What the care home does well

The manager works hard to ensure the residents who come to stay at Carr Manor feel welcome, safe, and confident that staff will give the necessary help with their care in a professional and friendly manner. She has a good knowledge of all the residents in the home and is well supported by her staff. There was a good level of verbal interaction between staff and residents. Staff stated they enjoyed working in the home and found the Manager very approachable and supportive.

What has improved since the last inspection?

Staff training continues to improve, care workers spoken to confirm they had training in first aid, manual handling and crisis intervention.

What the care home could do better:

Further developments are still needed, to ensure consistency is applied to the care plans and to provide sufficient detail to inform the care staff how people wish to receive care. They should always be updated to reflect changes identified. Some minor maintenance work is required as described in the body of the report. More thought should be given to introducing handrails to support residents to maintain their independence when accessing the back garden.

The care and services are provided by Community Integrated Care which is a registered charity caring for people with special needs. Mrs Sharon Hutchinson manages the home on behalf of the latter. Carr Manor is a semidetached house located within 2 miles of Leeds City Centre and is at the end of a quiet street. The home has been open since August 1996. There is a wide range of shops and leisure facilities within easy reach of the property. The home has 5 bedrooms. All are single rooms with wash hand basins. There are two fully equipped bathrooms comprising a bath, shower and toilet. All bedrooms are decorated to individual taste and choice. Service users are encouraged to buy personal items for their own use, which helps to create a homely atmosphere within the establishment. Service users are encouraged to make full use of the entire house and garden and the facilities within, including television, video and music system. This was an unannounced inspection carried out on the 20th September 2005 by one inspector. The purpose of the inspection was to ensure the home continues to provide a good standard of care for its residents. The Registered Manager was not on duty so the officer in charge made her self-available throughout the inspection. Service users and staff present at the visit were made aware of the inspection through discussion with the inspector. They were invited to give their views on the care and services provided. Records were inspected and these included residents care plans and policy documents. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5. The home produces up to date written information for residents and relatives about the home. This enables residents, relatives and friends to know their rights and obligations at Carr Manor. The home only admits a resident when a proper assessment has been undertaken. As much information as possible is obtained from health, social services and family about the care needs of residents before and on admission to the home. EVIDENCE: Statements of terms and conditions are available in each resident file. However residents were unable to say if these have been discussed with them because of their disability. The officer in charge confirmed that pre admission assessments are always completed before any resident moves into the home. This was evident from information in resident’s files. The care plans had information from external professionals, which enable the home’s staff to complete a relevant plan of care for residents. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8,9,10. Because of residents communication and sometimes-physical disabilities staff help resident choices by skilled observation. The health care needs of residents are met. Residents are treated with respect and their privacy is upheld. The care plans inspected were not easy to follow, there were no consistency and were not always updated to reflect changes identified. EVIDENCE: Through case tracking care plans were examined and their use was also discussed with the officer in charge. The layout of one care plan made it easy to read and follow. It showed clearly the action to be taken by staff to meet the needs of the resident. Other care plans available were, bulky and some information was difficult to find. It was difficult to find details of the residents’ personal preferences such as appearance, getting up and going to bed times, favourite types of music. One other file did include some specific guidance for staff on how to handle a resident seizure. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,14,15,16,17. Staff recognises that time and support for residents outside the home is an important part of their life. Local facilities are regularly used and all residents are encouraged to maintain links their families. The home encourages all residents to become involved in menu planning with individual choice. EVIDENCE: Information from resident’s files and staff spoken with shows the home encourages residents to visit local amenities for shopping trips, meals out and walks. Some residents attend the Gateway club or some form of college. One resident said they enjoyed the food at Carr Manor. Comments included “ I say what I want ” and “food nice”. Staff and residents participated in verbal banter and the social atmosphere was good throughout the day. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20,21. Residents’ involvement in the care documentation is limited because of their learning disability. Medication is well managed and external professionals are well used for any identified health needs. EVIDENCE: The home has a policy available for dealing with resident’s medication. Senior care staff are responsible for the administration of medicines and the manager has responsibilities for monitoring the drugs administration system. All medication records were correctly completed each day clearly indicating the initials of the member of staff administering the medication. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22,23. The home provides clear information on how to make a complaint about the service. It includes reference to the Commission for Social Care Inspection if people wish to take a complaint outside the home. Staff had an understanding of adult protection, however they must have adult protection training if they and residents are not to be at risk. EVIDENCE: The appropriate policies and procedures were seen to be in place with all the relevant details. Talking to staff on the day it was evident they are all clear on their responsibility about reporting any allegations of abuse. However from checking the training records there are some staff that who have not yet had adult protection training to ensure they and resident’s are not at risk. The home has not received any complaint since the last inspection. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,26,28,29,30. The home provides a safe environment for residents and staff. Physical standards within the home appear to be adequate and comfortable for residents. The individuality of each resident is promoted. Redecoration is required to two areas of the home where water damage has taken place, the hall ceiling and one resident bedroom. The bidet in the bathroom requires attention and residents would benefit from handrails to support them to gain access independently to the garden. EVIDENCE: Residents’ bedrooms meet their own needs, with all having single accommodation which meets minimum space. Their rooms are tastefully decorated are comfortably furnished and meet individual resident needs. The home provides appropriate communal space of a good standard for residents for dining as well as recreational purposes. Residents are able to meet with visitors in private in their own rooms, or communal areas if they wish. Talking to staff and looking at the movements of residents it was evident that residents would benefit from some form of handrails at the rear of the building to promote independence. With the exception of redecorated to the water damage in the hallway and one bedroom the decoration of the home is good. The policies for infection control are regularly updated. The home appeared safe from hazards that may put residents at risk. JUDGEMENT – we looked at outcomes for standard(s) 33,34,35. The home has a robust interview process when recruiting staff and training is now high on the home list. EVIDENCE: Several staff recruitments files were looked at. References were in files and Criminal Records Bureau checks were completed. There was three care staff on duty. All were very pleasant and appeared to be communicating well. A care worker confirmed she had done training in first aid, manual handling, and food hygiene and crisis intervention. The home holds regular meetings the minutes of the last one showed discussed were residents’ issues, staff issues and health and safety issues. Discussion with staff led to the conclusion that staff have a good knowledge of individual resident and seemed to understand the importance of encouraging residents to maintain their independence and dignity. JUDGEMENT – we looked at outcomes for standard(s) 37,38,3942,43. The manager is highly respected, hardworking and committed to providing a quality service. EVIDENCE: The minutes of meetings held with the staff in the home showed that she was dealing with situations where staff were forgetting to put the needs of residents first. People described her as approachable and were confident that they could bring any concerns to her.
