Conditions of registration: Date of last inspection 25th April 2005 Brief Description of the Service: Avonbloom is registered under the Care Standards Act 2000, to accommodate fifteen older persons aged 65 years and over. The accommodation includes one double room, six single en suite rooms and seven single rooms without en suite. Bathing and shower facilities are available. There are two comfortable lounge areas one of which is the designated smoking room. There is also a pleasantly furnished dining room and a first floor quiet lounge area. The premises are situated on a busy main road in the south shore area of Blackpool. The home is close to local amenities and facilities and has good access to all transport services and routes. Some of the residents go out independently. Mrs Mandy Jackson is the registered provider, she runs the home in an informal manner and aims to provide a homely environment for her residents.. This is an overview of what the inspector found during the inspection. The inspection was announced and started at 10.00am. It took place over five hours. and was carried out with Mrs Jackson, the proprietor. A pre inspection questionnaire had been completed prior to the inspection visit. The inspector spoke with four residents and two staff members. Case tracking (whereby the inspector focuses on a small number of residents and examines their care, from admission to the present time) of three residents, was carried out. Part of the inspection was spent looking at daily notes and information about the home and residents. Time was spent observing the workings of the home and how staff members supported residents. A tour of the home was part of this inspection. What the service does well: What has improved since the last inspection?
Since the previous inspection, the Home has produced a good information pack, which is given to prospective residents. The admission procedure has been improved and detailed assessments of residents needs and care plans, are now completed. The home now employs a cleaner, and is much cleaner than on the previous inspection. Many of the carpets have been replaced and rooms redecorated. The recruitment process for new staff is now in place, all staff have Criminal Records Bureau (CRB )clearances, training programmes are now in place for management and staff members. Suitable activities are now arranged for residents. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1&3 The home provides residents with sufficient information they need, to make an informed choice about where to live. All prospective residents have a comprehensive assessment to ensure that the home will be able to meet their needs and provide the right care for them. EVIDENCE: The home has now produced a good Service User Guide, a copy has been given to all residents. The Statement of Purpose of the home is now complete and provides further information about the home, its facilities and the way it carries out care practices in meeting resident’s needs. The admission procedure has been improved since the previous inspection and now provides staff with guidance to ensure that new resident’s needs are properly addressed and met. Staff members have access to resident’s assessments and care plans and can refer to them on a daily basis to determine what tasks needs doing for the individual person.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 &10 The health needs of residents are well met with evidence of good multi disciplinary working taking place on a regular basis. The systems for the administration of medication are good with clear and comprehensive arrangements in place to ensure resident’s medication needs are met. EVIDENCE: Individual care plans have now been developed between staff and each resident. These cover all aspects of physical, intellectual, emotional and social needs and include the person’s likes and dislikes. Falls and accidents are recorded in an accident record book. G.P. and district nurse visits are recorded in the home’s daily diary. Since the previous inspection, a visit was carried out by the Commission for Social Care Inspection, Pharmacy Inspector, who found that the home’s policies and procedures for dealing with medicines were very good. Residents who chose to self medicate are provided with a lockable facility for storage and sign a “self medication assessment” to confirm that this is their choice. Staff were noted to knock on doors and seek permission prior to entering bedrooms. New residents said they were asked how they wished to be addressed but added that everyone uses Christian names at Avonbloom Rest Home. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 & 14 Recreational needs are catered for, links with the community are good, these support and enrich resident’s chosen lifestyle. The meals are particularly good offering both choice and variety, whilst catering for special dietary needs. EVIDENCE: The pre inspection questionnaire showed that activities now include outings to Fairhaven Lake, shopping trips to markets and Marks and Spencers, as well as taking residents to the local “Sunshine Club and” “Ladies Fellowship Club”. Entertainers visit the home on a regular basis, birthdays are celebrated with cakes and sherry. Most residents have a television or radio in their own rooms as well as use the communal ones in the lounges. All residents spoke highly of the food offered. They said they had “good choice” and the food was “well cooked “ and “always on time”. The dining room provides enough space for all residents to dine at the same time and is pleasantly set out with matching table linen and crockery. Some residents prefer to eat in their own rooms, although they are encouraged to mix with others in the dining room, especially at dinner time. