Conditions of registration: 1. The home is registered for a maximum of 11 service users in the category of OP (Old Age, not falling within any other category). 2. Staffing must be provided to meet the dependency needs of the service users at all times and will comply with any guidelines which may be issued through the Commission for Social Care Inspection regarding staffing levels in care homes. Date of last inspection 1 December 2004 Brief Description of the Service: Chestnut Grove is a care home that has eleven beds for service users of either sex over the age of sixty-five years. The home is set in its own grounds in a quiet residential area in Lostock Hall, Preston. Chestnut Grove is close to motorway links and main bus routes. Accommodation is provided on two floors. The first floor is accessed by a stair lift, however there are three steps from the stair lift to the bedrooms. Accordingly service users with mobility problems are accommodated on the ground floor. The home has a large lounge and separate dining area.There is ramped access to the grounds which are safe and well maintained, outdoor seating is provided so service users are able to enjoy the garden. This is an overview of what the inspector found during the inspection. This was an unannounced inspection that took place over one day in August 2005. The inspection involved discussion with the people who lived and worked at the home, discussion with a visiting relative, examination of records, a tour of the home and information received from questionnaires sent to relatives and service users. What the service does well: What has improved since the last inspection?
Since the last inspection the registered person has ensured that all information required by law has been obtained for all existing staff in the home. A training matrix is in place to ensure that all staff are kept up-to-date with mandatory courses and the policy and procedure with regards to non prescribed medication has been reviewed and up dated. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 and 3 Service users are provided with information to ensure that they can make informed choice about where to live. The admission and assessment procedure ensures that the home is aware of the service users individual needs and are able to meet the needs. EVIDENCE: The home has a statement of purpose and service user’s guide, both documents contained comprehensive information on the services offered by the home. Each service user is provided with a copy of the service user guide and one was available in the entrance hall for all visitors to read. The home has a good admission procedure and a detailed assessment process, which ensures service users needs are fully assessed and planned for. Individual records were kept for each service user and both service users and their family told the inspector that they were aware of these records and the content. Two new service users told the inspector that they were offered a trial stay at the home before making a decision to stay permanently. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8 and 10 There is a clear consistent care planning system in place, which ensures that the health, personal and social care needs of the service users are met. Service users are treated in a way that respects their dignity, rights and privacy. EVIDENCE: Each service user had an individual plan of care, which sets out in detail the action that is needed to be taken by the care staff to meet their needs. Each service user had a daily record sheet, which recorded the care given and any significant events. Care plans are reviewed on a regular basis and service users and their families are invited to be involved in the process. Both a service user and their family told the inspector that they were aware of their care plan. The files of two service users were viewed in detail as part of the inspection process. Both files contained records for monitoring and recording any health care intervention and risk assessments were in place. One service user with visual impairment had a large numbered telephone installed in her own room along with a radio and a type recorder to play talking books. All service users spoken to said that they felt their needs were being met and that the manager and staff were very keen to monitor their health and wellbeing. Service users said that the manager and staff were very attentive to

Care staff were seen knocking on service users bedroom doors before being asked to go in, and all staff were seen speaking to the service users in a gentle and caring manner. A number of comments were received from relatives, all were very satisfied with the care provided. One comment received from a relative said the care and attention her family member receives is exemplary. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 15 The dietary needs of the service users are well catered for with a balanced and varied selection of food available that ensures service users individual tastes and choices are catered for. Social activities are well managed and provide variation and interest for people living at the home. EVIDENCE: Service users and their family told the inspector that home provides a lot of flexibility and choice with regards to the daily living and activities. One service user said that she can stay in her room for meals, get up and go to bed when she feels like it. Relatives can visit when they want and stay as long as they want. The manager takes responsibility for arranging activities, which includes trips out, pub lunches, garden party’s and visits from external entertainers. One service user said that she had especially enjoyed the summer barbeque. Birthdays are celebrated and one service user told the inspector that the home invited her relatives to her birthday party. Service users social interests, likes and dislikes are recorded on their personal file. The food is cooked in the homes kitchen and fresh produce is used each day. All service users said that the food was very good and that they were always offered a choice if they didn’t like what was on the days menu. The dining

JUDGEMENT – we looked at outcomes for standard(s) 16 The home has a complaints procedure, which ensures that all complaints would be acknowledged and investigated. EVIDENCE: At the time of the visit the home had not received any complaints. A complaints policy and procedure was in place and is made available to all service users in the service users guide. One service users told the inspector they had no complaints but knew who they could speak to if they had any concerns. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 The home was clean comfortable and homely and provided an environment that was suitable for its stated purpose. EVIDENCE: Chestnut Grove is a small homely residential home that is well maintained and cleaned to a high standard. There is a programme of routine maintenance and renewal of fabric and decoration. Since the last inspection a new kitchen had been fitted. One service user said that she felt very comfortable living in the home and another said she felt it was very homely. Outside the grounds are also kept to a very high standard and accessible for service users. The gardens at Chestnut Grove provide both a place of interest as well as a place to relax, one service user said that the recent garden party was very successful and both service users and relatives said they had enjoyed themselves. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 28 and 29 The policies and procedure for the recruitment of staff are robust and provide safeguards for the protection of service users. Staff at the home are provided with training to ensure they are competent to do the job. EVIDENCE: Chestnut Grove maintains a stable and motivated staff team. Since the last inspection there had been no new staff. The inspector viewed an existing staff file. All appropriate checks had been undertaken and the registered person said that all checks had been completed for the full staff team at the home. There was a training matrix in the home, which clearly showed what training had been undertaken by staff and certificates were seen on the staff files. Staff were provided with mandatory training which equipped them with the necessary skills to carry out their role confidently and competently. Staff said that they felt well supported by the management team and the training provided. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 The home is well managed which ensures service users interests are safeguarded. Staff are appropriately supervised to ensure they are confident and competent in the work they do. EVIDENCE: The registered person is in day to day control of the home and encourages feedback from staff, service users and family. The staff felt supported by the registered person and said that she will always listen to what they have to say. Relatives said that they could always approach the registered person about any issues and felt confident that they would be dealt with appropriately. Service users, staff and their families are provided with the opportunity to be involved in the running of the home through the process of surveys. The home had asked a relative to assist service users to complete a full survey on the home. The survey had been analysed and the findings published. The survey covered all aspects of life at Chestnut Grove from the environment to the care

The overall results were very positive with service user happy with Staff confirmed that they receive the care and attention they receive. appraisals and supervision and this was seen documented on the staff files. Staff had also had the opportunity through a recent staff survey to influence the running of the home on whether uniforms should be worn or not.
