First registered in 1971, Crossways Community is a Christian Charitable Trust. It provides a high standard of care, support and accommodation for 16 adults aged between 18 and 65 who have or are recovering from a mental health problem. Based on Christian values and community spirit, it provides a progressive service that is evolving and developing to meet the current and future needs of the residents. Crossways Community is a detached property with gardens and car parking facilities to the side of the building. The home is located in a residential area of Tunbridge Wells, close to shops and public transport with other town amenities a few minutes walk away. Accommodation is over three storeys. There is no shaft lift. The home has sixteen single rooms, all with en suite or designated facilities and television points. Crossways senior staff team includes the registered manager and care coordinator. The home employs staff, working a roster, which gives 24 hour cover. Residents and staff share the domestic duties as part of their programme towards more independent living. The staff who work at Crossways are Christians and their faith forms the basis of the philosophy of care offered within the home. The inspection report is available to residents. What the care home does well People thinking about moving into Crossways are told beforehand what it is like to live there. They have the opportunity to visit and stay before they move in to see what support they need and to make sure that the home is suitable. Crossways is friendly and relaxed. The manager runs it well and service users have the opportunity to participate in this. Service users are at ease with staff who listen to their views and concerns. Service users benefit from a comfortable, well maintained and attractive environment. Service users are supported to make independent decisions and choices. They have a good choice of activities and occupation for leisure, work and independent living skills. People are able to see their family and friends if they wish. People who live at Crossways benefit from the support of staff who are enthusiastic and committed to promoting their welfare. Care plans ensure staff are clear about the support that people need. Service users social and health care needs are supported. Service users are protected through staff recruitment, training and supervision procedures. There are enough staff to support people when they need it. What the care home could do better: Care plans would reflect peoples changing needs better if dates on all risk assessments were changed every time they were reviewed; mobility guidelines with specific moving and handling assessments were recorded more promptly where necessary. The manager said that they would confirm some medication administration procedures with guidance from the Royal Pharmaceutical Society. Care Homes for Adults (18-65 years) Page 3 of 10 What we found: This Random Inspection was undertaken on 19th April 2010. This report should be read in conjunction with two previous Annual Service Reviews dated 30th April 2009 and 28th April 2008 together with the last Key Inspection report of 20th April 2007, which looked at all core standards. The quality rating for this service is 3 star. This means the people who use this service experience excellent quality outcomes. The focus of this Random Inspection is to assess how the home continues to provide an excellent quality service. This Random Inspection included a visit to the home in Tunbridge Wells on 19th April 2010, where the Registered Manager, the Administrator, six support workers and seven Service Users were spoken with. Some judgements about the quality of the service provided were taken from observation and conversation. Some records and documents were looked at together with the premises and facilities of the home. All of the above have been included within this inspection process and some of the gathered evidence is mentioned within this report where appropriate. During this Random Inspection of 19th April 2010 we found the following: Written information about Crossways is available for people who are thinking about moving in, including a service users guide. There is a comprehensive admission process. Before they move in, people are assessed by the home to make sure that staff can meet their needs. Assessments are recorded and care plans for the support that people need are developed from these. There is a system of trial visits and stays in the home. Three care plans were looked at and discussed with the Manager. The care plans seen all contained up to date holistic information, which records individual personal, health and social care needs. Files were clear, easy to read and follow. Information is divided into risk assessments, mental health, physical health, finances, independent living, medication, reviews, evaluations, additional information and consent forms. All care plans and risk assessments are reviewed regularly. The manager assured the inspector that dates would be changed on some risk assessments to reflect current information. Discussion took place regarding any mobility problems and staff were aware of the guidelines to follow. The manager stated that detailed mobility guidelines were in the process of being recorded and would be completed shortly, together with the development of a specific moving and handling assessment where necessary. Service users are supported to make independent decisions and choices. Service users participate in regular resident meetings and discussion groups. They have the choice to be involved in the daily running of the as far as possible. Service users have a structured day with jobs in the community or in house. Day Care Homes for Adults (18-65 years) Page 4 of 10 activities are planned with their key worker. Crossways employs three designated life skills staff who support service users to be independent. Service users are