Date of last inspection Brief Description of the Service: Arrigadeen is registered for 29 older people requiring nursing care. Arrigadeen Nursing Home Ltd owns the service. Mrs Roberts is the responsible individual for the company. Arrigadeen is a large house situated on a hill in Clevedon and has been adapted to meet the needs of the residents. The area around the home is pleasant and level and mainly comprises of private houses. Transport would be required to access the facilities of the town. The accommodation of the home is provided on three floors, with split landings each served by a passenger lift. It is comfortably furnished and has the appropriate handrails and grab rails to assist independence. The home has twenty bedrooms (fifteen single, and five double), five of which have en suite facilities. The communal areas on the ground floor consist of one large lounge/dining room, one smaller lounge and a conservatory. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out over a 7.5-hour period. Before the site visit, all information CSCI has received about Arrigadeen since the last inspection was reviewed. Mrs Denise Morey, who intends to apply to become the registered manager of Arrigadeen, was available throughout the inspection; Mrs Roberts, the company responsible individual was present for part of the visit. All areas of the home were inspected. There were twenty people resident in Arrigadeen. Ten of these residents were consulted individually, whilst a further five people were observed in the communal areas of the home. Not all people were able to comment on the care they received, but time was spent observing interactions between staff and residents. Individual discussions were held with four members of the staff team. Other staff were observed as they went about their work. During the inspection, there was a steady stream of people visiting residents. Three of these visitors were approached for their views of Arrigadeen. After the inspection, health and social care professionals who have dealings with the home were contacted. Their comments are included in the body of the report. What the service does well:
Residents’ health care needs are well met by a motivated and knowledgeable staff team. They have forged good links with local health and social care professionals. Staff treat residents with a commendable level of dignity and respect. Residents said that they enjoy the food provided at Arrigadeen. The cook knows what the residents like, and makes sure that meals are made from fresh ingredients. One resident commented ‘the only criticism I have, is that they sometimes put too much on my plate, and I feel guilty leaving food’. The staff team work together well, and appreciate all the training opportunities they are given. They enjoy their work, and said that they feel well supported by Mrs Morey and Mrs Davies (Matron). Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4 and 5. Standard 6 does not apply. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Prospective residents have their needs assessed by a senior member of staff to ensure the home is suitable for them. Although they are given good information about the range of services offered at Arrigadeen, the way the fees are calculated is not clear. EVIDENCE: The home’s Statement of Purpose was last updated in May 2007. It contains all the required information, and is easy to read. The Service User Guide is also written in plain English. Mrs Morey and Mrs Davies plan to review this over the coming months, to include more comments from residents, and photographs of the home. One person has been recently admitted to Arrigadeen. This person’s care records contained a comprehensive assessment of needs, with clear evidence that other health or social care professionals had been consulted about these needs. The resident said that she had visited Arrigadeen before she moved in, to make sure it was what she wanted. She said that staff have been ‘lovely’, and respected her wishes to remain in her room. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 and 11. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ healthcare needs are clearly identified in care plans. These plans contain up to date guidance to enable staff to meet these needs. Social and emotional needs are not so well documented, but staff demonstrate a good awareness of these issues, and treat residents with great respect. EVIDENCE: Four sets of care records were reviewed. These all contained an assessment of the resident’s needs on admission. Care plans and risk assessments were in place to meet all identified needs. These had been updated regularly and gave staff good guidance to meet physical needs. This guidance was in accordance with current good practice, and demonstrated the involvement of other health and social care professionals. Each set of care records contained an ‘end of life’ plan. It was apparent that this had been discussed with residents and their representatives. Their wishes had been clearly recorded.

