What the care home does well

Provides good quality care to the people who live in the home. Staff listen to the people and provide appropriate care. Positive relationships have been developed with the local community.

What has improved since the last inspection?

The care hours have been reviewed and more staff are now on duty at weekends. The facilities for leisure activities have been increased both within the home and in the local community. New carpets have been fitted in all the lounges new chairs and curtains have been purchased and these improvements provide a more comfortable home for the people who live there.

There is a separate day centre within the complex. North Yorkshire County Council owns it and Marion Greenbank is the registered manager. The home is located on the outskirts of Pickering and is within walking distance of the shops, bank, post office and cafes. The home is purpose built. There are 44 single bedrooms and 3 double bedrooms that are available for two people wishing to share. The home has pleasant grounds that are available for use by service users in appropriate weather. A copy of the service users guide to the home is given to people wishing to be admitted to the home and a visit is made to the home before a decision is made to move into the home for a trial period. The fees payable on the day of the inspection were £329.50 per week. This key inspection has used information documented in previous inspection reports, survey forms, a site visit and discussions with people who use the service and staff. This unannounced inspection took place on the 27th of November 2007 commencing at 9:30 a.m. During this time the registered manager, a team manager, 3 resource workers, 2 catering staff and 10 people who use the service were spoken to, a number of records were inspected including service users assessments, care plans and health and safety records. A tour of the building was made. Survey forms were sent to 10 people who use the service and seven were returned, three were sent to care managers and one was returned and two were sent to GPs and both were returned. All people who use the service stated that they were satisfied with the service provided and that they received good quality care from the staff team. They were satisfied with their own accommodation and were very pleased with the meals and the service. One service user said If you want any help they are there to give it”. Another service user said If you ask in the office to get a doctors visit they always will be more than willing to help. A GP commented “Good set of senior managers. Very useful facility. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 People who use the service experience good quality outcomes in this area. The assessments prior to admission are comprehensive and provide informed decisions about moving into the home. We have made this judgment using a range of evidence, including a visit to the service. Standard 6 is not applicable to this service. EVIDENCE: All people considered for admission to the home have a full care management assessment and are only admitted if their needs can be met by the staff at the home. People who are not known to the staff are visited prior to admission for Whitby Road (5) - North Yorkshire County Council DS0000034278.V353509.R01.S.doc Version 5.2 Page 9 a trial period. A review is held six weeks after admission to ascertain if the placement is appropriate. Intermediate care is not provided. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 People who use the service experience adequate quality outcomes in this area. The staff have a good understanding of the support needs of people using this service. This is evident from the positive relationships that have been formed between staff and people living in the home and is confirmed by reading the care plans. Improvements to the record of controlled drugs is required. We have made his judgment using a range of evidence, including a visit to the service. EVIDENCE: All people living in the home have a comprehensive plan of care. This is developed from the care management assessment and the observation of the staff team. All files inspected showed that regular reviews had not been undertaken. All plans must be reviewed as necessary and at least once per month. Records show that referrals were made to health care professionals as necessary. A GP stated that “The home has a good set of senior manages. Very useful facility”. One person keeps her own medication in a locked cupboard in her bedroom. Medication is administered by staff that are appropriately trained. A monitored dosage system is used. All medication stored within the system and the records checked were in order. Controlled drugs are stored in the correct manner. Two of the three records for individuals checked were incorrect. The stock of these drugs did not correspond with the records. The senior managers on duty could not explain the discrepancies. All the people spoken to who live in the home considered that the staff were very good and kind to them. It was observed that staff treated the people they cared for with respect and dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15 People who use the service experience good quality outcomes in this area. Some social activities are arranged for people living in the home. Meals are nutritious and balanced and offer healthy and varied diets. We have made this judgment using a range of evidence, including a visit to the service. EVIDENCE: Activities available within the home include bingo, board games, cards and musical evenings. One person stated that she attends a domino drive in the community each week and goes out on two different days to local day centres. Comments from people living in the home included I like the musical evening on a Wednesday night I take part in bingo every Monday and Thursday, in which I enjoy. People spoken to said that the food was very good and there was always an alternative available. One person said “I have been here ten months and never had a bad meal yet”. Another person said The food is very good, I always enjoy my meals. There are satisfactory complaints and safeguarding policies and staff are aware of the action to take if a complaint or safeguarding issue comes to their attention. We have made this judgment using a range of evidence, including a visit to the service. EVIDENCE: There is a complaints policy and procedure. All people moving into the home receive a copy of the complaints procedure in their welcome pack. Copies of the complaints procedure are displayed in the home. Staff are aware of the action to take to safeguard the people living in the home. The standard of the environment within the home is good and provides a homely place in which to live. We have made this judgment using a range of evidence, including a visit to the service. EVIDENCE: The home has satisfactory furniture and fittings and is suitably decorated. New carpets, chairs and curtains have been purchased since the last inspection to improve the comfort of the home for the people who live in it. One person said “It’s a lovely place you can’t fault anything”. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30 People will use the service experience good quality outcomes in this area. The staff provide quality care to people living in the home. We have made this judgment using a range of evidence, including a visit to the service. EVIDENCE: The registered manager had reviewed the staffing hours and has increased the number of staff on duty at weekends. Staff receive the necessary training and 85 of resource workers have obtained NVQ level 2 in care. Staff spoken to had also attended induction training, dementia awareness, back care and fire prevention training. Staff records checked showed that the necessary employment checks are made before new members of staff commence work in the home. Comments from people living in the home included I like all the staff, nothing to complain about in any shape or form. All staff are very good, get all the help I need. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The home is managed in an open and inclusive manner by the present registered manager who enjoys the support of staff and people who live in the home. We have made this judgment using a range of evidence, including a visit to the service. All people spoken to considered that the home was well run and organised. The organisation has a quality assurance policy that involves checks being made every three months against the relevant standards. The latest check had been undertaken in September 2007. The health and safety records checked were up to date and in order.
