Conditions of registration: 1. AGE RANGE: The age range to be accommodated is 18 - 65 in respect of those service users in the category MD. 2. CATEGORY MD(E): Of the 11 (eleven) service users accommodated at the home 2 (Two) may fall within the category MD(E) - over 65 years of age. Date of last inspection 06 December 2004 Brief Description of the Service: Cherry Trees consists of two victorian houses that have recently had a new corridoor build that now links them both. The new development also provides a pleasant entrance lounge, office and aditional rooms. Cherry Trees is situated on a busy road that is part of a one way system close to the approach of Woking Town Centre. Metered parking is available outside the home and there is limited parking space inside the gated entrance. Cherry Trees is registered to accomodate eleven service users with a mental health needs. The aims of the home are to provide its service users the opportunity to work towards independent living. Both homes have adequate lounge and communal areas and a main kitchen and dining area is situated in one of the houses. Bedrooms are of various sizes and some bedrooms have been fitted with ensuit shower rooms. The new building povides adittional storage and a smoking room that over looks a spacious and well kept garden. A small workshop has also been been built in the garden and offers the service users a place for training.

This is an overview of what the inspector found during the inspection. This was the first inspection of two to be undertaken in the Commission for Social Care Inspection Year April 2005 to March 2006. It was an unannounced visit and took place over a period of 4½ hours. Lead Inspector Damian Griffiths was assisted throughout the inspection by the Registered Manager Mr Gengasamy and a member of staff. The inspector was able to talk to the six service users who are currently staying at Cherry Trees. They were able to contribute to the inspection report and show the inspector around the home. The areas of inspection included: service user assessments, care plans, freedom of choice, involvement with the home, medication administration, management and staffing. The services users consulted said they regularly visited the local shops cinema and leisure centre. They enjoyed the work experience offered at Cherry Trees and spoke of how they had been able to help with the building of the new corridor extension. One services user working in the large rear garden spoke highly of the home and his enjoyment of working in the gardens, i.e., flowers in front and rear gardens were all grown from seed. Another services user was pleased he had attended a recent French Polishing workshop. The inspector would like to extend his thanks to the service users, staff and management of Cherry Trees for their assistance and hospitality. It is recommended that the reader should also look at the previous report that can be accessed by using the CSCI website details on the last page of this report. What the service does well:
Cherry Trees is able to provide services users with a comfortable, safe and homely environment that is able to meet their care needs. It offers new service users, health care practitioners and families the opportunity of discussing concerns within a flexible timescale. The home has a good working relationship with mental health care practitioners that support the service users. The availability of regular reviews of health care needs known as the ‘Care Plan Approach’ plays a major role in the service users life and forms an essential contract between the home and the service user.

The home has been able to provide successful outcomes for three former service users who now live independently. What has improved since the last inspection? The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2.3 and 4. New service users to Cherry Trees will receive a comprehensive assessment of their needs. The services users benefit from a person centred approach to care planning. Due to the nature of the service offered at Cherry Tree’s, prospective services users benefit from flexible timescales for admission. EVIDENCE: Samples of three assessments were inspected and they all contained comprehensive details of services users needs. These included assessments from the health practitioners, risk assessments, sleeping patterns and personal care needs. Prospective services users were fully involved with the assessment process and record of the aspirations and needs of the service users were recorded. The admission process is flexible in order to allow for the assessment and care plan to be completed and agreed by the services user and the home. There is no set timescale for the completion of the process. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6 ,7 and 8 and 9. Services users take full and active role in devising their care plan needs and they were supported to make choices and decisions. Service users at Cherry Trees are encouraged to participate in the running of the home. EVIDENCE: Services users fully participate in the development of their care plan and how it is to be delivered, i.e., details may include regular visits to the GP surgery or work experience. The aim at Cherry Trees is for full integration into the community at the service users own pace. This involves the service users; receiving support, obtain the skill needed to be able to take charge of their own care needs. The care plan is regularly reviewed and once agreed the service users confirm the agreement by signing their respective care plans. Samples of risk assessments included detail of service user ability to self-medicate. An example of the equity at the home was well demonstrated by the inclusion of service users within staff meetings and within the interviewing of staff process. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,14,15,16 and 17. Family friends and personal relationships are encouraged and supported as appropriate. Services users can exercise personal choice within the home and local community and chose what meals they will have. EVIDENCE: Service users in contact with family are supported to maintain links. Family members are welcomed and were part of a network of support available at Cherry Trees. Services users without family support could obtain the support of an independent advocacy service. Service users consulted said their privacy is respected and confirmed that they had their own front door key. There were rules in place to support individual privacy. Service users wishing to bring home friends can do so but only after this has been agreed within the care plan review process and after consultation with other service users. The service users informed the inspector that they were confident with the process.

