What the care home does well

The manager makes sure she has a good understanding of the needs of residents before they move into the home. This provides re-assurance to residents that their particular needs can be met. These assessed needs are set out in a care plan, which provides information on how to meet them. Residents receive good quality care. Comments received by CSCI from relatives and representatives included, "The care and attention that Mrs Larmouth gives to guests is outstanding" and an "extraordinary degree of care shown." Residents are supported to attend all the health services they need. Residents take part in a wide range of activities they enjoy and a choice of good food is provided. Residents have not made any complaints, but are confident they would be listened to if they did. The home is safe, clean and provides a comfortable, homely environment for residents. The home is well managed and run in the best interests of residents.

What has improved since the last inspection?

Mrs Larmouth has obtained a copy of the local authority adult protection procedures and demonstrated a good understanding of what to do if an allegation of abuse were made. One of the bedrooms has recently been re-plastered and decorated.

What the care home could do better:

The plans to develop an annual survey of residents should be introduced and extended to relatives, friends and professionals involved in residents` lives.

The home is situated in a residential area of Southampton. Accommodation comprises three single bedrooms with an en suite toilet and shower. Residents have access to a lounge/ diner and sunroom. Residents have access to the home’s transport and therefore have the opportunity to get out and about in the community. The home is owned and managed by Mrs Larmouth who has owned the home for many years. Mrs Larmouth reported on 8th January 2007 that the fees for the home were £385 per week. 45 Westridge Road DS0000011543.V323801.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. The evidence used to write this report was gained from a site visit to the home on 7th February 2007 and a review of the information the provider sent to the Commission for Social Care Inspection (CSCI), including a pre-inspection questionnaire. During the site visit the inspector spoke with two of the three residents, the assistant manager and the manager. A tour of the building was made and the inspector observed the care that was provided to residents. Documents relating to the running of the home were inspected during the visit. Comment cards and letters were received from two relatives, a friend and a solicitor of a resident. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. This judgement has been made using available evidence including a visit to this service. There are good systems to assess the needs of residents before they move into the home, which assures residents that their needs can be met. EVIDENCE: Prior to admission to the home Mrs Larmouth completes a full assessment of prospective residents to ensure that their needs can be met. This covers both physical and psychological needs. Information is obtained from the resident’s family and other health professionals where appropriate. Following the assessment, Mrs Larmouth decides whether or not she can meet the needs of that prospective resident. The needs identified during this assessment form the basis for the care plans that are written when the resident moves into the home. Residents spoken with confirmed that they had been asked what their needs were before they moved in. The assessments were seen in residents’ care files for the three residents whose care was tracked. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 7, 8, 9 and 10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The details recorded in care plans, support to access health services and the way residents are cared for ensures that they are treated with dignity and respect and their needs are met. EVIDENCE: The care plans of three residents were viewed during the visit. These documents set out how the assessed needs of residents should be met and were reviewed every 2 weeks and amended if needs changed. All residents are registered with a local GP practice. Mrs Larmouth reported that they had a good relationship with the practice and the GP would visit the home if needed. Residents’ records showed that they were supported to attend regular chiropody, dental, audiologist and optician appointments. Residents spoken with confirmed that they attended the health services they needed. 45 Westridge Road DS0000011543.V323801.R01.S.doc Version 5.2 Page 11 Medication was securely stored in each resident’s shower room and administration records had been fully completed. The balance of medication held in the home matched the records. Records were available of medication that had been returned to the pharmacist to be destroyed. One resident is currently administering their own medication. Residents spoken with said that Mrs Larmouth treated them well and provides support in a manner that maintains their dignity. Comments were received from two relatives, both of whom praised the high standard of care provided. Comments included, “how very happy my mother is to be looked after so well” and “The care and attention Mrs Larmouth give to the guests is outstanding.” 45 Westridge Road DS0000011543.V323801.R01.S.doc Version 5.2 Page 12 Daily Life and Social Activities The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12, 13, 14 and 15. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The home provides excellent support to residents to take part in social activities and visitors are made to feel welcome. A choice of good food is provided for residents and meal times are a relaxed, social occasion. EVIDENCE: During the visit, one resident was in the lounge reading the newspaper, one was in their bedroom having breakfast and one had gone out for the day with a friend. Residents said they enjoyed going out into the New Forest with Mrs Larmouth, playing cards, reading and meeting with friends. Residents’ interests are recorded as part of their initial assessment before moving in to the home. Residents are supported to attend their local church services and other groups the church runs. Relatives and friends commented that they were able to visit at any time and were made to feel welcome. Residents are able to receive visitors in private and Mrs Larmouth reported that she has a spare room she makes available to relatives if they have travelled a long distance. 