Conditions of registration: Date of last inspection 25th October 2005 Brief Description of the Service: 45 Jubilee Road is a privately owned service for three people who have learning disabilities. Since the last inspection a new manager has been employed, although they are not yet registered with the CSCI. The home is a three-bedded bungalow with an office in the roof space. It is in a residential area of similar properties and within walking distance to local shops and leisure facilities. Twenty - four hour staffing is provided which is flexible to the needs of the service users. There is a vehicle available for service users, which is unmarked. This service would not be suitable for wheelchair users as the rooms and door sizes would not be able to accommodate a wheelchair. However, due to the needs of service users accommodated a ramp has been installed from the lounge and kitchen doors to secure access for all to the small garden to the rear. The manager informed the CSCI on 22 May 2006 that the weekly fees at the home are between £472 and £1350 depending on the needs of the service user. 45 Jubilee Road (Chartwell House) DS0000011746.V298617.R01.S.doc Version 5.2 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. The evidence used to write this report was gained from a review of the information the provider sent to the Commission for Social Care Inspection (CSCI) and a site visit to the home on 14th July 2006. A follow up visit was made on 18th July 2006 to meet with the service users as they were not at home during the first visit. During the site visit the inspector spoke with all three of the service users, staff on duty and the manager. A tour of the communal areas of the building was made and the inspector observed the way staff were supporting service users. Documents relating to the running of the home were inspected during the visit. What the service does well: What has improved since the last inspection?
The home is ensuring that medication is administered safely, in line with their procedures. The home’s environment continues to be improved, with decorations to the lounge and one of the bedrooms. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There are good systems to assess the needs of service users before they move into the home. EVIDENCE: The files of all three service users were inspected during the visit. Each contained an assessment of their needs and a copy of the care management assessment. This assessment covers the individual needs of service users, including communication and personal care needs. No new service users have moved into the home since the last inspection. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7 and 9 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home has an adequate care planning and risk assessment system, although changing some documents to make them clearer will help to ensure the needs of service users are met. EVIDENCE: The personal files of all three service users were inspected during the visit. Each service user has a care plan that has been re-written since the last inspection. These plans are reviewed monthly and the manager reported that they would be changed if the needs of the service user had changed. The manager said service users have not been supported to develop person centred plans and agreed to contact local advocacy agencies to find out what support was available for service users. Care plans contain details of how service users should be supported to make decisions, including the way staff should communicate. Daily records demonstrate the support staff have provided to help service users make decisions, setting out the various options and consequences. Risk assessments were in place for all three service users. These documents set out the assessed hazards to service users and action to minimise the risk of harm. Manual handling assessments have been completed for two service

The manager reported that her assessment was that it was safe for one staff member to provide support, following input from an occupational therapist on 22/3/06. The manager did not have a letter from the occupational therapist to confirm this. A staff member spoken with said that they were confident in the moving and handling support that was needed and felt that the practices kept service users and staff safe. It was also noted that one service user requires support to transfer from the floor to their bed, although there were no details in the care plan or risk assessment stating how this support should be provided. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16 and 17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home provides good support to meet the lifestyle needs of service users, including suitable activities, appropriate relationships and good meals that offer variety and choice. The rights and responsibilities of service users are recognised. EVIDENCE: Service users are supported to take part in a range of educational and leisure activities. Service users completed comment cards before the inspection and said “I like going to the pub” and “I like going on the mini bus to work and trips out”. Service users attend local day services, college courses and sessions at a local garden centre. Service users spoken with said they liked the activities they took part in and were particularly looking forward to a barbeque they were planning for the following weekend. Service users are supported to keep in touch with family and friends, with support provided to go to visit relatives and well as people being visited at the home.

