Date of last inspection Brief Description of the Service: The Lodge provides accommodation with personal care to older people (65 ) both male and female. The home is registered to provide a service for up to 11 residents. Accommodation is laid out over 2 levels with stair lifts connecting the ground and first floors. The Lodge offers 7 single bedrooms and 2 double bedrooms all with en-suite facilities. In terms of communal space, the home has a lounge, dining room and conservatory. There are also 2 communal assisted bathrooms. The building itself is a large detached property set in its own attractive grounds. It is located within walking distance of the town centre of Dawlish with its range of shops, amenities and sea front. The weekly cost of care at the home is: lowest £294.00 and highest £430.00. This is an overview of what the inspector found during the inspection. This inspection was unannounced and was undertaken over 7.75 hours during July 2007. Samples of care records were examined and evidence was also obtained from healthcare professionals that provide a service to the home. Four residents were interviewed individually in private to get their views of the service they received and what life was like at the home. A complete tour of the building was completed and two carers were also interviewed as part of the inspection process. The inspection was undertaken with the assistance of the new proprietors throughout. What the service does well: What has improved since the last inspection?
The new proprietors have introduced a maintenance and redecoration programme and residents’ rooms are refurbished when they become vacant. Some electrical work was being undertaken at the time of the inspection. The approach taken to the administration records is being modified and this will focus on individual care programmes rather than a communal approach,

The home now has a complete care team and a new proposed registered manager has taken up her post. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6. Quality in this outcome area is good. Comprehensive, detailed assessments are undertaken of all new residents prior to them being admitted. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Three assessments were examined in detail at this inspection, these were comprehensive and covered residents needs in the areas of health, personal and social. Assessments were normally undertaken when a visit was made to the proposed residents prior to them being offered a place at Barton Lodge. Proposed new residents were also encouraged to visit the home to assist them when making a decision. Four residents were consulted individually as part of the inspection process and they all confirmed that all their needs were met by the service provided and that they had no unmet need. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10. Quality in this outcome area is good. Appropriate care plans were in place for each resident and these covered all a residents’ needs well. Residents that are able can self medicate subject to a satisfactory risk assessment process. Service users felt they were respected and that their privacy was maintained at the home. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Three service user plans were examined in detail at this inspection; these had been constructed from assessments that had been undertaken on the individual residents. All residents’ needs were clearly recorded in the areas of health personal and social. Four residents were consulted individually during the inspection process and they all confirmed that their needs were met by the service provided. Care plans and the integral risk assessments are reviewed on a monthly basis or more often if required. A new care planning process is currently being introduced which will improve communication further and ensure that the right service is provided to the individual concerned. Residents that are able and wish could self medicate at the home, this is undertaken subject to a risk assessment process to ensure they have the

The home administers medication to the persons that are unable to self medicate and a complete set of records was available and appropriate secure storage was in place. Four service users were consulted in private, individually during the inspection and all residents were well able to express their views. Positive comments were made concerning the service provided at The Lodge. One resident said, “I’m very well looked after, the manager has taken the trouble to explain to me the issues about my digestion nobody has ever bothered before” Another resident remarked, “ The care team is wonderful nothing is too much trouble.” All the residents consulted confirmed that they were always treated with respect and care was taken to ensure their privacy. Staff always knocked before they entered a resident’s room and personal care was undertaken in residents own rooms. This was also confirmed from observations made on the day of the inspection. Three healthcare professionals were consulted as part of the routine inspection process and they all advised that they had no concerns about the service provided in the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15. Quality in this outcome area is good. There is a high degree of satisfaction expressed by residents concerning the lifestyle in the home, they felt all their needs were met. They are able to maintain contact with family and friends and are assisted to make real choices about their life. A wholesome balanced diet is provided. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Four residents were consulted individually and others in small groups during the inspection, they all advised that they enjoyed life at the home and that they liked the facilities provided. Some of the activities undertaken at the home are: accompanied walks, a local club, reminiscence sessions, music and bingo. Some residents go to church and services are also held in the home. The social life of the home will be developed further when the new care planning process currently being introduced is in place. The home has an unrestricted visiting policy and procedure; visitors are made welcome and they are able to have a meal with their relative/friend in the home for a nominal fee. It was very clear from observations made during the inspection that time is taken by carers to give residents room to make their own decisions. Residents

