The home was randomly selected for a visit by a CSCI pharmacist inspector to look at the arrangements for handling medicines. The visit lasted four hours and involved speaking with the manager and deputy manager and looking at the arrangements for the storage and recording of medicines. We gave detailed feedback to the manager at the end of the visit. Overall we found some shortfalls in medicines handling that need to be addressed to make sure medicines are handled safely. We found the medicines stock and records to be well organized. However when we checked the records of medicines brought into the home we found them not detailed enough to allow accurate checks to be carried out. Records of medicines given were not always complete because a clear record was not always made when people were not given their medicines, for example, if they were asleep or refused them. Some records were not accurate because the exact dose of medicine given was not clearly recorded, notably for medicines with a variable dose. We also found medicines for external use such as creams were not well recorded because the records were not kept in the rooms with the creams. Records of medicines disposal were not always complete for medicines that had been refused because the key code on the records was not used correctly. Our checks on the medicines stock and records showed most medicines had been given correctly as prescribed. However we found the times medicines were given were not always correct particularly for medicines that needed to be given before or after food. Giving medicines at the wrong time can stop them working properly. We looked at a sample of care plans and records and found they lacked information about how medicines prescribed as when required were to be given. We saw this with medicines used for anxiety and sleeping. One care plan for a medicine used to treat agitation did not state the dose and under what circumstances it was to be given and another was not clear when to omit the dose. A diabetic persons care plan did not have enough detail about how to use their insulin and how to monitor their blood sugars. We discussed this with the manager who agreed that some care plans lacked detail and she said that these would be reviewed and updated as soon as possible. This is important to help make sure people get their medicines when they need them. We looked at how medicines were stored and found the rooms and trolley to be clean and tidy. However on the day of visit the medicines room was left unlocked and unmanned whilst the medicines were being given out to people, which means some medicines were not kept safe. The fridge used for storing medicines was not lockable and contained several medicines that did not need to be kept cold. We checked how controlled drugs (medicines that can be misused) were stored and recorded. The cupboard was secure and a suitable special register was used for record keeping. However, not all entries in the register were properly witnessed by two staff. On the previous day one persons strong pain killer had run out of stock so staff had used someone elses, the manager said they had not ordered it in time so it was the only way to make sure this person got their prescribed dose. Using someone elses medicines is a breach of the law and could lead to misuse or mishandling when Care Homes for Older People Page 3 of 8 controlled drugs are involved. We looked at how medicines were checked and audited. The manager said some informal checks were carried out throughout the month but no formal recorded audits were made. Recorded audits and checks help make sure medicines are handled safely.
