The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Antron Manor offers a very attractive home for service users. The home is clean, well maintained, safe and homely. For example health and safety precautions are well maintained. Bedrooms are pleasant, clean and well decorated. The majority of bedrooms are ensuite. Fees are reasonable and represent good value for the care and facilities provided. Service users all stated they are very happy with the care provided by staff. Visiting health professionals also spoke highly of care provided. Management and staff attitudes are positive and caring, and this assists with helping ensure there is a calm and pleasant atmosphere in the home. Care records are maintained to a good standard.

What has improved since the last inspection?

Care and business documentation is much improved. For example service user money records have improved. Health and safety precautions e.g. regarding the prevention of Legionella have improved. The daily main meal is displayed in the dining room.

What the care home could do better:

Although improved since the last inspection, there are still some gaps in training required by regulation. One member of staff only had a standard Criminal Records Bureau check. By law an enhanced level check is required. These deficits could put service users at risk.

The home consists of a substantial older building with a modern extension in extensive mature landscaped grounds. The grounds can be used by service users, and are partly accessible for wheelchair users. The home is registered to provide residential accommodation and personal care to a maximum of 16 older people. All bedrooms, with one exception, are single with en suite (or individual) lavatories and hand basins. The one room that could be used as a double is currently occupied as a single. There are five communal rooms that are used for different purposes and activities. This inspection took place in just under seven hours. The inspection was announced. The inspector was able to speak to the majority of service users. The inspector also met with Mr Rogers-the registered provider. The inspector examined the care and business records, and inspected the building. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4 Suitable measures e.g. a pre admission assessment, and links with external professionals are in place so the registered provider can meet the needs of service users. All service users are issued with suitable information e.g. a service user guide and statement of terms and conditions of residency / contact when they become resident. EVIDENCE: Pre admission assessments are completed before the service user comes to live in the home. Copies of recent assessments were observed. The Inspector spoke to a Community Psychiatric Nurse, and Community Care Assistant who both spoke positively regarding care and staff attitudes at Antron Manor. Copies of suitable statement of terms and conditions of residency or contact were observed on service user files. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 10 Service users personal and health care needs are met appropriately. Service users are treated with respect and dignity. EVIDENCE: Care plans inspected were comprehensive to enable staff to provide service users with appropriate care. Mr Rogers said care plans are regularly reviewed. Mr Rogers said suitable links have been developed with GP’s, district nurses etc. Mr Rogers said support from external professionals is to a good standard. A Community Psychiatric Nurse visiting said staff always liaised appropriately with the health care trust, and she was positive regarding care delivered by staff. All service users the Inspector spoke to, said their rights were respected and they were treated with dignity by staff. Staff were observed working with service users in an appropriate manner. All service users personal care appeared to be maintained to a high standard. Service users confirmed they felt their personal care was carried out according to their wishes. There was a very pleasant calm atmosphere in the home on the day of the inspection. Antron Manor Care Home DS0000032603.V258874.R01.S.doc Version 5.0 Page 9 Locks are not fitted to bedroom doors. Mr Rogers said he had asked existing service users if they would like a lock on their doors, and all have said no. This has been recorded. All new service users must be offered a lock on their door when they are admitted to the home. Staff were observed knocking on service users bedroom doors before they entered. There seemed a genuine choice regarding whether service users wished to spend time on their own in their bedrooms or in the communal lounges. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13,14, 15 Routines are suitable to meet individual service users needs. Service users can receive visitors when they choose. Meals provided for service users are to a good standard. EVIDENCE: Service users said they could get up and go to bed when they wished, and have autonomy how they organised their time. Mr Rogers said some activities are arranged. These include music and aerobics groups and occasional trips out. Monthly visits occur from the Church of England Vicar. Activities have been arranged for Christmas. There seemed excellent interaction between service users and staff. For inspection purposes it would be useful if activities were recorded in an ‘activities book’ e.g. date / activity etc. Service users said suitable arrangements are in place for service users to receive visitors. Service users can meet with their visitors either in their bedrooms, the dining room or the lounge. Mr Rogers said staff look after some service users moneys. Appropriate records were observed regarding these. Mr Rogers said records are kept of fees paid. Information is provided regarding advocacy services. Mr Rogers said service users are able to bring their own personal possessions with them when they come to live in the home. Mr Rogers said service users are able to access their personal records if they wish. Mr Rogers said a choice of meal is offered on request. The daily main meal is displayed in the lounge. Staff were observed providing appropriate support to service users over meal times. Mr Rogers said appropriate arrangements are made for people with special diets and drinks are available to service users throughout the day. A water cooler is available in the conservatory. A service user said no cups were available recently for this. Mr Rogers said he would address this issue. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17, 18 Antron Manor has a satisfactory adult protection procedure-however all staff must have a Criminal Records Bureau disclosure at enhanced level. Service users legal rights are protected. EVIDENCE: Mr Rogers said appropriate arrangements are in place to enable service users to vote. Suitable information is also provided regarding advocacy services e.g. in the hallway or from Mr Rogers. The registered provider has a suitable adult protection procedure. All staff but one have a Criminal Records Bureau check, and where appropriate, staff have received a Protection of Vulnerable Adults check. Records show that one member of staff only had a Standard CRB disclosure-although an enhanced disclosure is required. Mr Rogers said staff are required to read the home’s policy regarding adult protection when they commence employment as part of their induction. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19-26 Antron Manor provides suitable facilities to meet the needs of service users. The home is well maintained, clean, comfortable and homely. EVIDENCE: The property was observed to be suitably maintained, safe, is comfortable and homely. Furnishings and decorations are to a high standard. The home has a lounge where service users can choose to relax. Other facilities include a dining room, and suitable bathrooms and toilets. The home was clean and hygienic on the day of inspection. The property has pleasant grounds which service users can use. Some of the grounds are accessible to wheelchair users. Bedrooms were observed to be appropriate to meet the needs of service users. All are decorated to suit individual tastes. Bedroom doors however are not lockable and current service users have said they do not want a lock. Size of bedrooms and communal areas are good. Appropriate precautions are taken against the risk of Legionella. Radiators are appropriately covered. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27-30 Staffing levels are suitable to meet the needs of service users. Suitable information is obtained regarding the recruitment of staff. The registered provider has an appropriate approach to ensuring staff receive National Vocational Qualification. However, other staff training must be improved so staff receive appropriate training as required by regulation. EVIDENCE: The inspector observed there was an appropriate number of staff on duty on the day of the inspection. Rota’s also provided evidence of satisfactory staffing i.e. at least two staff on duty during the waking day, a waking night staff and one member of staff who sleeps in. Suitable information is kept on staff files and this is much improved since previous inspections. There is evidence that staff receive an appropriate induction. Mr Rogers said opportunities are provided for staff to be able to study towards a National Vocational Qualification in care. Copies of certificates should be kept regarding NVQ qualifications on staff files. Staff training is much improved. However from the documentation provided there are still gaps in training required by regulation. This includes first aid, manual handling, infection control, handling of medication and food handling. The registered provider’s training policy should state the deadlines for when new staff will receive training required by regulation. Antron Manor Care Home DS0000032603.V258874.R01.S.doc Version 5.0 Page 15 The inspector suggests staff complete short courses from for example the health promotion agency / primary care trust (infection control), St John’s Ambulance (first aid), the pharmacist (medication), local college (food hygiene), in house training (manual handling).This will meet regulatory requirements. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 34, 35, 36, 37, 38 Service users benefit from a home, which is suitably managed, and run in their best interests. EVIDENCE: The registered provider has suitable skills, knowledge and experience to manage the home. For example Mr Rogers has an HNC in Health and Social Care, and the Registered Managers Award. Mr Rogers works alongside his staff on a day-to-day basis, and there appears to be good relationships between staff and management. Staff are suitably supervised on a day-to-day basis and appear well developed, caring and effective. Some gaps in training have been highlighted elsewhere in the report. Records examined regarding the management of the home, and regarding service users care and monies are appropriate. Policies and procedures appear satisfactory. It is suggested an annual development plan is written as part of the home’s quality assurance process. The inspector spoke to the majority of service users who all expressed high levels of satisfaction with care given. The home has a suitable health and safety policy which includes monthly health and safety checks. Health and safety precautions are satisfactory for example in regard to servicing of fire, gas, electrical equipment etc. Suitable records regarding these precautions were inspected. Suitable health and safety risk assessments have been completed and were inspected.
