The Old Vicarage is situated in the quiet country village of Hornby. Hornby is a small hamlet approximately 9 miles north of Lancaster. The Old Vicarage is situated just off the main road, which runs through the village and is accessible to local shops, pubs, churches and general amenities. The home is an impressive Victorian-style building, which previously provided accommodation to the Vicar of the next-door church. The home has a small extension built on the side and provides accommodation for four people. The home is a two storey building with accommodation on both the ground and first floors. There are bathrooms and toilets accessible on both floors. Most rooms are single (some with ensuite facilities) and two shared rooms. There is a separate dining room and lounge, and the addition of a conservatory, which leads onto the gardens, has provided an additional facility for people to use. The walled garden is well maintained and provides an excellent outdoor facility for people to use. The home is very well maintained and provides a pleasant and homely environment for those people accommodated. The current range of fees are from #370.00 to #417.00 per week. What the care home does well People`s care plans were found to be created following a comprehensive assessment and were found to provide the basis for the care to be delivered by the staff team. Information held within the plans demonstrated that people have access to and are supported to access health care services to meet assessed needs. Discussions with people at the home indicated that they are treated with respect and their right to privacy is upheld. The service has an appropriate procedure for dealing with complaints, the records held at the home show that the service has not received any formal complaints, but have dealt with minor concerns appropriately and in a timely fashion. Discussions with some of the staff demonstrated that they have a good understanding of how to protect people from abuse and neglect, and know what to do if anyone makes an allegation of abuse, or if abuse is suspected. What the care home could do better: Although the service has a written procedure for the safe handling of medication we found that the way medication is cared for on behalf of people needed to be strenghtened. We found that the medication was not being carried forward when new quantities of medication is brought into the home. This led to some minor discrepanices being found when the medication was checked. As a result of this, we left an Immediate Requirement form, requiring that the medication quantities were checked and correct balances recorded so as to promote and protect the best interests of the people living at the home. Although there are appropriate risk assessments and care plans in place for people, we found that these needed to be strengthened to ensure that people`s best interests were promoted and protected. For example, people who have on going health needs or medical problems must have appropriate risk assessments and care plans that give details of people`s needs, the risks associctaed with those needs and the work that needs to be undertaken by the staff (or others such as district nurses or healthcare staff) to ensure that those needs are met, moniotred and evaluated in a timely fashion. Once the registered provider receives this report, and has had a chance to read through it, we will have a discussion with them regarding these issues that were identified during our visit. The environment was clean and tidy, and there were sufficient numbers of staff working at the home to meet the needs of the people living there. The deputy manager assisted us with the visit, as the registered manager was not on duty. We found that on the whole standards had been maintained, however, we found that some work needs to be undertaken to strenghten the systems relating to medication, risk assessments and care planning.
