Conditions of registration: One named person under 65 Date of last inspection 22.10.2004 Brief Description of the Service: Chesswood Lodge is registered with the Commission for Social Care Inspection to provide care for up to fourteen persons in the registration categories Mental disorder and Dementia over 65 years of age and includes one named person in the category of Mental Disorder over the age of 50 years. The property is situated in Worthing in a residential area close to a wellestablished park, local transport, railway and shops. The sea front, main shopping centre with all its amenities is approximately ½ mile away. The home is a large mature house with plenty of parking to the front and a secluded grassed garden to the rear. The accommodation comprises of 10 single bedrooms - 4 with en-suite facilities and 2 double bedrooms. There is a lift to the first floor but the home is not suitable to accommodate anyone in a wheelchair due to dimensions of the corridoors Chesswood Lodge Limited privately owns the service. Mrs Shoai, a director of the company is the registered responsible individual. The registered managers position is currently vacant. This is an overview of what the inspector found during the inspection. This was the first of the two inspections (minimum) that an inspector must make in a year. On this occasion this inspection was announced and took place over one day in May 2005. The aim of this inspection was to find out how the home cared for the residents and the residents’ opinions as to how well the home did this. To gain more information about how the home cared for the residents, questionnaires were given to residents, their relatives and others before the day of inspection. No responses from relatives or others were received, although a visitor on the day of inspection told the inspector that he had no complaints about the way the home cared for his relative. A response from an unnamed relative/visitor was received after the inspection had taken place, and suggested that there were not always enough staff on duty and that they were unaware of when an inspection was to take place or how to make a complaint. During the inspection it was seen that a copy of the notice telling of our intention to make an inspection visit was put on the notice board in the main hall along with copies of the Statement of Purpose and the Complaint procedure. A tour of the home took place. Staff and care records were inspected as well as the Home’s Statement of Purpose, Service Users Guide and some of the Policies and Procedures. All of the staff on duty, and 10 of the residents were spoken to during the course of the inspection. What the service does well:
Most of the staff members have worked at the home for a long time and the good relationships between staff members and residents were shown during the course of the care staffs’ work with the residents, with much laughter and good humour. The residents said that the staff were hard working and always willing to do anything no matter how small that might be. They also informed that they were given choice in all that they did and that there were no petty rules to observe, for example they could go to bed and get up at whatever time they wished and staff would be careful to observe their privacy at all times. Regular review of the care plans and other records helps the care staff to give individual and suitable emotional and physical support to the residents. The residents enjoy the activity programme and confirmed that the person responsible for organising activities was careful to ensure that each person was asked what sort of activity they would like to do and made sure that it was made possible for them.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2. 3. 4. St6 N/A All of the residents had their needs assessed before moving into the home and they or their representative had been given information about how they would be looked after and what services they could expect from the home EVIDENCE: The files of five of the residents were examined and each one had a written pre admission assessment identifying the emotional and physical care needs of the person and their suitability for admission into the home. All contained a copy of the contract which was explicit about what the home could or could not provide and these had been signed by the resident or by a representative if they were unable to sign, one resident was the subject of guardianship. Copies of the Service Users Guide and Statement of Purpose were available on the board in the main hall and each resident had their own copy in their bedroom along with a shortened version of their care plan. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7.8.9.10. All the information needed to make sure that the residents are looked after in a safe and effective way is recorded satisfactorily, giving the staff members the knowledge needed to give each individual person the attention that they require. Residents are treated with respect and their privacy is considered at all times. EVIDENCE: If possible the residents care plans are drawn up with the person concerned or a relative/ representative of theirs. They are very detailed and reviewed on a regular basis, details of changes are regularly included and all of the care plans seen on the day of inspection accurately reflected the person’s mental and physical state and their particular wishes about their care. The medication policy was examined and observed practice showed that the policy was followed in all aspects. All of the residents spoken to suggested that they considered that they were treated with respect and that their privacy was considered “ the staff here have always treated me very well” “ there’s nothing here to change”.

