None The Grange Nursing Home is a privately owned service and is registered as a care home with nursing for up to 49 older people; registered nurses are on duty 24 hours a day. The property is a mature country house set in the centre of Stanford in the Vale; it has two lounges and a separate dining room on the ground floor. Bedrooms are situated over two floors, some bedrooms have en-suite facilities; there are five bedrooms that are large enough to be shared by two people. The site has ample car parking spaces and there are pleasant landscaped gardens for use by residents and visitors. The home has strong links with the community; the village is compact and includes shops, a public house and post office. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information we have received, or asked for, since the last key inspection. This included the annual quality assurance assessment (AQAA) that was sent to us by the service in October 2009. The AQAA is a self assessment that focuses on how well people who live here feel their care and support needs are met. It also gave us some numerical information about the service, for example, the number of staff trained to the nationally recognised standard - National Vocational Qualification in Care or Health and Social Care at Level 2 or above. We have looked at what the service has told us about things that have happened in the home in the past 12 months - these are called notifications that the home has to report to us by law. The previous report and AQAA for the home were reviewed, and AQAA record of any complaints they have received and how these were dealt with. We took into account the comments we received in the sample of our questionnaires (surveys) that we sent to residents, their families, visiting health and social care professionals and the homes staff. In December we received 3 completed surveys from residents, 6 from staff, 3 from health and social care professionals, and 3 from relatives, carers and advocates. We phoned the manager to let him know the kinds of things people had put in their surveys, and discussed with him how the home and provider organisation, Forest Healthcare Ltd, gets peoples views and acts on them. What has this told us about the service? The home sent us their AQAA when we asked for it. It was clearly written and gave us a satisfactory amount of detailed information we required. It showed us how the home continues to develop and meet the individual needs of people who live here. The AQAA shows that the managers have identified ways in which some things could be improved and how they plan to do this in the year ahead. Survey comments received were largely very positive about the home - the staff, environment and social life. Some of the comments made were - The entertainment and the cleaning of the building throughout is perfect Attitude of the staff is excellent. Physical environment is excellent. Good range of activities and entertainment. Good food. Excellent support in relation to hospital appointments - escorts and ambulance transport arranged by The Grange. Staff feedback was also very positive about the standard of care and support provided and their working conditions - Cares for residents and staff welfare. Family atmosphere. Relatives treated with respect in all circumstances. All necessary equipment is supplied and replaced as necessary. Very good care team with back up from managers at all times. Provides training and supports the staff. the home is well managed by the hardworking manager and competent staff. Annual Service Review Page 4 of 6 The manager and AQAA tell us that there are regular staff meetings, a low staff turnover and an increasing proportion of care staff who have achieved the NVQ Level 2 qualification,or above (more than 50 per cent, as reported in the AQAA completed in October 2009). The company has its own established system for quality assurance and getting the views of the people who live here. The information available to the Commission indicates that the service continues to provide excellent outcomes for residents and their advocates and that the home is run in the best interest of the people living here. Previous inspections and AQAAs indicate that residents and relatives have confidence in the homes complaints procedure. The home stated in its AQAA that since our last inspection it had addressed 2 complaints that were upheld and resolved satisfactorily; and one safeguarding adults referral was made. The Commission has not received any complaints about the service. The Commission is not aware of any current Local Authority safeguarding adults referrals. The homes AQAA tells us that they have improved in several ways in the past year. for example, in work done to develop care of peoples continence needs, end of life care, and emotional and psychological care of people living at The Grange. The home has employed a maintenance person, rather than use outside contractors. The homes managers continue to work with the provider organisation, Forest Health Care, to develop new managment systems so that they can improve and demonstrate value for money services for the people living here, and their families. The home has a new computer system that will improve the way in which staff document the care and support they provide. There is a significant programme of improvements and alterations to the building planned that will provide better communal and recreational space for residents, and choices about where they spend their day. A new kitchen and laundry facility is planned. Conversion of the few shared rooms to single en-suite rooms, with an aim to provide all single room accommodation in the home, will offer improved dignity and privacy for residents. What are we going to do as a result of this annual service review? We are not going to change our inspection plan and may do a key inspection by 31/12/2012. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
