Residents in the MD (E) category are not to be accommodated under the age of 65 years 18th February 2005 Date of last inspection Brief Description of the Service: 45 Westridge Road is a small care home, for people over 65 years of age. The home is registered to accommodate three people. The home is situated in a residential area of Southampton. Accommodation comprises three single bedrooms with en suites. The home is well maintained, very homely and decorated to a high standard. Service users have access to a lounge/ diner and sunroom. Service users have access to the home’s transport and therefore have the opportunity to get out and about in the community. 45 Westridge Road DS0000011543.V253755.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This inspection was an unannounced inspection that took place over five and half hours. The purpose of this visit was to carry out an inspection of the home. Evidence for this inspection was obtained from several sources. These included a pre inspection questionnaire completed by the manager prior to the inspection, comment cards from residents and relatives, discussion with residents, the manager and her son, observation of care practices and an inspection of files and records. A tour of the premises took place that included looking at service user’s bedrooms. At the time of this inspection there were only two residents living at the home. What the service does well: What has improved since the last inspection?
The standard of record keeping is constantly improving and this assists with the efficiency of the day to day running of the home as well as minimising the risk that any care needs will be overlooked. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2,3 and 4 Service users and or their representatives are made aware of their rights and of the terms and conditions, including the cost of living at the home. The admission procedure however must be improved to ensure that only those whose needs can be met by the home are admitted. EVIDENCE: All residents had received a written contract informing them of their rights and responsibilities while living at the home. These had been signed by either the individual or their representative and were well laid out and easy to understand. One resident had been admitted to the home since the last inspection. The manager had ensured that the person had been properly assessed prior to them moving in and felt that the home could meet the person’s needs. However it was found that the home had admitted a resident who had a diagnosis of dementia and whose category of need was beyond that of the homes’ current conditions of registration. Although this resident’s current needs appear to be met within the home. This may not be sustainable should their needs increase. The home does not have

If the home were to admit other people with dementia there is a strong likelihood that the needs of the other residents would not be met or would be overlooked. This issue has been addressed separately from this report. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9, and 10 Service users receive adequate and appropriate support with their personal and health care needs and their privacy is respected. However an improvement is needed in ensuring that all needs are fully assessed and recorded. EVIDENCE: Each resident has an individual plan of care detailing his or her care needs. This includes an extensive ‘life history’, giving details of significant events in their lives, work history, memories and likes and dislikes. A separate page had been used to record and detail the individual’s preferred daily and weekly routines. It was evident that considerable time and thought had gone into obtaining this information and was evidence of good practice. Although it was clear from discussion with the manager and her son that they were fully aware of the support needs of the service users, care plans needed to fully reflect this. Some specific needs such, as those associated with dementia were not fully addressed. This will be especially important should the home employ any staff in the future. 45 Westridge Road DS0000011543.V253755.R01.S.doc Version 5.0 Page 11 Residents are fully supported with their healthcare needs. All residents are registered with a local GP. The inspector was informed that the GP is always willing to visit residents at the home in the privacy of their own rooms. A podiatrist visits the home and residents are registered with dentists and opticians and residents have access to District Nurses as required. Residents’ medication is managed appropriately and safely. Each bedroom has en-suite facilities and therefore all personal care is given in the privacy of service users bedrooms. This was done in complete privacy throughout the inspection. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 and 14 Residents are supported and encouraged to pursue their own interests and to access the local and wider community. EVIDENCE: It was evident that residents are given the opportunity to take part in activities of their own choosing. One resident was engaged in painting. The resident told the inspector that the manager supplied him with all the materials. Another resident had recently taken up knitting again and was relearning this skill with the support of the manager. Residents said that they enjoyed frequent trips out in the home’s transport. A selection of games was available including large print playing cards and dominoes with raised surfaces to assist service users with failing sight. Newspapers and magazines were available in the home and residents interests and hobbies were recorded in their care plans. During the inspection the manager was observed to spend one to one time with residents and both residents went out for a drive in the afternoon. Residents religious and spiritual beliefs are respected and one resident was being supported to attend weekly church meetings and the manager had also obtained large print Bible readings as this was something that the person said they enjoyed doing.

Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Satisfactory systems are in place for service users to address any concerns or complaints that they may have. EVIDENCE: There had not been any complaints made about the home since the last inspection. A complaints procedure is displayed within the home informing residents of their right to complain and how they can go about this. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25 and 26 The home provides a friendly, welcoming and homely environment to live in. EVIDENCE: The home is very well presented both inside and out. The décor and maintenance are kept under regular review and furnishings are replaced and renewed regularly. The home offers a friendly, welcoming and homely environment. The paving around the home had recently been replaced to provide a more even surface and had made the home more easily accessible to residents. The existing stair lift had also been replaced with one that provided greater comfort and safety. Bedrooms are personalised, well furnished and very clean. Each resident has their own single room with en suite toilet and shower facilities allowing them privacy. Beds had recently been replaced with electric motorised beds to provide more comfort and minimise the amount of moving and handling. All residents have a
45 Westridge Road DS0000011543.V253755.R01.S.doc Version 5.0 Page 16 portable call bell that can be moved to any part of their room, enabling them to summon assistance should they require. The home was clean and hygienic and there were not any unpleasant odours. Infection control procedures are followed to ensure the safety of residents. Residents said that they were happy with their rooms and were observed to be able to use all communal parts of the home or spend time in their own rooms as they pleased. Residents appeared to be relaxed and at home in their surroundings. 45 Westridge Road DS0000011543.V253755.R01.S.doc Version 5.0 Page 17 Staffing

The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 Staffing levels are sufficient to meet the current needs of residents. EVIDENCE: At the time of this inspection there were not any staff employed in the home. The proprietor and her son currently staff the home. Mrs Larmouth lives within the home and her son is on the premises daily. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,34,35,37 and 38 Residents live in a safe, well-managed home with thoughtful caring staff. EVIDENCE: The manager has owned and run the home for many years. She demonstrated a caring, kind and sensitive approach to residents needs. Residents said that the manager was helpful and well liked. The Commission for Social Care Inspection had recently received written feedback from a doctor praising the level of care and consideration that Mrs Larmouth had shown a resident in the last stages of her life. Investment in the home continues and this was evidenced by the recent improvements to the physical environment and the purchase of new equipment. The home had adequate insurance cover and residents’ belongings are insured to a maximum of £500.

All records were available for inspection upon request and there has been a steady improvement in the level and standard of record keeping over the last few inspections. One omission was noticed and will need to be addressed.