The staff members confirmed that the National Vocational Qualification training that they were undergoing had raised their awareness of how their behaviour affected the residents. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12. 13. 14. 15. The residents are provided with the opportunity to follow their chosen lifestyle at all times. Where the resident is unable to make those choices and decisions for themselves, family or representatives are consulted to make sure that the person has as much control over their lives as possible. An interesting and appealing menu is provided and the residents’ consider the standard of food good. EVIDENCE: All preferences and interests are recorded on the care plans. The home has a visiting Occupational Therapist, who, twice weekly, provides various activities, which the residents can take part in if they wish. At the time of inspection 9 residents were taking part in a trivia quiz and thoroughly enjoying it. Some of the residents had been out for coffee that morning. Notices informing of each day’s activity were displayed on the wall of the lounge. The residents told the inspector “ The staff are all very pleasant “ “ It’s all right to be here it’s a very nice place” “They’ve always treated me very well “ “ Mrs Shoai is very good and will do anything to try and help “. The food is prepared in the central kitchen and transported in a heated trolley, this had not been harmful to the quality of the food in any way. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16. 18. The residents are sure that they can trust the home to protect them as far as possible from bad practice and unacceptable behaviour from others. EVIDENCE: All of the residents spoken to knew how to complain if they needed to “ I would tell Maureen ” “ I’d go to Mrs Shoai “, but most said that they had no need to. The home has policies and procedures in place to ensure that action is taken if a resident was worried and the complaint book has two recent issues clearly recorded with the outcomes. One of those complaints had been the subject of an Adult Protection Investigation that is still to be concluded by the Social Service Department but the inspector was satisfied that the registered provider had acted appropriately and that there was no risk to the residents. The home has clear instructions for staff members as to what to do if abuse of a resident is suspected and both the members of staff that were spoken to in depth were knowledgeable about the procedure to take and had both received training. The recruitment files examined showed that all the checks to ensure proper security screening on all applicants had been carried out. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19. 23.24.25.26. The home is a comfortable, pleasant and well-maintained building, which has a satisfactory standard of cleanliness. As well as a choice of communal day space it provides each resident with a room that has been furnished to meet their wishes and needs. Regular maintenance makes sure that the environment is safe. Chesswood Lodge H60-H11 S14445 Chesswood Lodge V220139 100505 Stage 4.doc Version 1.20 Page 15 EVIDENCE: During the course of the inspection the majority of rooms were visited to make sure that the environment was safe and comfortable for people who live there. It was seen that many residents had brought personal possessions into the home, including small items of furniture, ornaments and photographs. On the day of inspection, Chesswood Lodge was clean, and free from offensive odours. Policies and procedures were available for staff about the control of infection, and the safe disposal of clinical waste. Residents told the inspector how much they liked their bedrooms, and one said that “nothing is too much trouble, they have moved my furniture around several times for me”. A resident, who had chosen to share a bedroom, said that she was very happy sharing and found nothing to complain about. One bedroom was having some repairs and redecoration carried out. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27.29.30. Chesswood Lodge has an adequate number of staff members with appropriate training and skills to provide competent care to the residents at all times. EVIDENCE: The registered manager has recently resigned and an Acting Manager is in place with an application to register pending. The rota for the week showed that there would always be a suitable number of staff on duty to care for the residents at any time. Examination of two recruitment files, (one of a recent appointment and the other of a member of staff who had been in post for two years), evidenced that all security checks such as written references and Criminal Records Bureau checks had been obtained. Proofs of identity were available and in one case an appropriate visa. The newest member of staff confirmed that he had been given comprehensive induction training and had considered that he had been well prepared for the work and the residents’ requirements. The other members of staff on duty were undergoing National Vocational Qualification Level III and said how helpful it was in raising their awareness of their own behaviour and how it could affect the residents. Other work related training organised by the Company had also been undertaken and the staff files contained certificates to evidence attendance. The majority of staff members are either undergoing National Vocational Qualification level II training or have the qualification. Two members of staff are currently undertaking Level III, the Acting Manager National Vocational Qualification Level IV and the Registered Manager’s Award. Observation of the care staff members’ actions whilst going about their work further reinforced that the residents were supported in a discreet and respectful way to follow their chosen lifestyle. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33. 37. 38 The home is run in a manner that offers protection to all aspects of the residents’ interests. EVIDENCE: Although the registered manager’s position is vacant an acting manager is in place that has all the skills and competence to discharge her responsibilities fully. Other senior managers in the company support her. There are comprehensive policies and procedures in place to provide protection to the residents and guidance to staff members on how to carry out their duties; records were found to be accurate and up to date. In particular the individual care plans contained vital information regarding the residents’ health and welfare needs and promote a uniform approach to the care and protection of the residents.