The home currently operates a three-week menu system that is to the liking of residents. Residents are asked daily what they would like to eat and changes to the menu are always possible. A nutritious meal was served on the day of the inspection, which was much enjoyed by residents. Residents made unsolicited positive comments about the food, one remarked, “I really like the food here, it’s as good as I could do” Meals are usually taken in the dining room. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18. Quality in this outcome area is good. Residents are confident that if they raised an issue a resolution would be found quickly to their satisfaction. Service users are protected from abuse. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The residents consulted were very confident that if they raised an issue with the management it would be taken seriously and a resolution found quickly to their satisfaction. This level of confidence was clearly stated with regard to the new management of the home. There are no complaints currently being processed and none were made during the inspection. The home has an appropriate adult protection policy and procedure that meets the legislation and carers are trained in its use. Two carers were interviewed individually in private as part of the inspection, they both were aware of the procedures, the different types of abuse and what action should be undertaken if abuse was discovered in the home. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26. Quality in this outcome area good. Barton Lodge is a well-maintained home that has a safe, comfortable environment for residents. The home is pleasant and clean throughout. This judgement has been made using available evidence including a visit to this service. EVIDENCE: A complete tour of the building was undertaken during the inspection and all rooms were seen. The home was well maintained and all residents’ rooms were well decorated to the residents’ choice and demonstrated personalisation. Bedrooms are refurbished as they become available. All the residents consulted were very happy with their rooms and the facilities of the home. The building was clean throughout and high standards of hygiene were evident. No malodour was present. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 and 30. Quality in this outcome area is good. The care team at the home are well trained, competent and experienced and are employed isn sufficient numbers to appropriately meet resident needs. The recruitment policy and procedure undertaken by the home ensures residents are safe. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The staffing levels at the home remain at their previous level this ensures that residents’ needs can always be appropriately met. Overnight two carers sleep in on the premises. All the residents consulted during the inspection were most complimentary about the care they receive and their carers. A new proposed registered manager has been taken on at Barton Lodge and will be undertaking the registration process in the near future. Three personnel records were examined at the inspection, the home ensures that it obtains two references on new carers and carries out appropriate checks to ensure residents are safe. There is an active training programme in place at the home to ensure that residents’ needs are met appropriately. Two carers were interviewed as part of the inspection process, they were knowledgeable and were conversant with the home’s policies and procedures. They advised that they were happy working at the home and wanted to ensure that the residents’ received a good service.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35 and 38. Quality in this outcome area is good. The Lodge proprietors who are managing the home currently are fit to be in charge, of good character and able to discharge their responsibilities appropriately. Residents benefit from the leadership and management provided. Residents’ financial interests are appropriately safeguarded and the health and safety and welfare of residents and staff were given appropriate priority. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The Lodge is run by an experienced person, who is also a qualified nurse with a current pin number; she is of good character and fit to be in charge. The management processes of the home were discussed at length again at this inspection and it was understood that administration recording systems are currently being developed further to provided more appropriate tools to provide a good quality service to the residents. A new member of the care

Currently the home does have a quality assurance system in place to ensure the wishes of the residents are known and they receive the service they want. All the residents consulted during the inspection advised that they were very happy at the home and could not think of any improvements that could be made. The home does assist some residents with the administration of their pocket monies and three residents’ records were checked and were correct. Two staff signatures are used to record all transaction undertaken for residents, this ensures that they are adequately protected. Health and safety issues are seen as important by the management and are given appropriate priority. The recording undertaken of the fire precautions were examined and were complete; the regulations concerning harmful chemicals and the reporting of incidents in the home were in place. Electrical appliances were tested to ensure they were safe and the management is currently obtaining an electrical installations certificate for the building.