This person said ‘ I was feeling unwell this morning, so the staff arranged for my doctor to come straight away’. Other health and social care professionals confirmed that staff at Arrigadeen contact them appropriately, and follow any guidance they offer. At the moment, care plans contain little information about social or emotional needs. In the Annual Quality Assurance Assessment, which Mrs Morey and Mrs Davies sent to CSCI before the inspection, this had already been identified as an area for development. Staff on duty were able to describe how they planned to make sure this information was recorded. It was clear that they already had a good knowledge of individual needs. One staff member explained why a resident sat in a particular place in the lounge; when another resident became agitated, staff very promptly assessed what the problem was, and were able to reassure the individual. Staff on duty showed a good awareness of the home’s policies and procedures regarding the administration of medicines. Residents’ prescribed medications had been regularly reviewed, and all the recording sheets were well completed. Since the last inspection, the medicine storage facilities have been improved. There was much evidence to confirm the home’s commitment to ensure that residents are treated with dignity and respect. Staff said that Mrs Morey and Mrs Davies are going to identify a ‘Dignity champion’ within the staff team, to promote good practice. Interactions observed during the inspection suggested that staff already have a clear understanding of these issues. Staff spoke respectfully to all residents. When care was delivered, staff made sure that residents’ privacy and dignity was respected. For example, when residents were transferred by hoist in the lounge, all the staff were careful to make sure that screens were arranged to shield the resident from view. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are able to choose how to spend their time. They enjoy the planned activities, but also appreciate being able to spend time chatting with staff, or sitting in the garden. The menu is varied, and residents are able to choose the food they prefer and like. EVIDENCE: There is currently no formal record of residents’ likes, dislikes and preferred activities. This is an area that the service has identified for development. Despite this, staff showed a good awareness of individual wishes and preferences. The home offers a programme of ‘low key’ activities, such as quizzes, bingo and memory games. Residents said that they enjoy this, and told the inspector how the activities organiser gears the quizzes to individual abilities. Residents also said that they like spending time chatting with staff, and sitting in the garden.

Visitors consulted, said that they were always made welcome. Staff greeted regular visitors by name, and offered everyone trays of tea. It was clear that residents were able to choose where to receive their visitors. Staff helped one person back to a bedroom, and arranged extra chairs in the lounge for another person. Everyone said that meals at Arrigadeen are very good. Only one person made a criticism, ‘they put too much on my plate, and I feel guilty about leaving anything’. The Cook regularly talks to residents, and knows about individual needs and preferences. The lunchtime meal of faggots, mashed potatoes and peas looked tasty; two people had chosen either a salad, or an omelette instead. These were freshly prepared. Staff gave appropriate and discreet assistance to residents who were unable to eat independently. Homemade cake was served with afternoon tea. This judgement has been made using available evidence including a visit to this service. Residents and other people involved with the home understand how to make a complaint, and feel confident that their concerns would be addressed. Staff at Arrigadeen know what to do if a complaint is made. EVIDENCE: There is a clear and accessible complaint procedure. Residents and relatives said that they would have no hesitation raising concerns with the person in charge. Staff record complaints on a form, which is given to the matron for action. Complaints received by CSCI were passed on to the provider, and were dealt with appropriately by the service. Staff showed a good awareness of safeguarding issues. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25 and 26 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Since the last inspection considerable improvements have been made to the accommodation provided at Arrigadeen. These have benefited both residents and staff. Further work is needed to make sure that the home is kept free from unpleasant odours. EVIDENCE: Since the last inspection, there have been lots of environmental improvements made. The kitchen has been refurbished and a new assisted bath ordered. There is a sluicing disinfector in the home, but this is currently not working. A replacement has been ordered, and was due to be delivered the week after the inspection. Staff reported a good supply of manual handling equipment to meet residents’ needs. Residents said that they like their bedrooms, and are able to bring pictures and small items of furniture to personalise these. The home was clean and tidy, but there was a noticeable odour in a number of rooms. This was discussed with Denise Morey during the inspection. She plans to consult her supplier about alternative cleaning products to address this problem. Residents and relatives said that they were very happy with the laundry service at Arrigadeen. This judgement has been made using available evidence including a visit to this service. Recruitment and training procedures are robust, ensuring that staff have appropriate skills to care for the residents at Arrigadeen. EVIDENCE: Records for two staff appointed since the last inspection were reviewed. These confirmed a robust employment procedure. Staff said that they appreciate the training opportunities they are offered. There was evidence of a thorough induction programme, and ongoing training. Staffing levels meet the needs of the current residents. Staff said that they are kept busy, but have time to chat with residents. Residents said that their needs are met in a timely fashion. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 33, 35, 36, 37 and 38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents and staff benefit from an effective management team who promote an open and inclusive style of management. EVIDENCE: Denise Morey is about to submit an application to become the registered manager at Arrigadeen. She oversees the day to day management of the service, whilst Marie Davies, who is a registered nurse, leads on clinical issues. According to staff and residents this arrangement works well. It is recommended that Mrs Morey and Mrs Davies draw up job descriptions to ensure that there is no confusion about roles and responsibilities. Since the last inspection, a new quality assurance system has been introduced. Copies of this were on display around the home. Staff said that they had been involved in the development plan for the home, and showed a good awareness of the areas for improvement. Copies of a recent resident satisfaction survey were available for inspection. It was apparent that their suggestions had been included in the development plan. The home manages small amounts of money for residents, where requested. Records of all transactions are kept. Staff said that they receive regular supervision and appraisals.
