Conditions of registration: 1. 2. A maximum of 34 service users requiring personal care who fall into the category of OP - Old age, not falling within any other category. Staffing must be provided to meet the dependency needs of the service users at all times and will comply with any guidelines which may be issued through the Commission for Social Care Inspection regarding staffing levels in care homes. The registered provider must, at all times, employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. 18th October 2005 3. Date of last inspection Brief Description of the Service: Arranmore Park provides 24-hour personal care for up to 34 older people. Arranmore Park is a detached residence, situated close to the village of Burscough in West Lancashire. The home provides both single and shared rooms, many of which have en-suite facilities on both the ground, first and second floor. The home has a passenger lift, two dining areas and three lounge areas. The home also has an enclosed patio area to the rear. This is an overview of what the inspector found during the inspection. The visit was unannounced and took place over a seven-hour period. At the time of the visit there were thirty residents living at the home. During the course of the visit the inspector spoke to the manager, consulted records and interviewed staff on duty. The records of five residents were looked at and these residents were seen privately about the care they receive. A tour of the building and garden areas was also carried out. Further information was received from five General Practitioners serving residents in the home and questionnaires were returned from residents and relatives. What the service does well:
Arranmore Park provides a good standard of personal care to its residents, their comments include: “I am looked after very, when Ii ring my bell I get immediate attention”. “It is excellent here I couldn’t be better looked after, I can’t speak higher about the home, staff are most kind and helpful” “It’s absolutely wonderful, they are very kind and I cannot fault them”. “The Staff and owners are very approachable and obliging”. There is a good staff team who carry out their tasks respecting the privacy and dignity of residents. They are supported in this with regular training opportunities. Meals served to residents are of a very high quality and there are always choices available to suit all tastes. The home employs a qualified chef. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 The assessment process is more structured and provides good information to produce care plans for residents to ensure their needs will be met. Quality in this outcome group is good EVIDENCE: A format for carrying out a full needs assessment has been developed and implemented by the home. This includes medical, social and personal needs. Risk assessments are carried out to provide safety and protection for residents. The assessments of five residents were looked at and their care discussed with them. Comments from these residents regarding discussions are contained in Standard 9 in the next section. The assessments form the basis for care plans, which are agreed with residents and their representatives.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 Quality in this outcome group is good. Care plans have been improved and areas of risk are included in the plans. The health care needs of residents are effectively met and there are good records contacts with local health providers. Medication policies and procedures are good and ensure medication is given out safely. Personal care is carried out in a sensitive caring manner in which residents rights, privacy and dignity are upheld and respected. EVIDENCE: The assessments and care plans of five residents were looked at and the care they receive was discussed individually with them. The care plans were easy to follow and had been drawn up in cooperation with the resident and their

Any risks are identified and discussed with the resident and actions recorded of how to deal with them. Independence of residents is seen as a priority and staff are trained to encourage residents to do as much for themselves as they can. Assistance is offered if and when required. Care plans are reviewed monthly with the involvement of residents. Residents expressed satisfaction with the levels of care they receive and feel that their privacy and dignity is respected in the way staff care for them. Comments from residents about their care include; “I am looked after very, when Ii ring my bell I get immediate attention”. “It is excellent here I couldn’t be better looked after, I can’t speak higher about the home, staff are most kind and helpful” “It’s absolutely wonderful, they are very kind and I cannot fault them”. “The Staff and owners are very approachable and obliging”. Medication records were looked at and the medication trolley was inspected. Only named staff who have had training are able to give out medication. All records were well kept and up to date. An audit of medication was carried out for three residents and the right amounts of medication were in their supplies. The controlled drugs register was looked and records maintained correctly. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15 Quality in this outcome group is good. There are a variety of activities that are provided both within the home and through outside activities and outings. Residents are able to maintain contacts with family, friends and the local community. The home offers good levels of support, while ensuring residents are able to exercise choice and control in daily living. Meals provide are of a high standard and there are always plenty of choices available which meet the needs of individual residents. EVIDENCE: Activities records showed that a wide range of activities are available to residents including trips to shops, craft centres, theatre, the local marina, local schools. There are music sessions in the home and craft sessions. Entertainers are arranged to provide music and songs. Two staff have been designated to ensure there are always a variety of activities on offer. Residents are consulted about their interests both individually and in residents