Service users are encouraged and supported to plan and cook their own meals and advice from a dietician was available if required. Service users take responsibility for their own laundry and it was clean and tidy at the time of the inspection. Service users are able to take advantage of a smoking room that was part of the new extension. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 20. Service users are encouraged and supported to self medicate after an assessment of risk has been completed. EVIDENCE: There are services users that can self medicate and some that required the input from the local Health Centre for regular administration of medicine. Cherry Trees follows the appropriate procedure for the distribution of medication and ensures that the service users have undergone a risk assessment before being allowed to self-medicate. They are closely monitored for any signs of difficulty and care plans are reviewed regularly. Files were inspected and all staff involved with medication administration had received training in safe handling of medication. Specimen signatures were in place and policy and procedures were evident. The administration of medication was inspected and in order. The inspector was informed that a new blister pack system is to be introduced. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 25 ,26,27 and 30 Cherry Trees provides a homely atmosphere and comfortable communal areas. It was clean and tidy but in need of decoration due to the recent improvements to the building. Service users benefit from improvements to the building and opportunities to develop basic painting, decorating and building skills. EVIDENCE: The new building completed in the home highlights the areas in need of improvement. The home had begun this process by stripping out all carpets to be fitted with replacement wooden floors that were planned. The service users stated that they would be willing to gain experience of painting and decorating identified in the following areas: hallways, lounge and stairs, bedrooms. The manager explained that this might form part of the service users care plan but only if requested by the service user and after the appropriate risk assessment had been completed. It must be emphasised that this was not in any way linked with the homes own maintenance schedule. A soap dispenser situated in the kitchen requires paper towel dispenser. Service users have their own bathrooms. A tap in a service users bedroom was above the required safe water temperature. The services users bedrooms were all very different and reflected the individual needs of each resident The new extension to the home provides a light and airy space to the entrance area. The two houses either side of this are quite different and in places being prepared for redecoration, overall the home was clean and tidy. The registered manager has agreed to keep a log of maintenance and repairs as recommended. JUDGEMENT – we looked at outcomes for standard(s) 31,34,35 and 36 Clear job descriptions assist staff to be aware of their role and responsibilities. The Registered Manager supports staff to gain specialist knowledge of relevance to the service. EVIDENCE: Staff have clear job descriptions and GSCC guidelines were available. Responses between staff and services users were observed to relaxed and courteous. There has been no new staff members employed at Cherry Trees since the last inspection. CRB checks of existing staff members had been completed and recruitment requirements had been followed. There was currently six full time staff available for three shifts. Staff files and training programs were inspected and showed that staff at Cherry Trees receive core training in the required areas: Food Hygiene, health and Safety, Fire safety training and PoVA training. There was also the availability of more specialists’ courses, i.e., Safe Handling of Medication, Mental Health Awareness, Behavioural, Care Planning and Risk Assessment. Staff were also encouraged to study regularly and are able take an hour a day to read periodicals or study.

The manager confirmed that he encourages staff and service users alike to consult him if they have any concerns. Cherry Trees was been awarded an ‘Investors In People Award’ in March of this year. Standard 24 Regulation 13(3) Requirement The registered person must ensure that suitable arrangements are in place to prevent the spread of infection by providing soap and paper towel dispensers in the visitors toilet and kitchens. The registered person must ensure that the home is free from hazards and ensurer that the correct water temporatures are maintained near to 43C. The registered person must ensure that the premises are kept in a good state of repair and ensure that dripping taps are repaired.
