It is one mile from the centre of Liskeard, which provides a range of shops and community facilities. Coombe House provides accommodation and personal care for up to 16 older persons, some of whom may also have a mental disorder/dementia. Accommodation is provided on two floors. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on the 14th March 2006 over a period of approximately 5 hours, commencing at 09.30am. The registered provider was present throughout. The inspection comprised meeting with 4 service users, a tour of the premises, discussion with staff and the registered provider, and inspection of care documentation and other records. Coombe House is an established home offering a relaxed, comfortable setting in which service user’s care needs are appropriately met. What the service does well: What has improved since the last inspection?
The registered provider has responded positively to the recommendations identified at the previous inspection, amending risk-assessments to reflect the need for bed rails where appropriate, recording her hours spent at the home, and providing appropriate fire safety signs. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2, 5, 6 A contract/Statement of Terms and Conditions is provided only to privately funded service users. Prospective service users and their representatives do have opportunities to visit the home prior to making any decision about permanent residence. The home does not offer intermediate care. EVIDENCE: A contract/Statement of Terms and Conditions is provided only to privately funded service users. The registered provider must provide each service user with a Statement of Terms and Conditions, setting out the fees paid and the identification of the accommodation offered, as well as any restrictions on the service user’s freedom of choice, liberty of movement, and power to make decisions. Prospective service users may visit for coffee, meals, or stay for the day. The registered provider ensures that all prospective service users or their representatives are aware of this at the time of carrying out the home’s preadmission assessment.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10, 11 Service users are treat with dignity and their right to privacy is respected. The home has suffered a recent service user death and the registered provider was observed to be understanding when dealing with relatives. EVIDENCE: Service users spoken with confirmed that they are happy with the care provided and the manner in which it is provided, they said that the staff were considerate and polite. The service users felt that their privacy was respected. The registered provider was observed and heard to be very empathetic with the relative of a service user who had recently died. The registered provider liaises well with service users and their families to ascertain their wishes in this respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14, 15 The home could do more to demonstrate that service users are able to exercise choice and control over their lives. The home offers freshly cooked meals and a balanced, attractive diet. EVIDENCE: Social/recreational records tend to show where a service user was on any given day, rather than what they were doing. The registered provider should ensure that the care records more clearly show the options available to service users and the choices made on a daily basis. Service users spoken with did not express a preference in respect of social/recreational activities, and similarly did not say they were happy or unhappy with the options available. A weekly menu is in operation. A record of food provided, including whether a service user eats well or not, is kept. Alternative meals are available to service users, and the registered provider said she would ensure that all service users were asked on a daily basis if the alternative would be preferred. The home is undertaking a ‘Making Food Safely’ programme and this involves the frequent attendance of the Environmental Health Officer. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 Service user’s legal rights are protected, and where possible, they are able to participate in civic processes. EVIDENCE: Service users spoken with confirmed they receive their mail unopened. The service users are entered on the electoral roll and are therefore able to participate in civic processes, where possible. The registered person lives on premises adjacent to the home and works at the home on a daily basis. This ensures she is available to monitor standards and support service users and staff as required. Service users are supported in maintaining their personal finances and affairs through relatives and Power of Attorney processes. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 25, 26 Service users live in safe, comfortable, clean accommodation and a homely environment, enhanced by their own possessions around them. EVIDENCE: The premises were clean throughout on the day of the inspection. The lavatories and bathrooms were seen to have hand-washing provisions. Heating, lighting, and ventilation are domestic and suitable to the home. Radiators have protective guards and are individually controlled. Restrictive window opening devices have been installed in all bedrooms to protect service users. All the hot water outlets are provided with mixing valves to reduce the risk of scalding. Communal areas are light, spacious and suitable for a range of activities, with plants and pictures in all areas. Downstairs, there is two lounges, a dining room, and a large office used for visiting relatives and service users. The gardens are maintained and a patio has been built with extending views across the surrounding countryside. Service user’s rooms were seen to be comfortably

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28, 30 There is an evident commitment to staff training and service user safety in an active and ongoing staff training programme. EVIDENCE: There are 20 care staff employed, of which 6 have achieved NVQ Level 2 or above, and a further 6 are doing this training. Each member of staff has an individual training record. Recent and planned training includes: manual handling, infection control, activities for dementia, medication, communication and swallowing, food hygiene, and 1st Aid. All staff have been provided with General Social Care Council handbooks. The registered provider must make arrangements for new staff to undertake a National Training Organisation (Skills for Care – www.skillsforcare.org.uk) compliant induction programme. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 36, 37 The home’s leadership, management, and policies and procedures benefit and protect service users. The home has achieved Investors in People status. Staff are appropriately supervised. EVIDENCE: There is a clear management hierarchy, with the registered provider taking day-to-day control and therefore being available to service users and their representatives. The registered provider does not handle any service user money, any transactions being invoiced to the service user or their representative as appropriate. The supervision is further complemented by an annual appraisal for all staff and frequent staff meetings. The home has achieved the Investors in People Award. The home’s policies and procedures are comprehensive, and as issues arise for discussion the relevant policy is brought to the attention of staff.
