Conditions of registration: Date of last inspection 6th February 2006 Brief Description of the Service: Avonbloom is registered under the Care Standards Act 2000, to accommodate fifteen older persons aged 65 years and over. The accommodation includes one double room, six single en suite rooms and seven single rooms without en suite. Bathing and shower facilities are available. There are two comfortable lounge areas one of which is the designated smoking room. There is also a pleasantly furnished dining room and a first floor quiet lounge area. The premises are situated on a busy main road in the south shore area of Blackpool. The home is close to local amenities and facilities and has good access to all transport services and routes. Some of the residents go out independently. Mrs Mandy Jackson is the registered provider, she runs the home in an informal manner and aims to provide a homely environment for her residents. At the time of this visit (7th June 2006) the information given to the Commission For Social Care Inspection showed that the fees for care at the home are £241.15 per week with added expenses for hairdressing, chiropody and hairdressing. This is an overview of what the inspector found during the inspection. The inspection was unannounced and took place over a two day period. It was carried out with Mrs Jackson, the proprietor, Mr Jackson, Fire officer Timperley and Fire Officer Mundy were also present for part of the time spent at Avonbloom. A pre inspection questionnaire had been completed prior to the inspection visit. The inspector spoke with seven residents and two staff members. Case tracking (whereby the inspector focuses on a small number of residents and examines their care, from admission to the present time) of three residents, was carried out. Part of the inspection was spent looking at daily notes and information about the home and residents. Time was spent observing the workings of the home and how staff members supported residents. A tour of the home was part of this inspection. What the service does well: What has improved since the last inspection?
Staff supervision and appraisal systems have been introduce, this makes sure that residents are getting the care they need and also the support and guidance that their carers need in the daily lives too. The home is now clean and free from offensive odour in all areas. Redecoration and reflooring has improved the look of many areas in the home. All staff have completed training in Adult Abuse Issues. The manager has completed her National Vocational Training Level 4.

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3 & 5 Quality in this outcome area is good. The judgement has been made using available evidence including a visit to this service. The home provides prospective residents with sufficient information, to make an informed choice about whether or not to make Avonbloom their home. All prospective residents have a detailed assessment to ensure that the home will be able to provide the right care for them and meet their needs. EVIDENCE: The home has produced a good Statement of Purpose and Service User Guide, copies were seen in bedrooms. This provides information about the way the home carries out care practices in meeting resident’s needs. Two comments received from resident’s were that they knew the home from visiting friends, prior to their own admission. Contracts are in place between the residents and the provider, these means that the residents know what to expect from the home and what is expected of them. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The health needs of residents are well met and people are treated with dignity and respect. The systems for the administration of medication are clear and comprehensive arrangements are in place to ensure resident’s medication needs are met. EVIDENCE: Care plans were well detailed and reflected the care that was being given, they also listed resident’s likes and dislikes. Falls and accidents are recorded, GP visits and those from nursing services are listed in the home’s diary. One comment card from a resident said “ the district nurse comes every week and Dr. Abbas is always on hand when needed”. All residents spoken with said they were very happy with their care, comments from new residents included “ staff are more helpful here” and “its more homely here”.

Residents who chose to self medicate are provided with a lockable facility for storage and sign a “self medication assessment” to confirm that this is there choice. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Links with the community are good and social activities are provided, these support and enrich resident’s chosen lifestyle. EVIDENCE: The pre inspection questionnaire showed that activities continue to include outings to Fairhaven Lake, shopping trips to markets and Marks and Spencers, as well as taking residents to the local “Sunshine Club and” “Ladies Fellowship Club”. Entertainers visit the home on a regular basis, birthdays are celebrated with cakes and sherry. Most residents have a television or radio in their own rooms as well as use the communal ones in the lounges. One comment card included” Mrs Jackson takes me home to my house, when I feel like it, or rides in her car and shows”. Residents said their visitors were always welcomed and refreshments were always offered. Staff said they encourage family members to visit and on previous visits, relatives have said they are made most welcome at the home.

Records of charges and payments were seen and were up to date. Avonbloom places emphasis on home cooked food, all residents spoke very well of the choice and quality. One comment card said “they give too much food” and “we have a good choice.” The dining room provides enough space for all residents to dine at the same time and is pleasantly set out with matching table linen and crockery. Some residents prefer to eat in their own rooms, although they are encouraged to mix with others in the dining room, especially at dinner time. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has a satisfactory complaints system with evidence that residents feel their views are listened to and acted upon. All staff have recently undertaken accredited training in the protection of vulnerable adults to further protect residents from abuse. EVIDENCE: Information is clearly provided for residents in the event of them, or their relatives wanting to make a complaint about the home or its staff. Residents spoken with said they would tell the manager if they had any worries or concerns about their care or accommodation. Since the previous inspection, there have been no complaints received by the Commission for Social Care Inspection. All comment cards received from residents confirmed they knew who to speak to if they were not happy. Since the previous inspection, staff have completed training in abuse awareness, those spoken with said it was very good. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 25 & 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The residents are now provided with a clean and comfortable environment, with well personalised bedrooms. This means that residents will feel at home with their belongings around them. EVIDENCE: Since the previous inspection, Health and Safety and Environmental Health visits have resulted in recommendations rather than concerns, the kitchen has been improved and new flooring is being laid in that are and in the passage way. A recent visit from The Fire Officer, made at the request of the Commission for Social Care Inspection, had resulted in recommendations. Mrs Jackson confirmed these have now been implemented throughout the home. The home continues to redecorate and replace floor coverings, all residents said they were happy with their accommodation.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The numbers of staff on duty were satisfactory to meet the needs of the people living at the home. The policies and procedures for the recruitment of staff now provide safeguards for the protection of residents. EVIDENCE: Again on this inspection, all residents spoke highly of the staff, comments included “all the staff are very friendly, we are not a number, we are a person”. Residents said their call bells were answered promptly during the night and that staff were always pleasant to them. Staff rota was included in the pre inspection questionnaire, and a consistency of staff members was noted. Many of the staff have been at the home for a long time, the home has very little turnover so both staff and residents get to know each other well. A good relationship was again noted between staff and residents and there was a relaxed atmosphere in the home. Staff files contain information to ensure that persons employed are fit to work at the home and competent to do their jobs, training is set up and National Vocational Training (NVQ) is ongoing at present. All staff hold first aid certificates.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 & 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There is evidence of leadership guidance and direction to staff to ensure residents receive consistent quality care. EVIDENCE: Since the previous inspection, Mrs Jackson has completed National Vocational Training Level 4 and has encourages all her staff to participate in regular training sessions which she is accessing for them. Regular formal supervision and appraisals are now in place for all staff members and staff confirmed they still have daily contact with Mrs Jackson for advice and updating resident’s well being. Avonbloom Rest Home DS0000009785.V292046.R01.S.doc Version 5.2 Page 20 Mrs Jackson now includes quality surveys in Avonbloom’s Service User Guide, she also spends time, on a daily basis with both residents and their relatives, to ensure the home is meeting their needs and they are content with the service offered. Residents meetings have proved unsuccessful in the past. All residents spoken with said they felt that Avonbloom is “our home”, some commented they felt safe. Residents were seen to be spending their day as they chose and making use of different areas both indoors and outdoors. Mrs Jackson has complied with requirements made by the Commission for Social Care Inspection on the previous inspection and has confirmed that she has addressed recommendations made by Environmental Health and the Fire Department. Record keeping in general has improved and Mrs Jackson recognises the importance of this as well as the importance of providing good care for her residents.
