The Haven is a registered care home for older people, which provide nursing care for up to forty-eight residents. An additional variation also allows the home to provide care to residents under the age of 60 years with a physical disability. The home is situated at Telscombe Cliffs, approximately half a mile from the sea front and the local shops, has space for car parking and is served by public transport. Previously a family home, it has been converted to provide accommodation for residents on two floors consisting of thirty-one single rooms and five double rooms, communal accommodation comprises two lounges and two dining rooms and assisted bathing facilities. All areas of the home are accessed by a shaft lift and there are gardens to the rear of the property, which are accessible to residents. What the care home does well The Haven provides a homely and comfortable environment for residents. A number were sitting in the garden or taking part in a tennis match, and strawberries and cream was offered to all residents as part of the Wimbledon Day. Communication between residents, visitors and staff was relaxed and friendly. Visitors said that they are `always made very welcome` and some were joining in the tennis. Residents involved in the inspection said that `the staff are very good`, there are a range of activities for them to join in if they wish, and that they are `very well looked after`. What the care home could do better: Staff were noted to be wearing gloves when transferring residents from the home to the garden and when moving between rooms in the home. The deputy manager said that all staff have attended training in the control of infection and should be aware of the correct use of gloves and aprons. A requirement has been made to ensure staff have attended infection control training and are aware of the appropriate use of gloves. Care Homes for Older People Page 3 of 9 What we found: The reader should be aware that the Care Standards Act 2000 and Care Homes Regulation Act 2001 often use the term service user to describe those living in care home settings. For the purpose of this report those living at The Haven will be referred to as residents. The inspection was undertaken over two and a half hours on 30th June 2010 by a Regulation Inspector. The inspection was undertaken to assess the specific standards medication and staffing. The inspection involved looking at the homes records, discussions with residents, visitors and staff, and observations on the day. The manager was not available and the deputy manager facilitated the inspection. The outcome of this visit is as follows:Health and Personal Care - Medication There are two medicine trolleys which are used to take medicines to people living in the home. These are kept in the clinical rooms, one on each floor, which also have fridges for medicines that need to be kept at a particular temperature, and separate secure storage for controlled drugs. The inspector was unable to observe medicines being given out, therefore the medicine administration record (MAR) charts were examined, and the homes policies and procedures for medication were discussed with one of the nurses. The MAR charts were noted to be completed appropriately and the nurse was able to demonstrate an understanding of the policies for the administration of medicines. One resident self administers some of her medicines, and a risk assessment had been completed to ensure that this is a safe practice. However when looking at the homes policies for self administration it was noted that the GPs consent form was not in place. The deputy manager dealt with this during the inspection, therefore a requirement has not been made at this time. Staffing - staff complement The deputy manager advised that there are sufficient staff working in the home to meet the needs of the people living there. Staff rotas were examined and it was clear that the number of staff on duty in the morning is considerably more that in the afternoon and evening, although there is an additional staff member to cover the twilight shift between 5 and 10pm. It was also noted from the rota that at weekends and some shifts during the week some staff were absent, and there was no evidence that additional staff had been covering for them. Staff involved in the inspection said that this often happens at weekends, this puts a strain on the staff working these shifts, and can affect the care they provide. Care Homes for Older People Page 4 of 9 Residents involved in the inspection said that the staff are very good, they look after us well and some said that there are enough staff to look after them, while others said they sometimes have to wait a long time for their call bell to be answered. Visitors also said the staff are very good and provide the support and care their relatives need. However they also said that it would be better if there were more staff, particularly at weekends. A requirement has been made to ensure there are sufficient staff on all shifts, including weekends. Staffing - Recruitment Four staff files were viewed and found to contain the relevant information including application forms, interview details, references and Criminal Record Bureau CRB checks. This reflected a robust recruitment procedure. Staffing - Training Records are kept of the training staff have attended and certificates are provided and kept in their personnel files. Mandatory training including fire training, infection control and moving and handling is provided, as well as specific training which relates to the needs of people living in the home. The deputy manager confirmed that all new staff are required to complete induction training and staff are encouraged to work towards National Vocational Qualifications NVQs, when they have completed this.