Resident’s social cultural and religious needs are respected and clergy from various denominations are made welcome and visit residents in the home. Family and friends are made welcome in the home and are able to see resident in the privacy of their own rooms if they wish. The homes routines are relaxed and residents are able to make their own choices about what they wish to do. This was confirmed in conversation with residents during the visit. The inspector also observed a relaxed atmosphere in which there was good interaction between residents and staff. Resident’s rooms are personalised and personal possessions were seen in each room. All residents’ rooms are individually and differently decorated. Those residents who are able control their own finances; though in most cases this is their families carry out function. The home is not involved in any resident’s financial arrangements other than keeping personal allowances. Records of these are maintained. Independent advocacy services are available to residents and information about these is contained in the Service Users Guide. All residents spoken to expressed satisfaction with the high quality and choice of food provided by the home. “The food is excellent and there are always alternatives”. “We are very well fed”. “Food is excellent”. “We are asked what we want to eat”. Resident’s surveys and meetings showed that they are asked about what should be included in the menus. A nutritional assessment is carried out on admission and the chef is made aware of individual needs. The home employs a qualified chef who is aware of the individual needs of residents. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Quality in this outcome group is good There is a good clear complaints procedure, which enables residents to raise any concerns they have with confidence that these will be listened to and acted upon. There are thorough policies and procedures in place to ensure the protection of residents. Management ensure these are implemented by providing training for staff in the protection of vulnerable adults. EVIDENCE: . The complaints procedures are contained in the homes procedure manual are available to all residents in their service users guide and a copy is pinned on the homes notice board. Residents said that they know what to do if they have any concerns and would speak to staff, the manager or the owner. One resident said that when a concern was raised with the owner of the home about wanting to change rooms this acted upon as soon as was possible. Staff spoken to were aware of the complaints procedure and knew where to find it. Any concerns residents have are acted upon and senior staff made aware of them.

Six staff have received training in the protection of vulnerable adults and this training is being offered to more staff as it becomes available. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome group is good. The home provides a comfortable, clean, well-maintained and safe environment for residents. EVIDENCE: A tour of the home was conducted during the visit and five residents were seen in their own rooms. The environment was warm and comfortable and regular maintenance take place to ensure standards are maintained. All residents’ rooms are decorated in different colour schemes and are personalised with their own possessions around them. Residents said they are happy with their accommodation and one resident spoke of a faulty call bell being repaired immediately it was reported broken. All areas of the home were spotlessly clean and good cleaning and infection control practices are in place. The laundry is suitable for ensuring clothing is cleaned appropriately.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30 Quality in this outcome group is good. The skill mix and number of staff on duty was sufficient to meet the needs of residents. The home places importance on providing training for staff. There are recruitment procedures in place to ensure staff are suitable to work in the care sector, however education and work histories on application forms need to be tightened up. EVIDENCE: Staff rotas were seen and showed that there are always enough staff with experience and skills to meet the needs of residents. Residents said that they are happy with the levels of care they receive and that when they need help staff are always available. See resident’s comments in Health and Personal Care section above. The recruitment policy of the home ensures that staff only begin employment at the home when satisfactory Criminal records bureau checks and references have been received. Personnel files were seen for four staff and confirmed this to be the case. Some of the application forms only contained very brief information about previous education and employment. One did not have any detail about previous employment. A recommendation is made that a full employment history is obtained with explanations regarding any gaps.

The home has also provided a wide variety of training courses for staff. These include, care of the dying and bereaved, medication training, first aid, protection of vulnerable adults and infection control. Staff receive good induction and supervision and there is ongoing commitment by management to ensure staff receive appropriate training to give them the skills to look after residents effectively. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35 and 38 Quality in this outcome group is good. The manager has a clear development plan for the home that is supported by the proprietors. Residents are consulted about developments in the home and are encouraged to take part in surveys and residents meetings. There are good records maintained for residents personal allowances. The homes policies and procedures and staff training ensure the health and safety of residents and staff is promoted. Arranmore Park Rest Home DS0000061200.V290464.R01.S.doc Version 5.1 Page 20 EVIDENCE: The home has a new manager who is currently in the process of being registered by the CSCI. She has over twenty years of experience in a senior capacity in running care homes for the elderly and is a qualified nurse. She is currently undertaking the Registered Managers Award. The manger has been engaged in reviewing all policies and procedures and the assessment and care planning process for residents. There have been good improvements in these and better systems have been put in place. Staff are more informed regarding care plans and are required to read them and keep them up to date. As a measure of the quality of service provided Arranmore Park holds a five star RDB rating. which is undertaken independently by assessors visiting the home. A residents survey has taken place and the results published. This survey was very positive, one resident commented, “The home for me is warm, cosy and very friendly”. Residents are encouraged to take part in residents meetings and have discussed in them issues such as meals and activities. The home holds for safe keeping residents personal allowances. Main financial arrangements are controlled either by residents or their families. Good records of transactions are maintained for personal allowances. Policies and procedures relating to health and safety have been revised and maintenance records are maintained and up to date. To ensure health and safety standards are maintained staff receive mandatory training in moving and handling, food hygiene and health and safety. Seven staff have been trained in First aid.
