Conditions of registration: 1. Service users can only be admitted over the age of 60. Date of last inspection 29/09/2004 Brief Description of the Service: Alverstoke House is a privately owned care home located in a residential area of Gosport. The manager, Mrs Rose Cook, has been in post since the home opened. The home offers nursing care for up to 30 clients from the age of 60 years. The home is sitauated within easy access of local amenities as well as being in easy access to the coastal area of Stokes Bay. The home is furnished and decorated in a homely fashion enhancing the comfort of the clients at the home. Discussion with the manager and those that live in the home revealed that service users like to be called clients. This shall be reflected throughout this report. This is an overview of what the inspector found during the inspection. The inspection was unannounced, starting at 9.30 am and lasting six hours. The inspector spoke to ten clients and five members of staff. Clients commented that ‘staff are kind and do everything to help you’ and that ‘the food was good but has even improved.’ Staff commented that they receive the support and training to meet the needs of the client group at the home. It is judged that the management of the home creates a supportive environment for both the clients and the staff at the home. What the service does well: What has improved since the last inspection?
Polices and procedures regarding medication at the home are being enhanced by the use of a link pharmacist. The staff at the home see this as a positive input into the home that will enable them to meet recommendations and requirements received following a pharmacy inspection. Advocacy information is now available for those clients that might require it. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 A procedure for the assessment of prospective clients is in use and utilised well to ensure that prospective clients needs will be able to be met. EVIDENCE: Records viewed indicate that prospective clients receive a comprehensive assessment performed by a trained nurse from the home of their needs prior to admission to the home to ensure that the home will be able to meet their needs. The manager discussed that the decision is made after this process as to whether the home will be able to meet that individual needs, she was able to share assessments of prospective clients that were unsuitable for the home. This was confirmed in conversations with several service users and a relative. Discussions with several care staff confirmed that they have the information to be able to care for and meet new clients needs on admission to the home. For clients moving to the home from out of the local area assessments of their needs are obtained via telephone conversations and professional reviews local to their area. Viewing hospital documentation that had been received for one client that had moved from out of the area evidenced this. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 8 & 9. The health needs of the clients at the home are met. There is good multidisciplinary involvement with the monitoring and the meeting of the health care of the clients. The medication at the home is well managed promoting good health. Staff at the home are able to care for those clients in the final stages of their life with sensitivity. EVIDENCE: Client’s health needs are assessed and monitored in conjunction with the registered nursing care allowances assessments, thus involving and utilising other health care professionals. These assessments of needs are then reflected in the client’s plan of care that is reviewed monthly. Clients are able to access opticians, chiropodists and hearing clinics either via domiciliary visits or visiting clinics in the community. The home has a large supply of equipment to enhance tissue viability. Clients stated that the staff at the home will contact their GP’s if there is any concern regarding their health. During the inspection it was noted that the home will contact the GP if relatives have concerns regarding their relative in the home. The manager informed the inspector that the home now has a link pharmacist from the local pharmacy that supplies the medications to the home. This link pharmacist is in the process of assisting the home meet the requirements and

The link pharmacist has put several new procedures in place. These still need to be reflected in the home’s policies regarding medication. Policies and procedures are in place regarding the care of the client that is in the final stages of his life. A member of the trained staff acts as the link palliative care nurse attending specific training sessions. The home has adopted a nationally recognised care programme for clients in the end stages of their life. Discussions with three care staff indicated that they feel they have appropriate training and support to enable them care for clients in the end stages of their lives. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 & 13 The home provides stimulating and varied activities thus enriching the life of the clients with staff at the home acknowledging the interests and social needs of the client group. The home encourages and supports clients to maintain links with the local community. EVIDENCE: A member of staff co-ordinates a regular Tuesday afternoon activity session that can consist of quizzes, karaoke sessions, musical sessions, armchair exercises and general discussions. The inspector joined in one of these session observing that the session although structured was flexible to enable the sessions be directed by the interests of the clients taking part. Groups visiting the home such as musicians, school groups and a mobile library provide further entertainment. Conversations that the inspector had with clients suggest that most clients feel that there are enough varied activities provided at the home to meet their interests. Client’s interests are documented within the pre assessment and care planning process. Clients indicated that it is their choice whether to join in with organised activities. Care staff informed the inspector that they spend time interacting with the clients on a one to one basis as well as in group sessions, this was confirmed in the conversations that the inspector had with clients. Conversations with clients and relatives visiting the home confirm that visitors are welcome at any time of the day. The manager discussed with the inspector

EVIDENCE: Following recommendations from the previous inspection a list of advocacy services have been drawn up which are to be reviewed annually. The manager informed the inspector that no clients use an advocacy service at present as all clients have relatives that are able act on their behalf. It was noted at the time of the inspection that voting cards were arriving for the clients. Clients are able to vote by either the use of postal votes or by the use of the local polling station that is located opposite the care home. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 Regular surveys ensure that the client’s opinions and wishes are an integral part of the running of the home. EVIDENCE: The manager and one of the registered providers shared with the inspector the annual financial plan thus indicating that the home remains financially viable and that there is continual reviewing and planning of the services provided by the home. Annual client and relative surveys indicate the level of satisfaction of the clients. Regulation 26 reports regarding the conduct of the home have still not been initiated. It is a requirement that monthly visits to the home are made and a report regarding the conduct of the home are provided to the home and to CSCI. The home continues to utilise a recognised audit tool to continually review the services and environment of the home. Service users that the inspector spoke with stated that their views and considerations are
