What the care home does well There is an established staff team at the home who are keen for high standards to be maintained so the people living at the home receive good quality care. Residents` plans of care and individual case notes are well documented so staff know what they should do to meet people`s needs. Day to day supervision of staff was good with the manager working alongside the care staff regularly so they had guidance and support. Regular staff meetings were taking place so staff could have a say in how the home was being run. The home is clean and well maintained so people live in comfortable, homely surroundings. What the care home could do better: Regular formal supervision should be held with staff so that they have the opportunity to discuss their work and training needs. Regular residents` meetings should be held so that people can have a say in how the home is being run and to ensure that continues to be run in their best interests. It is a privately owned family run business. The home is in a rural setting in the Cheshire village of Malpas, situated close to a small range of local shops and other village facilities and amenities. The Old Hall is a two-storey building and people are Care Homes for Older People Page 2 of 9 accommodated on both floors. Access between floors is via a stair lift or the stairs. Accommodation consists of ten single and four double bedrooms; all but one have ensuite toilet and bathroom facilities. There are currently fourteen people living at the home so each has a room to themselves. There is a separate lounge, dining area and conservatory that overlooks the garden. The Old Hall has gardens to the rear with steps leading down to the lower levels. Handrails are provided and the top level is fully accessible to people who live at the home. Car park spaces are available to the front of the property. The fees at The Old Hall are between 367.00 and 450.00 pounds per week. Optional extras include hairdressing, chiropody and newspapers. The quality rating for this service is 2 stars. This means that people who use this service experience good outcomes. We (the commission) carried out an unannounced visit to The Old Hall on 10 January 2009 that lasted three hours. The purpose of our visit was to check that the good outcomes for people who live in the home were continuing to be met. During our visit we looked at various records to see how things were running and we talked with a number of people who live in the home and the staff to find out what they think about it. We also spoke with the manager and one of the owners who gave us their views and further information. Choice of Home The statement of purpose and service users guide we saw had been kept up to date and were reviewed in April 2008. Copies were available in the hallway in bound folders so that people in the home, visitors and people who might be interested in moving in could get information about the home. Health and Personal Care We looked at care plans of four people who had moved into the home not long before our visit. They all contained the information required so that the staff could support these people appropriately. Assessments had been done before they moved in to find out what peoples needs were before they moved into the home. Care plans, health assessments and a range of risk assessments had been completed for each person to guide staff on how to provide care for them. They had been reviewed monthly to keep them up to date to make sure that the care being provided for each person continued to be effective and to meet their needs. Other care plans we looked at had been kept up to date and reviewed regularly. Staffing Eight out of twelve staff working at the home have obtained a National Vocational Qualification at level 2 in care and this is above the recommended fifty percent of staff to be qualified with this award. These are nationally recognised qualifications for people working in the field of care and show that they have the basic skills and knowledge to provide appropriate care for people. All staff had undertaken training on safeguarding adults so they would know the signs of abuse and what to do to make sure that people living in the home are protected from harm. Staff had also done fire awareness training so they know what to do to keep people safe if a fire broke out at the home. We checked the formal supervision system of staff working in the home and found that the last sessions had been done in May 2008. The manager explained that the process had been interrupted by staff sickness which had meant that she had been working Care Homes for Older People Page 4 of 9 alongside staff more than usual and also had been cooking for the home when the cook was off. She told us that the formal supervisions would start again and each member of staff would have a formal recorded supervision by the end of February 2009. The chance the manager had to work alongside staff was valuable in helping her observe how they did their work but formal supervision enables staff to have the opportunity to discuss their work and future training needs, so should be held regularly for their guidance and support. Staff meetings had been held in March and October and the manager told us that another one was due to be held within the next few weeks. A residents meeting had been held in March 2008 and the manager told us that more regular meetings would be held. She said she would organise one within the few weeks following our visit and would then hold them regularly. Management of the home The co-owner of the home who was present at the home during our visit told us that the improvements he was planning for over the next twelve months included a hearing loop system for the lounge area and a mobile loop system which can be used in any room; upgrading the fire alarm system and providing automatic door releases for the bedrooms and other key areas; emergency lights for all communal areas as well as the existing emergency lighting provided; and new call system which will include pressure pads and door sensors. He expects this to be completed over the twelve months following our visit. These will significantly improve the general building and update the current system in place.
