Conditions of registration: Date of last inspection 23rd June 2005 Brief Description of the Service: Braeburn House is a care home providing personal care and accommodation for 40 older people. It is located approximately four miles from the centre of Scarborough. It is close to shops, the post office and other amenities. The home was opened in 1968 and consists of a three-storey building. The second floor is used as a separate day centre. The ground and first floor are used for the residential section of the building. All service users have their own bedroom. Two of the bedrooms can be used as double rooms if two people wish to share. There is a passenger lift. The home has grounds to the rear that are easily accessible. This is an overview of what the inspector found during the inspection. This inspection including preparation time took place over 8 hours. A tour of the building was conducted. A number of the service users records, staff records and records about the management of the home were inspected. Service users, and staff on duty were spoken with. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 The service users can be assured that their care needs will be met. EVIDENCE: The service user files seen contained copies of an assessment by a care manager. The duty manager confirmed that one of the management team visits all prospective service uses before their admission. A record of their assessment is kept in the service users file. Service users spoken with confirmed they had seen someone from the home prior to their admission. Braeburn House does not provide intermediate care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 Service users are treated with respect and their privacy is respected. EVIDENCE: The service user files seen contained information for the staff on how they liked to be addressed and any special needs they had. This information enables the staff to approach each service user as an individual. Staff were observed treating the service users with respect at all times. They were seen to knock on bedroom doors before entering, calling the service users by their preferred name and speaking to them in a quiet and calm manner. The service users said that staff always treat them with respect and try to preserve their dignity at all times. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 and 14. The service users are able to keep their own routine within the home and join in with social activities EVIDENCE: Service users spoken with said that they were able to follow their own routine within the home and that staff were supportive of them. During the inspection service users were seen coming and going from the building at will. Staff were knowledgeable about the service users routines and encouraged them to keep them. Regular social activities are organised and the service user were looking forward to the bingo night where members of the local community attend. The activities are advertised on the notice boards and in the quarterly newsletter produced by staff. Visitors spoken with said that they are always made welcome and there are no restrictions as to when they visit. The service users confirmed this was the case. One visitor said that the managers were good at keeping them informed about events affecting their relative or generally what’s going on in the home. A visitors’ policy is in place and all visitors sign a visitor’s book on entering the home. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Service users are protected from abuse through staff training and rigorous employment policies. EVIDENCE: There is an Adult Protection Policy in place. Staff spoken with had received training in Adult Protection issues and they had a good understanding of their responsibilities if they believed any inappropriate behaviour was taking place. Policies are also in place regarding the management of service users money. Staff are also subject to a criminal records bureau disclosure and a protection of vulnerable adults check to ensure that they are suitable to work in a care setting. JUDGEMENT – we looked at outcomes for the following standard(s): 27 The current staffing levels needs meet the service users. EVIDENCE: Whilst there has not been an increase in the staffing levels the management team use the admissions process to ensure that the current staff group are able to meet the service users needs. The staff spoken with said that the management approach meant they were able to provide a good service. The service users spoken with said that the staff always responded to requests of assistance promptly. JUDGEMENT – we looked at outcomes for the following standard(s): 33 The service provided is monitored through a quality assurance programme. EVIDENCE: North Yorkshire Social Services has introduced a quality assurance programme. This involves the home assessing the service they provide against the National Minimum Standards for Older people every three months. This information is then used to develop an action plan to improve services. Service user meetings, staff meetings, team briefings, and the quarterly newsletter supplement this document.
