Staffing must be provided to meet the dependency needs of the service users at all times and will comply with any guidelines which may be issued through the Commission for Social Care Inspection regarding staffing levels in care homes. 24th May 2006 Date of last inspection Brief Description of the Service: Arranmore Park provides 24-hour personal care for up to 34 older people. Arranmore Park is a detached residence, situated close to the village of Burscough in West Lancashire. The home provides both single and shared rooms, many of which have en-suite facilities on both the ground, first and second floor. The home has a passenger lift, two dining areas and three lounge areas. The home also has an enclosed patio area to the rear and has recently included a new conservatory area.. This is an overview of what the inspector found during the inspection. This was an unannounced site visit and took place over a six-hour period. The manager completed an Annual Quality Assurance Assessment. The inspector consulted care records and spoke to most of the residents living at the home. He discussed their care with them and visiting relatives. The inspector toured the building, spoke to staff on duty individually, had discussion with management and consulted records and policies and procedures. Questionnaires were sent out to relatives, residents and involved health care professionals. Comments received from residents and relatives were positive and were complementary about the care they receive. Their comments include: “I like it here, they are very good, Ii can’t find fault with any of them”. “ The staff are very friendly helpful and considerate” “They treat me very well”, “ The home is constantly improving, I regularly visit and I cannot fault it, the food is good, there is always choice, my relative is very happy and staff are always helpful”. “The home provides a warm caring staff, good food and good personal care.” “There is good all round care.” A GP commented, “They know their residents well and have a good relationship with them.” What the service does well:
Arranmore Park continues to provide a good standard of care to its residents. The staff team is well established and many of the staff have worked there for some years. There is good leadership and team working. Residents spoke highly of the care they receive and the staff approach to care. The homes philosophy of treating residents as individuals is maintained through the policies and staff induction and training. The home is very well maintained and provides and excellent and homely environment for residents. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 3 Quality in this outcome area is good. The assessment and introduction process ensures prospective residents are able to make an informed choice about moving into the home. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The inspector looked at the admission and care records for three residents. They or their representatives had been given the opportunity to look around the home and were given a service users guide. Each had received a contract and were given a settling in period prior to making a final decision about accepting a permanent place. All had been given a pre admission assessment, which formed the basis for a care plan. These were detailed and included risk assessments, which indicated

The assessment process includes a personal profile, however on files looked at this had not been completed. Improve the system these profiles should be completed more fully to ensure that care planning can be more person centred including resident’s social interests. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 Quality in this outcome area is good. Residents care plans contain good information and ensure health and personal needs are fully met, care is provided ensuring privacy and dignity is maintained. This judgement has been made using available evidence including a visit to this service. EVIDENCE: During the visit the files of three residents were looked at and their care discussed with them. Each resident had an individual care plan, which provided good information and detail to ensure that health and personal care needs can be fully met. Care plans are reviewed monthly, residents and their families are able to take part in the process. Care plans and reviews contained signatures wherever possible of the resident or their representative.

