Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection of February 2009. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys returned to us by people who are involved with the service and visit the service in a professional capacity. Information about how the service has managed any complaints. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection. Relevant information from other organisations. What other people have told us about the service. What has this told us about the service? The home sent us their annual quality assurance assessment (AQAA) when we asked for it. It was clear and gave us all the information we asked for. We looked at the information in the AQAA that identified the areas of good practice and our judgement is that the home is still providing an excellent service and that they say they want to make further improvements to maintain that level of excellence over the next twelve months. The AQAA identified the main barriers to improvement was the overall increase in costs of running the home which has limited their plans for much improvement. The home has tried to reduce the impact of these barriers by working more efficiently with the resources they can afford. The AQAA states that the home aims to provide the highest levels of care and environment for all the residents, at the most reasonable price, however with costs rising so fast it is difficult for a home of that size to accommodate social care funded clients only. The AQAA identified that the service provides value for money by the level of bed occupancy, which has been 80 per cent during the year, the other 20 per cent was usually due to assessing residents for admission, preparing the room for a new client (new carpets and painting etc.) and also making sure that the room available matched the assessed needs for the client . The AQAA identified the improvements the home has made in the last twelve months as being a large increase in staff attending study days. One of the RGNs has had training in Dementia care and is giving study days and updates for carers. The AQAA states that the home has increased the percentage of NVQ carers to non NVQ carers to 88 per cent and are striving to increase this further whilst emphasising Annual Service Review Page 4 of 7 ongoing training and career pathways. The responsible individual has completed an Institute of Leadership and Management course Certificate in Strategic Leadership at Level 7. The deputy matron is currently completing her managers qualifications. As a result of surveys distributed to service users and relatives the activities available to the residents has been increased and consequently more residents are attending the activities. The home now has a list of interpreters they can use for non-English speakers. This has been accessed from Gosport Borough Council. The home has implemented a falls prevention action plan for each resident which has provided much better monitoring of the causes of falls. The home has increased the hours of housekeeping staff to ensure the home is clean and fresh at all times. The home is in the process of installing a Care IT package to computerise the written records. The service has continued to redecorate the home to a high standard and invest in equipment and training. The AQAA identified what the home does to ensure that race, gender, disability, sexual orientation, age, religion are promoted and incorporated in every day practice. Survey questionnaires were returned to CQC from service users, relatives, staff and other visiting professionals. There were positive comments made on the surveys and indicated a high level of satisfaction with the service. Comments from service users said: I am very well cared for and staff are friendly. I am quite comfortable here and would not like to move. I have never had to complain. I do like my food and I like to see everyone doing their jobs. I cannot grumble about anything. The food is lovely. I am not always happy with the puddings. I like my room and it is very clean. The homes present matron is very efficient and when not here she is missed but she has a good supportive staff. They have respected my wishes as far as diet is concerned and it is now satisfactory. The food is lovely here always but I would like fish and chips. The staff care for me well. The home is kept very clean and most of the staff are very nice. I would like milky coffee more often and a more varied diet. I feel I am cared for very well and my buzzer is always answered promptly. I like having two windows in my room. They do everything well but would like more activities in the afternoons. The food is nice and the girls are good to me. I think all the staff are very friendly and happy and will do some shopping for me which is a great help to me. Visiting professionals comments on surveys said: A very good care home it would be better if they had more rooms to take more patients. One of the best nursing homes in Gosport. I find the home very co-operative and friendly and they look after the service users well. My patients are well cared for. I am confident that high standards will be maintained despite loosing a superb matron in the near future. Relatives surveys commented: My relatives are currently receiving the care and attention they need. Both are in a comfortable state of health and appreciate the friendly caring attitude of all members of staff. Alverstoke House provides a high quality food. My father has a pleasant room with ensuite facilities which are always clean and tidy. His personal care is well catered for and his clothes are well laundered. Visitors are made most welcome and there is an inclusive atmosphere for residents and Annual Service Review Page 5 of 7 visitors alike. Residents have choice about where they eat meals and spend their time during the day. The home is clean and tidy and staff are always helpful and cheerful. The matron has high standards and she gets the best from her staff. The home provides a very friendly and extemely caring environment. Some staff provide care over and above what is expected of them. Visitors are made welcome and staff are always available to answer any questions. The home made cakes are excellent. Dad is very well cared for and the staff are really kind and very caring and go out of their way to cheer him up. Staff surveys returned to CQC indicated that staff receive good training opportunities and that they are well supported by the management and enjoy working at the home. Comments made say: I feel the home has a very individual approach to client care. Every effort is made to give care appropriate to each client. Staff are generally approachable and have a caring attitude. The home is clean and people are very friendly. The standard of food is excellent and service user have activities to participate in, if they wish. A very well run home with experienced and caring staff. The home promotes a good team spirit among the staff and listen carefully to staff feedback and respond accordingly. It would be good if they introduce a computer system. I think the home does well in providing good support to service users and staff. Alverstoke House is a well presented home and well maintained. We respect clients wishes at all times. It would be better if we had separate laundry baskets for each service user. The clients seem happy with the care they receive and we make sure the service users are comforatble and well cared for at all times. I really enjoy working at Alverstoke House and can see myself working there for many years to come. The home treats everyone as an equal. Training is very good and we are always adviced about when they are to take place. I have worked at the home for many years and have progressed well and am now the NVQ assessor. The Care Quality Commission has not received any concerns, complaints or allegations since the last visit. The AQAA informs us that the service has dealt with two complaints in the last twelve months, both of which have been resolved. The Commission has received notifications (regulation 37s) appropriately in the last 12 months, these primarily inform us of any incidents in the home that affect the service users. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by the 5th February 2011. However, we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