encouraged to attend college courses, find work placements and other community based activities such as computer, literacy and numeracy skill courses. Currently five service users work as volunteers in local health care facilities and charity shops, whilst others attend adult education. Service users artistic talents are encouraged. Some enjoy art, singing, upholstery and a creative group. The chapel room has a piano. There is a TV, computer, DVD player and various games and sports equipment. Following risk assessment, service users are free to come and go as they please. They have their own key to access the front door and their room. Service users go out walking, to the gym or shopping, sometimes accompanied by a member of staff or as a group. Chapel time is available twice a day for those who wish to participate. Service users have the opportunity to go away on holiday. In addition an In House holiday is held during which numerous outings and activities are organised. Visitors are made welcome and encouraged as part of the overall ethos of the home as a community. Service users can see visitors and family members if they wish. Service users have considerable choice. Once a week they meet to decide what to eat and shop and cook together. Every two weeks there is a service users meeting. Service users are encouraged to pursue their own individual interests in addition to group activities. Personal support takes account of individual service users preferences. All service users have a designated key worker. Service users are supported to attend appointments with social and health care professionals when necessary. Records showed that service users health is monitored. Medication is managed by the home. The home has a medication policy and procedure in place, which aims to protect all service users and ensure the safety of those self medicating by using risk assessments where necessary. The manager stated that they have previously sought guidance from the Pharmacy Inspector and health care professionals about any secondary dispensing regarding medication that cannot be provided by the pharmacy in a monitored dosage system for those service users who self medicate. The manager stated that they would confirm this using guidance from the Royal Pharmaceutical Society to confirm correct procedures. Service users are at ease with staff who listen to their views and concerns. The home provides a written complaints procedure. A record of all complaints and the action taken is kept. This has the facility to record minor niggles in addition to formal complaints. The manager demonstrated a good understanding of safeguarding procedures. All service users have their own bank accounts and keep their own money. There are lockable facilities available. Some service users are supported by the home to save for their holidays. All cash kept on behalf of service users by the home is stored and recorded appropriately. Service users benefit from a comfortable, well maintained and attractive environment. All Care Homes for Adults (18-65 years) Page 5 of 10 areas visited were well furnished and decorated, bright and spacious and clean and tidy. There are two lounges, two dining rooms and a chapel.quiet room. The chapel room is used twice a day by staff and service users, if they wish, for prayer, singing and reflection. Two kitchens are provided, one of which is shared by service users. Service users rooms were not visited on this occasion but all spoken with indicated they were happy with their room. All rooms with the exception of two have ensuite facilities. The remaining two have their own designated bathrooms. There are also shared facilities. Currently building work is underway to provide a new administration block together with a conference room and quiet offices. The gardens are extensive and include a water feature, private spaces and a summerhouse. There a number of allotments and a greenhouse for residents use. At the time of this visit service users were enjoying working in the allotments, where they are growing fruit and vegetables. People who live at Crossways benefit from the support of staff who are enthusiastic and committed to promoting their welfare. Staff spoken with were clear about their roles and demonstrated a good knowledge and understanding of individual service users needs and wishes. A good rapport was observed between service users and staff, who were attentive and respectful during this visit. Service users are protected through recruitment procedures, which aim to appoint suitable staff to meet their needs. Records examined showed appropriate pre employment checks such as written references and a criminal records disclosure. There were sufficient staff on duty to support service users at the time of this visit. A range of staff training is provided. Records are kept individually and in a training matrix. Staff spoken with confirmed courses as including food hygiene, health and safety, mental health and the mental capacity act. Staff are provided with National Vocational training at level 3. Staff spoken with enjoyed working at the home and felt well supported by the system of supervision in place. Crossways registered manager is highly experienced and has a management qualification. A good management structure is in place. There was an open and friendly atmosphere in the home and the people who live there were relaxed, spending time with the staff and manager. The views of service users are sought about the support they receive; they are involved in the running of the home and in staff recruitment; regular meetings are held. Surveys are carried out three times per year. A representative of the Trust visits monthly. Policies and procedures are regularly reviewed and are available to service users. Records seen indicated the testing and maintenance of systems and equipment within the home. Accident and incident reports are completed appropriately.
