The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home provides a comfortable, homely and friendly atmosphere. Under Mrs Lew Kum Hoi`s management and leadership. Staff work hard to meet the needs of the residents in a caring and professional manner. Staff are friendly, approachable and helpful and show respect for residents. Records are clear and well organised. A variety of good home cooked and nutritious meals are served.

What has improved since the last inspection?

The requirements of the fire service have been met and a fire safety risk assessment is now in place.

What the care home could do better:

A number of residents records did not have photographs attached and care should be taken to ensure care plans reflect and record the changing needs of residents. Some items of furniture needed cleaning and some items of bedroom furniture were broken. Staff should be reminded of when and how to report adult protection issues. All staff should follow safe procedures and record temperatures before bathing residents.

Date of last inspection 13/1/05 Brief Description of the Service: Belle Vue Country House is a Care Home registered to provide 41 places in the category of Elderly Mental Disorder and Dementia (including 3 places for 50 ). Belle Vue is an adapted and extended two-storey house, standing its own extensive grounds near the village of Warninglid. The home is situated within a rural location in reach of the A23 and there is a bus service that stops near the home. Accommodation is provided on two floors with a lift connecting the two floors. The home currently has 29 single rooms and 6 shared rooms. The home has a extensive gardens around the home and an enclosed patio area. This announced inspection took place on June 15th 2005. Prior to the inspection the inspector looked at previous reports and reviewed letters, reports from the home and complaints held on file. Information included in reports will be collated over time and will not just be a picture of the home on the day of the inspection. A pre inspection questionnaire was completed by Mrs Lew Kum Hoi the Registered Manager and returned to the Commission in good time for the inspection. Satisfaction survey forms were returned to the Commission on behalf of 17 residents and 10 were returned from relatives or representatives. All were positive about the home and the staff. Comments from relatives included “They were very pleased with all aspects of Belle Vue”, “staff were always friendly and helpful” and “we count ourselves lucky in finding such a nice home”. A general tour of the home was undertaken and all resident’s rooms visited. Case tracking from records, to care given was carried out for a number of residents. The recruitment records of new staff and relevant records relating to the management of the home were also examined. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5 The home has a good pre assessment and assessment process, relatives and their representatives are able to visit to ensure the home will meet their needs. Residents are given a written contract with the terms and conditions of the home. EVIDENCE: The majority of resident’s records comply with legislation requirements and are clear. Photographs are still needed for a small number of residents and Mrs Lew Kum Hoi stated that they have been taken but not printed. Care needs documented can be tracked from the assessment process to the care plans. The Statement of Purpose, Service User Guide and previous inspection reports are available in the hallway of the home. A notice of this inspection was posted on the main notice board. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10,11. The care planning system is clear and in the majority of cases provides care staff with the information they need to meet resident’s needs. Staff practices respect the privacy and dignity of residents. EVIDENCE: Care plans are compiled from the assessment process, residents and relatives are involved and they are reviewed regularly. The majority were in order, a small number of those examined were not consistent with the present condition of the residents although staff when they were spoken with were aware of the changes and were caring appropriately. A key nurse system is in operation which provides continuity for the residents. Staff were observed directly and indirectly and were noted to speak to residents in a caring and respectful manner. One relative said that “staff were always pleasant and helpful” and another said, “staff take great care of all residents care needs”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 Activities are provided to meet the needs of residents and residents maintain contact with family and friends. Nutritious home cooked food is available. EVIDENCE: The residents are mentally frail and a number immobile. Activities such as word games, quizzes, sing-along, musical bingo and arts and crafts are provided to meet the varying needs of the residents. Trips out of the home are organised and a BBQ is planned for the near future. A patio area with garden seats to the side of the house is available for residents and an aviary with birds is in that area. Relatives said that they are always made welcome by the staff when they visit. Two lounges and one lounge/ dining room are available for residents to use. Televisions are also in many of the resident’s rooms although a number of them could not be connected to a power point or an aerial. When asked, Mrs Lew Kum Hoi stated that they residents who could operate a television did have a working one in their rooms and the ones that were not working were the property of residents and were there at the request of relatives. Residents enjoyed their meal of Irish stew during the inspection; one resident was having cheese on toast at his own request. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18. The complaint procedure is clear and enables residents, relatives and representatives to be sure that their complaint would be taken seriously and acted on within an appropriate time scale. Staff did not follow the adult protection procedure following the latest incident. EVIDENCE: The Commission was informed by the home of two complaints that have been made since the last inspection. These were both referred to Social and Caring Services under the Adult Protection Guidelines. Social and Caring Services were satisfied with the investigations carried out at the home. One relative did say that they were satisfied with the open and positive way the home conducted one investigation. Since this inspection was carried out another adult protection alert was reported by the home to Social and Caring Services and is presently being investigated. Unfortunately two members of staff did not follow the procedure which resulted in the resident not getting the prompt treatment they needed. Mrs Lew Kum Hoi has taken immediate action to remind staff of the procedure to follow. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25,26. Residents live in premises that are homely, well maintained, comfortable and clean. Safety checks on equipment and training for staff should ensure residents are safe at all times. However procedures were not followed in the latest incidence, which resulted in the adult protection alert. EVIDENCE: Residents are provided with a well-maintained, homely, and comfortable environment in which to live. All areas visited were clean and tidy and residents are able to furnish their rooms with their own possessions to make them more homely. Residents are able to use a large dining/ lounge and a rear lounge on the ground floor and another lounge on the first floor. The lift was in working order and the call bell system was noted to be in working, residents who could speak to the inspector said that staff respond promptly to the calls. The Inspector did discuss some minor issues relating to an airflow mattress, cot sides and some bedroom furniture that is in need of repair with Mrs Lew Kum Hoi at the conclusion of the inspection. The latest incident involved a thermostat which had been altered on a bath. Mrs Lew Kum Hoi has now taken action to ensure the thermostats cannot be altered. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29,30. The procedures for the recruitment and training of staff in the home are robust and do offer protection to residents living in the home. Staff are employed in adequate numbers for the needs of the residents. EVIDENCE: Staff training has been undertaken and evidence showed that staff are offered training appropriate to their work and the resident’s illnesses. Duty rotas are available and showed sufficient staff with the appropriate qualifications are employed over the 24-hour period. The recruitment procedure and induction process in operation is good, with all required safety checks being made for new staff when they are employed. Induction training is carried out and staff are encouraged to undertake a National Vocational Qualification while employed at the home. Staff when spoken with were positive about the home and the ongoing support they get for their work and for training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,36,37,38. Belle Vue Country House provides an open environment for residents to live in and staff are trained to undertake safe working practices to ensure the safety and welfare of residents. However one incident has highlighted more training is needed for a number of staff. The record keeping system safeguards resident’s best interests. EVIDENCE: Mrs Lew Kum Hoi is the Registered Manager and under her leadership the home runs well and the staff supported to carry out their work. It is apparent that the home is run in the best interests of residents. Staff training in health and safety issues is provided and clear records are available. Accidents are recorded by staff and any follow up action recorded. An accident that has recently occurred in the home was dealt with according to the adult protection procedure as soon as Mrs Lew Kum Hoi became aware. This was reported to the Health and Safety Executive and an investigation by Social and Caring Services is underway.