Service users have a key to their bedroom and those spoken with said that staff respected their privacy. The home has a planned menu that takes into account the likes and dislikes of service users and provides a varied and balanced diet. Mealtimes are flexible to fit in with service users’ activities. The kitchen was well stocked with a variety of good quality food. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19 and 20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The personal and healthcare needs of service users are well met by the way staff provide support and access to health services. The home has good systems for administering medication. EVIDENCE: All service users completed a CSCI comment card and stated that staff treat them well and that they are able to visit the doctor and dentist when they need to. Service users spoken with confirmed this. The recommendation made at the last inspection that the registered person should develop a cross gender policy and procedure has not been met. The manager said she has made sure that male staff are not providing intimate personal care to female service users, although this has not been formalised. Service users are supported to attend a wide range of health services, including GP, dentist, physiotherapist, district nurse, optician and chiropodist. Records were kept of visits and included any advice that was given by the practitioner. Medication is stored in a locked cupboard and records relating to administration of medication had been fully completed. All staff have completed medication training and the manager reported that they were all

JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users are confident their complaints will be taken seriously and acted upon and the home has good adult protection systems, which help to keep service users safe. EVIDENCE: The home has a complaints procedure available, which sets out who will deal with a complaint and how long the provider will take to respond to a complaint. The procedure has been supplied to all service users and they all sated on their comment card that they know what to do if they want to complain. The relative who completed a comment card said they are aware of the home’s complaints procedures. Service users spoken with said that they were confident any complaint they made would be taken seriously and investigated. No complaints have been received since the last inspection. The home has an adult protection policy and a copy of the local authority adult protection procedures. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 and 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is well maintained and provides a safe, homely environment for service users. EVIDENCE: Since the last inspection the lounge and one of the bedrooms have been decorated. The service user whose room has been decorated said they had chosen the colours and the carpet. The home was clean and well maintained and the manager reported that she was able to access money to repair and replace items when necessary. The manager reported that she planned to redecorate the hallway and bathroom by the end of 2006. Service users spoken with said that they had sufficient communal space in the home. Since the last inspection the hoist in the bathroom has been serviced The home has a domestic washing machine in the kitchen, although there are procedures in place to prevent it being used whilst food is being prepared or eaten. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 and 35 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The qualities and qualifications of staff and robust recruitment procedures help to ensure the needs of service users are met. Staff receive adequate training, although this will be improved by an induction in line with the learning disabilities awards framework. EVIDENCE: The manager reported that three of the five care staff have the National Vocaional Qualification (NVQ) at level 2 or above and that she was looking for an assessor for the other two members of staff. The records of the most recently recruited staff member were inspected and contained all of the information and checks required. Since the last inspection the manager has re-assessed the staffing needs and said she feels the current levels meet the needs of service users. Service users and staff spoken with said they felt there are sufficient staff on duty at all times. Staff complete the skills for care induction, although the manager said they do not currently use the learning disability award framework. The manager agreed to look at how she could introduce this. Staff have undertaken training in first aid, fire safety, health and safety, food hygiene, moving and handling, adult protection and medication.

This judgement has been made using available evidence including a visit to this service. The home is run by a qualified manager who takes action to promote the health, safety and welfare of service users and staff. The registration of a manager will help to ensure that the service continues to develop. EVIDENCE: The manager reported that she has completed her application for registered manager with the CSCI and is waiting for the return one piece of information before it is submitted. The manager reported that she has completed the NVQ level 4 in care and the registered manager’s award. The registered provider completes monthly visits to the home and makes a report of his findings, including an action plan to address any deficiencies. The home has a development plan and regularly consults with service users, including an annual survey. The manager has implemented weekly quality assurance checks, to ensure that the service is continuing to meet the needs of service users. 45 Jubilee Road (Chartwell House) DS0000011746.V298617.R01.S.doc Version 5.2 Page 19 Since the last inspection the manager has completed assessments of chemicals used in the home. Fire evacuation risk assessments have been completed for all service users, although the manager agreed to provide more information about the understanding each service user has about the evacuation process. Regular tests are made of the fire alarm system and equipment and the system and extinguishers are regularly serviced. Staff have received fire safety training. The electrical wiring was checked in 2004 and portable electrical appliances are tested annually.
