The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

What was evident from discussions held during the inspection and one file viewed was the commitment to ensuring Service Users are central to all care provided and lead a full and positive life style. Service Users have a range of in house and community based activities to choose from and are supported to go away on holiday if they choose every year.

What has improved since the last inspection?

The staff team have continued to build on Service users independence both within the Home and the community. Activities pursued by individuals have increased with staff ensuring Service Users have the support and confidence to widen their interests . Staff have received training in Adult Protection which was a requirement from the last inspection. The Service User Guide is being compiled in a pictorial format which will ensure they are accessible to all Service Users.

What the care home could do better:

Service User`s care plans need to be reviewed. Due to their being no Service User`s accommodated who can read menus would be more accessible to Service Users if presented in a pictorial format. Consultation needs to take place with the District nurse in relation to a specific health need of one Service User. This should be documented in their care plan ,shared with staff and regularly reviewed.

Service users in the category LD are only to be accommodated between the ages of 30 and 65 years. Date of last inspection 29/09/05 Brief Description of the Service: 45 Jubilee Road is a privately owned service for three people who have a learning disability . The owners are David and Anita Rodgers. Mrs Rodgers is the Registered Manager. The Home is a three bedded bungalow with an office in the roof space. It is in a residential area of similar properties and within walking distance to local shops and leisure facilities. Twenty - four hour staffing is provided which is flexible to the needs of the Service Users . There is a vehicle available for Service Users which is unmarked. This service would not be suitable for wheelchair users as the rooms and door sizes would not be able to accommodate a wheelchair.However, due to the needs of Service Users accommodated a ramp has been installed from the lounge and kitchen doors to secure access for all to the small garden to the rear. 45 Jubilee Road (Chartwell House) H54 S11746 45 Jubilee Road V218685 230505.doc Version 1.30 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. The inspection took place over 4 .5 hours and was the first unannounced inspection of the 2005/2006 inspection programme. Twelve of the forty three Standards relating to Younger Adults were assessed. There were two requirements identified at the last inspection one of which has been met and one of which is in the process of being completed. During this inspection there was one recommendation identified details of which can be found in the main body of the report. The inspection included a tour of some areas of the Home and a discussion with the Proprietor. The Manager and staff were not available. Due to all Service Users attending their Day Service the inspector arranged to visit one of the Service Users at their centre later in the morning. Understanding of practices and Service User views were also gained through case tracking. One Service User file was viewed and care provided by the Home in all areas of their life assessed and discussed with the Proprietor. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 and 4 The assessment process undertaken by the Home is satisfactory ensuring Service Users needs are identified . EVIDENCE: There have been no new admissions to the Home since the last inspection. However, the manager confirmed that there are systems in place to ensure any prospective Service Users needs are assessed by a suitably experienced and competent person. All prospective Service Users would be invited to the Home prior to admission with staff and Service Users having an opportunity to meet and express their views in terms of compatibility. Introductions would consist of a meal and overnight stay. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6and 9 The arrangements for care planning are satisfactory ensuring all Service Users care needs are met. EVIDENCE: One Service User file was viewed and care “tracked” with the Proprietor David Rodgers. The file had a range of information relating to the individual including a care plan ,risk assessments and monthly reviews . It was evident from the risk assessments compiled that Service User’s are encouraged to take risks with the relevant support networks in place. The details held relating to the history of the Service User were particularly informative and had clearly contributed to how the individual’s needs were being met. There was evidence that the Service User had contributed to their care plan through documentation viewed. However, there was no evidence to confirm when the risk assessments had been undertaken and whether they had been reviewed . 45 Jubilee Road (Chartwell House) H54 S11746 45 Jubilee Road V218685 230505.doc Version 1.30 Page 10 Information relating to medication prescribed held in one Service User’s file was also out of date with Mr Rodgers confirming this should have been removed . The Proprietor indicated that he would need to look at all of the three Service Users files as he was unsure as to how up to date they were. As all Service Users were attending Day Services the inspector arranged to meet with one individual at their Day Service later in the morning to discuss care received from the staff team at Jubilee Rd. Due to their specific needs they were supported by their Key Worker at the Day Service. Much praise was given by the keyworker as to how the Home are meeting the social, health and general needs of the individual. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13 and 17 Links with the local community are good with appropriate activities taking place based on Service Users interests to meet their needs. Menus are well balanced, creative and offering choice. EVIDENCE: From one file viewed it was evident that the Home is endeavouring to ensure Service Users have a community presence with appropriate support to pursue their individual interests. These include attendance at church, discos, shopping, day trips , clubs and the “pub”. The Proprietor advised that Service Users had expanded their social network and leisure activities pursued since the last inspection. This was evident from the individual’s file. All three Service Users accommodated attend day services five days a week . 45 Jubilee Road (Chartwell House) H54 S11746 45 Jubilee Road V218685 230505.doc Version 1.30 Page 12 Rotas confirmed there are sufficient staff on duty during the evening and weekends to support activities identified. The Home has an unmarked vehicle with the majority of staff insured to drive Service Users to their activities or community events. There are currently no Service Users accommodated who would understand the concept of voting although all are registered to do so. The manager confirmed that this would be pursued if appropriate for an individual. Menus were seen to be well balanced , nutrious and offering choice. The Proprietor advised that Service Users are involved in the planning, preparation and cooking of meals . However, the menus would be more accessible in a pictorial format due to none of the Service Users accommodated being able to read. The Proprietor confirmed that this piece of work would be undertaken with Service Users. 45 Jubilee Road (Chartwell House) H54 S11746 45 Jubilee Road V218685 230505.doc Version 1.30 Page 13 Personal and Healthcare Support The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 20 The health needs of Service Users are well met with evidence of multidisciplinary working taking place on a regular basis. The systems for the safe handling and administration of medication are good ensuring the medication needs of Service Users are met. EVIDENCE: All Service users have access to a GP with one care plan viewed confirming that the Service User has access to the local Health team. Medical reviews are regularly held. Due to the specific needs of one Service User, the Proprietor was advised to contact the District Nurse to seek guidance as to the training requirements of staff. Details of preferred personal support needs for Service Users were found in the one file viewed and are commendable in their detail and guidance to staff. There is one Service User who is prescribed medication . Records were held in the Home with staff signing to confirm they had administered the medication. There are currently no Service Users who self medicate and no controlled drugs administered. Records are kept of over the counter medication administered. The Home would discuss with the Service User’s GP prior to administering. 45 Jubilee Road (Chartwell House) H54 S11746 45 Jubilee Road V218685 230505.doc Version 1.30 Page 14 All staff are required to undertake an accredited safe handling of medication training course and, a period of shadowing before administering any medication to Service users. There is a copy of the Royal Pharmaceutical Society Guidelines held in the Home in addition to the Home’s policy and procedure which all staff are required to read and confirm their understanding and application to service delivery. 45 Jubilee Road (Chartwell House) H54 S11746 45 Jubilee Road V218685 230505.doc Version 1.30 Page 15 Concerns, Complaints and Protection The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22and 23 The complaints policy and procedure is accessible and, in a suitable format for all Service Users accommodated ensuring their concerns are addressed . Arrangements for protecting Service Users are satisfactory. EVIDENCE: There have been no complaints to the Home or Commission since the last inspection. Due to Service Users needs the complaints policy and procedure is currently being produced in a pictorial format with Service User’s fully involved in the process. Service User’s families are informed of the complaints policy and procedure . For those Service User’s who do not have families advocacy can be accessed. The Home has a copy of the Department of Health No secrets documentation and Hampshire Adult protection Policy. The Home also has a whistle blowing policy. The Proprietor indicated that staff have access to the polices at all times which are held in a folder in the kitchen. They further receive training during induction as to there application to service delivery and, if there are any amendments made. The Proprietor demonstrated a good knowledge of the process of reporting abuse to Social Services. He further confirmed that a requirement made at the last inspection for all staff to undertake Adult Protection training had been met 45 Jubilee Road (Chartwell House) H54 S11746 45 Jubilee Road V218685 230505.doc Version 1.30 Page 16 with the exception of one new staff member . However, in house training had been provided during induction with a course to be arranged shortly . Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 30 There are good systems in place to ensure the Home is clean and hygienic. . EVIDENCE: The Home was clean, bright and hygienic with policies and procedures and systems in place including infection control /Control of substances hazardous to health ( COSHH )/food hygiene and moving and handling training for staff . The Proprietor confirmed that staff were aware of their responsibilities in relation to hygiene in the Home , were provided with gloves and aprons as required and had received infection control training.
