lounge and a dining room, both of which overlook the front garden and main street, which are easily accessible. The weekly fee charged at the home is £421.76. Care Homes for Adults (18-65 years) Page 2 of 9 What we found: The aim of this inspection was to confirm that, 1 Service users finances are appropriately recorded and transacted. 2 That the health and safety of service users is safeguarded with paticular attention to whether the home complies with the Electricity Regulations 1989. Clear records are kept in the home regarding financial transactions related to service users money. The amount of money needed to pay fees to the home is paid directly from DSS and Social Services into the proprietors bank account. Service users collect their weekly allowance from the office and keep and use it independently. This is recorded clearly and appropriately and for those who wish or need to have their money kept by the home clear records and receipts confirm transactions are dealt with transparently. It was noted however that not all service users have a locked facility in their personal rooms to keep their money safe. This was discussed with the manager who is to address this issue directly. An audit of the records kept regarding service users weekly allowances was carried out by Durham Social Services in December 2009. This had a positive outcome. All but two service users who receive Disability Living Allowance (DLA) have this paid directly into individual post office accounts and have individual bank cards to independently withdraw their allowance from the post office. The remaining two have their DLA paid directly into the proprietors account who pays this to them in cash with their weekly allowance. This arrangement was inherited by the proprietor when he first took over the home. Records confirm this payment. The manager is to discuss this issue with the service users and care managers with the aim to change this arrangement and to give the service user more autonomy. Records in service users financial files demonstrate how the fees charged by the home are to be paid. Financial assessments are carried out by the contracting agency for everyone living at the home. These show that all but three have their fees fully paid by a combination of DSS benefits and contributions by the contracting agency. Three service users contribute a proportion of their fees from their personal finances. Two of the three service users who personally contribute to their fees pay this to the proprietor in cash. Clear records are kept of such transactions. The third person pays their personal contribution directly into the proprietors bank account. The service users bank statements for the past two months confirmed this. There were no discrepencies found in relation to fees charged and the money paid. However the personal amounts paid by service users to the proprietor are not recorded in the home. This was discussed with the manager who agreed that this should be recorded and would address it directly. The homes Service User Guide includes information about the homes terms and conditions but does not include the range of fees charged by the home. Although individual records clearly demonstrate how fees are paid service users have not been Care Homes for Adults (18-65 years) Page 3 of 9 issued with individual contracts that tell them the fees charged and the breakdown of how these are to be paid. Each service user has the right to know this and to be given the opportunity of agreeing with this, therefore should be given the opportunity to read, agree with and sign an individual contract. The home demonstrates their understanding of safeguarding adults and has demonstrated good practice regarding the assistance given to service users to safeguard their money. Recently the manager has discussed concerns regarding the possible inappropriate use of one service users finances with Durham Finance Safeguarding Team and a protection order has been organised for another person who has recently inherited a some of money. Health and Safety issues were looked at and in particular to ensure that the registered person complied with the Electricity at Work Regulations 1989. PAT testing records are in place regarding each electrical item used in the home. The last tests completed are dated April 2009 and all of these items passed and are therefore safe to use. A Minor Electrical Installation Works Certificate is in place for work carried out in January 2007 when new electric sockets were installed in one bedroom. As there is no confirmation that the home complies with the Electrics at Work Regulations 1989, arrangements were made on the day of the inspection to have an inspection carried out by a registered electrical contractor. This was completed 14/01/10 following this inspection and arrangements were made to carry out identified work needed before a compliance certificate is awarded. This work is to take place 19/01/10 and the manager has agreed to fax the certifcate to CQC directly. Records show that an inspection was carried out by the Fire Service in July 2009. As a result of a visit from Environmental Health 12/10/09 several health and safety issues found throughout the home were raised and requirements made. The result of how the home addressed these issues was evident in a report following a second visit 15/12/09. Comments confirmed that the new manager had competently addressed all of the issues raised. The manager is currently addressing the new Health and Safety Regulations to ensure that the home fully complies.