45 Westridge Road DS0000011543.V323801.R01.S.doc Version 5.2 Page 13 Residents are able to bring in their own possessions into the home and Mrs Larmouth was supporting one resident to arrange delivery of items from their home. Residents are able to manage their own financial affairs and are provide with details of an advocate who will act on their behalf if they wish. During the visit a mealtime was observed. Residents were provided with appropriate support, with one meal pureed to meet specific needs. There was a relaxed and friendly atmosphere in the dining room and Mrs Larmouth engaged residents in meaningful conversations about events in the news. Residents are able to take meals in their bedrooms if they wish. Residents spoken with said that the food was good, with one person saying it was excellent. The likes and dislikes of residents are obtained when they move into the home and the menu reflects these preferences. The home has good systems to respond to complaints and protect residents from abuse. This gives residents and their representatives confidence that their concerns will be taken seriously and acted upon. EVIDENCE: The home’s complaints procedure is supplied to residents and their relatives and is displayed on a notice board in the home. Residents spoken with said that they were aware of the home’s complaints procedure and were confident that any complaints would be taken seriously and responded to appropriately, although said they did not have any complaints. No complaints have been received by the home or the Commission since the last inspection. Since the last inspection Mrs Larmouth has obtained a copy of the local authority adult protection procedure and demonstrated a good understanding of the action she must follow if an allegation of abuse is made. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19 and 26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is safe, clean and provides a comfortable, homely environment for residents. EVIDENCE: A tour of all areas of the home was made during the visit. The home is cleaned to a high standard throughout and well maintained. Residents spoken with said “it really is a home from home” and felt very comfortable. The assistant manager reported that they had a variety of contractors who did maintenance work for them and did not have difficulties getting work done. Since the last inspection one of the bedrooms has been re-plastered and decorated. There is a stair lift to provide access to the first floor bedroom. 45 Westridge Road DS0000011543.V323801.R01.S.doc Version 5.2 Page 16 The home has a laundry room in the cellar and Mrs Larmouth ensures soiled laundry is not taken through the kitchen whilst food is being prepared. An environmental health officer inspected the home in January 2007 and did not identify any areas that required improvement. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27, 28, 29 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is sufficiently staffed by people trained and competent to meet the needs of residents. EVIDENCE: At the time of the inspection the only staff in the home were Mrs Larmouth and her son, who is the assistant manager. No other staff have been used since the last inspection. Residents spoken with said they felt the home was sufficiently staffed to meet their needs. A friend of a resident who completed a CSCI comment card said they thought there were sufficient staff on duty at all times. Mrs Larmouth said that she had considered using an agency to provide cover staff recently, but in the end it was not necessary. If using agency staff in the future Mrs Larmouth said she would ensure the agency provided details of the employment checks they have undertaken on staff supplied and the training they had completed. The assistant manager has completed training in risk assessment, first aid, manual handling, medication administration, dementia and the National Vocational Qualification in care at level three. 45 Westridge Road DS0000011543.V323801.R01.S.doc Version 5.2 Page 18 Management and Administration The intended outcomes for Standards 31 – 38 are: 31. 32. 33. 34. 35. 36. 37. 38. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31, 33, 35 and 38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is well managed and run in the best interests of residents. There are good systems to check and service equipment, which helps to keep residents safe. EVIDENCE: Mrs Larmouth has managed the home for many years and residents spoken with said she did an excellent job. CSCI received letters from two relatives of residents and a former resident’s solicitor, all praising the high quality of the service provided by Mrs Larmouth. 45 Westridge Road DS0000011543.V323801.R01.S.doc Version 5.2 Page 19 Mrs Larmouth lives on the premises and has daily consultations with residents about the quality of the service provided and any improvements that could be made. The assistant manager reported that he was in the process of developing an annual survey for residents to express their views on the service provided. Following discussions with the assistant manager it was agreed that this survey would be extended to include relatives, friends and any professional involved in residents’ lives, for example GPs. The home does not look after money on behalf of any of the residents. Residents have a lockable storage area to keep items secure and Mrs Larmouth reported that she planned to install a mini safe in each of the bedrooms for added security. An inventory of possessions residents bring in to the home is completed. Records demonstrated that the fire safety equipment was being regularly checked and serviced. Hazardous chemicals were suitably stored, in locked cupboards.