All these records were up to date and provided detailed information. While care plans had a section regarding social background, these had not been fully completed and it is recommended that these should contain more information to inform the care planning process. Medication policies and procedures were looked at and records examined, these are well kept and been up dated. A risk assessment is carried out to ensure any resident wishing to self medicate is able to do so. There is a clear description of all drugs dispensed stating what they are for and when they should be given. All staff giving out medication have received training in this. Residents said that they are happy with the care they receive and said that staff are kind and considerate, their comments included; “I like it here, they are very good, Ii cant find fault with any of them”. “ The staff are very friendly helpful and considerate” “They treat me very well”, “ The home is constantly improving, I regularly visit and I cannot fault it, the food is good, there is always choice, my relative is very happy and staff are always helpful”. “The home provides a warm caring staff, good food and good personal care.” “There is good all round care.” A GP commented, “They know their residents well and have a good relationship with them.” Good interaction between residents and staff was observed during the visit, all residents spoke highly of the care they receive, all were happy and content. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15Quality in this outcome area is good. Routines are flexible and enable residents to make choices about all aspects of living in the home. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Resident’s files contained a “daily living needs assessment form” which records their likes and dislikes. Through this residents are able to make choices as to how they wish to live their lives. This is risk assessed to ensure that individual health and safety matters are addressed. An activities organiser id employed to ensure that there is a regular programme of activities available both individually and collectively for residents. Residents said that they are happy with life in the home and are encouraged and enabled to take part in activities. They confirmed that they are able to rise and retire when they wish and can receive relatives and friends in the privacy of their own rooms. Links with churches and community are able to be maintained and the home has very good relationships with local clergy of different denominations.
Arranmore Park Rest Home DS0000061200.V338856.R01.S.doc Version 5.2 Page 13 The policies and procedures of the home ensure residents have flexibility and the aim is to ensure independence is maintained for as long as possible. The home has a resident’s rights charter, which promotes residents rights, all staff receive training in this when during the induction process. Staff spoken to had a good understanding of these policies and said that they saw their role as enabling residents to maintain independence and reach their potential. A fully qualified chef is employed by the home, which ensures residents receive a variety of well-prepared nutritious meals. Menus showed that there is a wide range of choice with alternatives always available at each mealtime. Resident’s surveys spoke positively about the food and residents told the inspector that the food is always good. Resident’s surveys carried out by the home and residents meetings showed that the home consults residents about the menus. Their initial assessments gather information about what individual needs are and these are passed to the chef to ensure correct diets are followed. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. There is a good clear complaints procedure available to residents and the homes policies include the protection of vulnerable adults. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The complaints procedures are contained in the homes procedure manual and are available to all residents in the service users guide, which they each receive. A copy of the complaints procedure is pinned on the homes notice board. Complaints records seen showed that they are taken seriously and are responded to within the requirements of the procedures. Residents said that they know what to do if they have any concerns and find the manager and owner quite approachable. The home has a statement of values, which clearly states that it’s philosophy of care is to strive to ensure each resident is treated as an individual and is able to experience a fulfilled life within the home. This includes residents being able to practise their own religion and to exercise their right to vote in local and general elections. Records showed that at present none of the staff have been on a protection of vulnerable adults course. To fully understand these procedures all staff should receive this training. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome area is excellent. The home provides a comfortable, clean well-maintained and safe environment for residents. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There is an on going maintenance programme for the home, all areas seen were of an excellent standard. During the past year improvements have been made, which include the provision of three more ensuite bedrooms and the refurbishment of some bedrooms, the provision of a telephone room, the addition of a new sun lounge, the addition of a new drugs room, better office facilities for manager and staff, new carpets in some communal areas, Toilet facilities have been refurbished. Residents said they are happy with their accommodation and can personalise their rooms to make them more homely. There are garden areas, which is

Cleaners are employed to ensure the home is kept clean the cleaning and regular cleaning and hygiene programmes are in place. All areas of the home were observed to be spotlessly clean and hygienic. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30 Quality in this outcome area is good. The training, skill mix and number of staff on rota are sufficient to meet the needs of residents. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There are currently 53 of staff at Arranmore Park qualified with NVQ2, two more staff are in process of completing NVQ2 and two are doing NVQ3. Prior to this training all staff are provided with a basic practical induction programme. Following induction staff are taken on for a six-month probation period to ensure they are right for the job, this may be extended in some cases. Rotas were examined and showed that there are always enough staff on duty with experience and skills to meet the needs of residents. Residents said that there are always staff available to ensure their needs are met and that the right levels of care are provided. Residents are given the opportunity to comment on staff employed by the home in the homes resident’s questionnaire. The homes recruitment policies are robust and ensure that those appointed are suitable to be carers. Application forms are completed, references from previous employers are obtained and Criminal Record Bureau clearances are

All staff are provided with clear job descriptions, an employee handbook, an employment contract and the Code Of Practice for Social Care Workers. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,35 and 38 Quality in this outcome area is good. The home is well managed, which ensures residents interests are protected and health and safety issues are promoted. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The recently appointed manager has seventeen years experience working in the home and was deputy manager for eleven years. She has almost completed the Registered Managers Award. There is good support from the e homes owners, who have an office in the premises.
Arranmore Park Rest Home DS0000061200.V338856.R01.S.doc Version 5.2 Page 21 Risk assessments in place for the home and maintenance records are available to ensure machinery, hoists, electricity installations are safely maintained. There are budgets to ensure all areas of the home are maintained properly. Health and safety is taken seriously and policies and procedures are in place for t he guidance of staff. However it was noted from training records that training in Moving and handling, infection control, health and safety and fire training had all lapsed and required attention. Thirteen staff have received training in first aid, ensuring that there is someone with this qualification available on all shifts. At the time of the inspection the inspector was told that staff had been booked on for moving and handling training on 27th July and 1st August. Kitchen staff have received food hygiene training. Staff are happy with how the home is run and there are clear lines of accountability. Staff spoken to confirmed this to be the case, staff meetings are held on a regular basis and staff are able to express their views. Resident’s views are taken seriously and resident/relative surveys are carried out. Copies of the latest survey were seen during the inspection. Residents meetings are arranged and the manager is in daily contact with all the residents. The owner is also regularly available to residents. Residents feel they are able to approach the manager or the owner with any ideas or issues they may have. The home is currently accredited with five-star RDB rating. The home looks after some resident’s personal allowances, these are kept in a safe and good records are maintained of any transactions.
