This inspection was carried out on 6th December 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

This is a care home where residents are well looked after. The staff team work well together and show a good understanding of the needs of the people living at the home. The Inspectors received very positive comments from the residents spoken to and from the comments cards that were completed. The Inspectors were also able to observe the friendly and caring attitude of the staff towards the residents and visitors, all of which added to the homely environment. A visitor informed the Inspectors that the visited every day and were made to feel welcome. They said, "this is a lovely home and the staff are kind".

What has improved since the last inspection?

The manager has now been in post for approximately nine months and has identified a number of improvements that could be made. Following advice from the Pharmacy Inspector, the medication procedures have significantly improved.

What the care home could do better:

The manager should ensure that any outstanding matters raised by the Pharmacy Inspector are complied with in order to ensure that residents are not placed at risk. The homeowner should look at providing additional storage space if possible, so that any potential risks to the safety of staff and residents are prevented. The manager should obtain the relevant management qualification. Additional qualified staff should be available to enable the manager to undertake her role and responsibilities. The homeowner or their representative should carry out visits to the home at least once per month, unannounced, and send a copy of their report to the Commission for Social Care Inspection.

The home is registered for a maximum of 32 service users to include: Up to 20 service users in the category of OP (Older Persons over the age of 65 years) Up to 6 services users in the category of PD (physically handicapped) Up to 6 services users in the category of TI (Terminally Ill) The service should employ a suitably qualified and experience manager who is registered with the Commission for Social Care Inspection. 8th June 2005 2. Date of last inspection Brief Description of the Service: Cleveleys Nursing Home is registered to accommodate a maximum of 32 elderly persons. 6 beds may be used for residents with a physical disability and 6 beds may be used for residents with a terminal illness. The home is situated in a residential area of Cleveleys and is close to local amenities and within walking distance of the seafront. The home was purpose built and the accommodation consists of 3 double bedrooms and 26 single bedrooms, 8 of which have ensuite toilet facilities. There are lounge and dining room areas on the ground and first floor and there is a no smoking policy throughout the home. A passenger lift is available and aids are provided throughout the home to assist residents as required. This was an unannounced inspection, which was undertaken by two Inspectors. The Inspection stated at 9.15am and took place over two hours. The Inspectors looked at various records relating to the standards not looked at during the previous inspection. The Inspectors spoke generally to the staff and residents seen during a tour of the building and looked at four comment cards that had been completed by residents. The Inspectors also spoke to one relative who was visiting at the time. Since the previous inspection an additional inspection has been carried out by the Pharmacy Inspector to examine the medication policies and procedures of the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 The systems in place for the administration of medication have improved since the last inspection, however shortfalls still remain that require attention to ensure that residents are protected by the procedures carried out in the home. EVIDENCE: Following the previous inspection an additional inspection was carried by the Pharmacist Inspector, the outcome of which was that several requirements and recommendations were made. As part of this current inspection the Inspectors were able to evidence that whilst the majority of these matter had been dealt with, there were still some that required attention to ensure that the policies and procedures for the receiving and disposal of medication were being complied with. The Inspectors were informed that prescriptions were not being seen and checked by the home prior to the pharmacist dispensing the medication. Medication no longer required was not being disposed of. Advice was given and the Inspectors informed the manager that they would ask the Pharmacist Inspector to contact her. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 25 The standard of the environment within the home is good, providing residents with an attractive and homely place to live. Storage space is very limited and could potentially put residents at risk. EVIDENCE: A tour of the home confirmed that the home was well maintained, clean and free from any offensive odours. Radiators throughout the home have a guaranteed low surface temperature to protect the residents from the risk of burning. The hot water temperatures throughout the home are checked and delivered at a safe temperature. The Inspectors noted that storage space was very limited and on the first floor a small area previously used as a ‘nurses station’ was being use to store continent pads. The Inspectors were concerned as there was no door on this area and therefore this could pose a possible risk to the safety of the residents. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 & 35 The leadership from the manager is good and promotes a consistency of care. Additional support is needed to ensure that the manager continues to undertake her role and responsibilities fully. EVIDENCE: The manager is a qualified nurse who has many years of experience in caring for residents. The manager is aware that she must obtain a management qualification. The Inspectors were informed that she has been unable to commence training so far but that she fully intends to commence the appropriate course next year. Whilst at the time of the inspection there were sufficient staff on duty to meet the needs of the residents accommodated, the Inspectors found that often the Cleveleys Nursing Home DS0000006032.V269467.R01.S.doc Version 5.0 Page 17 manager was the only qualified member of staff on duty and therefore she had to carry out her management role in addition to nursing tasks. Since the previous inspection there has been a change in the ownership of the home. The Inspectors found that the responsible person was not undertaking monthly visits as required by Regulation 26 of the National Minimum Standards.