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 The home has a satisfactory complaints system with evidence that residents feel that their views are listened to and acted upon. Staff have some understanding of Adult Protection issues, but are to attend formal training to further protect residents from abuse. EVIDENCE: Information is now clearly provided for residents in the event of them, or their relatives wanting to make a complaint about the home or its staff. Residents spoken with said they would tell the manager if they had any worries or concerns about their care or accommodation. Since the previous inspection, there have been no complaints received by the Commission for Social Care Inspection. Since the previous inspection, accredited training programme has now been set up for staff members and the manager, including Protection of Vulnerable Adults training. The home holds copies of Blackpool Vulnerable Adult Committee guidance and all staff have copies of Joint Code of Practice, Guidelines and Procedures. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 25 & 26 Some improvements to the décor and furnishings have been made however the outstanding matters do not provide the people living in the home with safe comfortable surroundings. EVIDENCE: The date and the report of the most recent visit by the Health and Safety Department and Environmental Health Officer, was not available, nor was the Fire Officers last visit or guidance and action on risk assessments. The inspector contacted Health and Safety and Environmental Health, and asked for a copy of the most recent report on the home. A further visit to Avonblooom will be carried out in April 2006.The fire safety officer agreed to visit the home on 9th February 2006 and forward his report to this office. Any issues arising from these reports will be addressed on an additional visit to the home. The home must ensure that suitable records are kept in future to ensure the safety of residents and staff.
Avonbloom Rest Home DS0000009785.V281532.R01.S.doc Version 5.1 Page 15 Since the previous inspection the home has continued to redecorate and recarpet bedrooms and the communal areas. Residents spoken with said they were pleased with the results. Toilet doors now have locks fitted, bedroom doors are being fitted with locks, as rooms become vacant. Bedrooms are well personalised to each resident’s choice. Since the previous inspection , a permanent cleaner has been employed which has improved the overall cleanliness and hygiene throughout the building. However two bedrooms are still malodorous and require attention, the manager agreed to recarpet one, within three days and to seek advice from the medical profession, on behalf of the resident in the other bedroom. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28, 29 & 30 The numbers of staff on duty were satisfactory to meet the needs of the people living at the home. The policies and procedures for the recruitment of staff now provide safeguards for the protection of residents. EVIDENCE: All residents spoke well of the staff. Comments included “they are lovely girls” and “they are magic”. Residents said their call bells were answered promptly during the night and that all the staff were friendly and pleasant to them. A good relationship was noted between staff and residents and there was a relaxed atmosphere in the home. Since the previous inspection, staff files now contain information to ensure that persons employed are fit to work at the home and competent to do their jobs, training is set up and National Vocational Training (NVQ) is ongoing at present. Criminal Records Bureau (CRB) clearances are on file for all employees, any new staff will need clearances before starting work at the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 36, 37 & 38 There is now some evidence of leadership guidance and direction to staff to ensure residents receive consistent quality care, however some maintenance issues need to be addressed to safeguard the health, safety and welfare of the people using the service. EVIDENCE: The manager and deputy are currently undertaking the Registered Manager’s Award and NVQ level4. Policies and procedures relating to the Care Homes Regulations 2001, have been set up since the previous inspection. The manager intends to carry out appraisals of all staff, this will continue on an annual basis. Formal supervision has not yet been completed, as recommended following the previous inspection, although the manager does spend time with her staff. It was agreed that a more formal supervision system be instigated.

As mentioned previously in this report, the date and the report of the most recent visit by the Health and Safety Department and Environmental Health Officer, was not available, nor was the Fire Officers last visit or guidance and action on risk assessments.
