What the care home does well

Farmborough Court continues to provide an excellent range of intermediate care services for older people within one central building. When people were asked in satisfaction surveys what the service did well, some responded; "The service is caring and supportive." "They all do their jobs well and help people to get back home after being ill." "It is good to know that even when you are ill in hospital you can go somewhere to get better before returning home." People staying at the service have access to specialist staff who are based within the same building such as, occupational therapists and physiotherapists. These people work directly with the care staff and share their knowledge and expertise. There are also sufficient care staff, known as reablement staff, to help people with their mobility and daily living exercises, so that they can get fit enough to get back to thier own homes. The quality of the accommodation is very good. All areas are spacious, well decorated and furnished and all bedrooms have en-suite facilities. Cards of thanks are displayed around the building and these reflect service users, main carers and families appreciation of the service delivered. These were displayed in great number. Staffing levels are flexible to ensure that the number and needs of people staying here can be met and staff committment to delivering a good service remains staff who responded to the surveys said;"I think the staff team works very well together in helping the service users to gain their independence and confidence to return back to their homes." "We provide a good service to help service users reach their optimum level of independence." "We motivate service users to reach their goals by working with other agencies." "An excellent service is provided and it is an excellent place to work." "I feel that Farmborough Court does well in all areas and that the service we provide is very valuable in ensuring that people are at a level where they can go back to live in their own home independently or with a care package." The centre is managed by experienced, competent people, so that service users receive a very well run service. Service users` views are positively used to review the service, so that it is run in their best interests. The service is continually reviewing their goals, looking at new ideas and putting plans in place to develop the service further.

Date of last inspection Brief description of the care home Farmborough Court is a large modern, 2-storey purpose built care home, which provides a base for a range of intermediate care services for older people who need convalescence or a rehabilitative stay. The Social Services Department is registered as the Provider of the personal care and accommodation provided at Farmborough Court Care Centre. The intermediate service is provided in partnership with Sunderland s Teaching Primary Care Trust, the Mental Health NHS Trust and the City Hospitals NHS Trust. There are 4 distinct units within the building that provide up to 54 places for older people, some of whom may also have mental health and physical needs. Each unit contains up to 14 bedrooms each with en-suite, a lounge, a dining room, a bathroom and a shower room. There is wide, level access into the building and wide corridors throughout the building. There are 2 passenger lifts to serve the first floor. All areas of the building are accessible by people with mobility needs. There is a driveway and car park to the front of the building. A paved area to the rear of the building is used as a patio area for the people who use this service. Farmborough Court is sited in a housing estate on the northern boundaries of the City of Sunderland. It is very close to a small number of local shops, club, church and local bus routes. All the elements of this service are free of charge to service users. Care Homes for Older People Page 3 of 9 What we found: The aim of this inspection was to establish that; The excellent standards assessed at the previous key inspection carried out 22/05/07 have been maintained and that the way the service is managed provides service users with a safe and consistent service that is appropriate to their needs. The outcome: Service users continue to receive support and therapeutic treatment within; an intermediate treatment environment and a framework that continues to be well managed and that is guided in service users best interests. It was evident that the manager, staff and the multi disciplinary team, continue to work hard to maintain high standards throughout this service, resulting in service users receiving a service that meets their assessed needs. A review of the service is currently taking place to ensure that its aims and focus of development, are appropriate to the needs of the people it serves. The service has been proactive in encouraging and including service users, their carers and families in discussions about how best to develop the service and as a result of this the following developments are some of the improvements made: Service users support plans now include relatives comments sheets and families and carers are now encouraged to participate in service users meetings. This has resulted in more effective communication between the service, service users, their families and carers and has empowered service users to say what they feel about the service they receive and how they would like to see it develop. Links with the community police have strengthened and this has given the service more confidence regarding security and safety for its service users. All health and safety records are up to date and these include fire records, electric wiring certificates and PAT testing. The maintenance file confirms consistent safety checks througout the building. To improve communication regarding discharges from hospital, a designated link discharge nurse is now identified. This has improved the discharge process and ensured that important information is received about the individual being discharged. A pilot project is also in process and this enables patients to be discharged from hospital directly to their own home with the support of the services reablement staff team. This has the potential to extend the intermediate treatment service delivered within the Farmborough environment to the outside community. Due to demand 12 extra staff have been recruited for a temporary 6 month pilot scheme to enable; service users with a higher level of physical need and increased number of service users with a dementia type illness, to access the service. Care Homes for Older People Page 4 of 9 And so that staff develop appropriate skills a comprehensive induction programme has been developed for all new staff and in addition to this the in house training programme has also developed further. Links with the PCT Infection Control Team has been strengthened which has resulted in the following: additional training has been accessed and policies have been put in place to ensure that there are procedures put into practice regarding people being discharged from infected hospital wards. This has meant that no patient is discharged from hospital to residential homes until the infection has been cleared. The registered manager is currently on secondment until the beginning of July and alternative arrangements are in place to manage the home. This is being carried out competently with the registered manager staying in touch and now having some dedicated time to Farmborough. The service agreed to inform CQC of the return of the registered manager or of any changes to be made.
