This inspection was carried out on 14th March 2007.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Many service users and visitors told the inspector that the staff looked after them very well and meet their needs. The home meets service users needs with Dementia, needs were clearly identified in plans of care. Staff training was regularly updated ensuring they had good knowledge to be able to care for the service users. The home also had regular input from the Community Psychiatric Nurses through the home care liaison team. Recruitment was thorough and robust a selection of files were seen, they were well organised and contained all required information to protect service users. Adult protection procedures were robust and protected service users.

What has improved since the last inspection?

The improvements to the environment had commenced. The lounge and dining room on the unit for service users with dementia had both been re-decorated and had new furniture. This was due to continue throughout the home.

What the care home could do better:

The risk assessments were all identified and in place but lacked information in order to minimise the risk to service users.

The service may admit four persons 60 years of age and over. Date of last inspection 8th March 2006 Brief Description of the Service: Eastfield Care Home provides Nursing and Residential care to older people, the home is registered for 59 beds, 34 beds offer nursing and residential care this includes 4 beds to accommodate people aged 60 years, also separate unit with 25 beds offering residential care for service user with dementia. The home is situated in the village of Askern and is within walking distance of shops, churches and other community facilities. All the homes bedrooms are single and three bedrooms have en-suite facility. The fees at Eastfield at the time of the inspection were £375 to £618. These fee charges only applied at the time of inspection, more up to date information may be obtained from the home. This was the homes first key inspection in the year 2006/07 it was unannounced and took place over two days on 14th March 2007 at 10.30 and finished at 15.45 then on 22nd March 2007 at 11.15 and finished at 16.30. As part of the inspection process the inspector spoke to 12 residents, 2 visitors, 7 staff, the acting manager and on the second day the operations manager was at the home. The acting manager is currently being registered with Commission for Social Care Inspection. A tour of the building took place, observing environment, staff and practices. A number of records were examined these included medication, three service users care plans, menus, staff rotas, recruitment, service users finances and quality assurance systems. Feedback was given to the Manager and the operations manager when the visit was completed. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3. The home does not offer intermediate care so standard 6 was not assessed. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. Assessments showed the home could meet service users needs. EVIDENCE: Three service users were case tracked as part of the inspection process. The assessments seen in the plans of care were very detailed and clearly identified that their needs and gave reassurance that the home would meet the needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. All health care needs of service users were met. EVIDENCE: Three service users were case tracked as part of the inspection process, the plans of care were very detailed and comprehensive and clearly identified service users needs and action to take to meet those needs. The plans were reviewed monthly and any changes were documented. Risk assessments were in place for all identified risks however, they did not clearly identify all precautions or necessary instructions to minimise the risks to the service users The acting manager and staff promoted service users health and maintained access to all health care services to ensure their needs were met by referring Eastfield Care Home DS0000065778.V297834.R01.S.doc Version 5.2 Page 10 service users to appropriate health care professionals when required. Details of the services accessed were clearly recorded in the plans of care. Staff spoken to talked about service users in a sensitive and respectful way and understood the need to promote their dignity. Staff observed treated service users and visitors with respect. Service users told the inspector that staff look after them very well and were very nice. Medication records were seen for all medication received, administered and disposed of. The home had the appropriate systems in place for disposal of medication and all staff that administered medication had received accredited medication training. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users maintain community contact and were able to exercise their choice. Meals were appealing and balanced. Social and recreational needs of service users were met. The activities organised were varied and were arranged either as a group or individually. Service users told the inspector that the activities were good and the organisers worked very hard. Service users maintained contact with family and friends. Visitors were welcomed at any time and service users were able to receive visitors in private if they wished. The relatives spoken to told the inspector that they were always made welcome and the staff were polite and friendly, and looked after the service users very well. It was observed during the inspection that staff offered service users choices and gave assistance to facilitate their choice. Service users spoken to told the inspector that they were always given choices and staff respected their choice. Service users received a wholesome balanced diet there was always a choice of food at each meal. The inspector observed a meal during the visit, which was well presented and appealing. Staff were observed offering assistance where necessary discreetly, sensitively and individually. Staff asked service users if they had finished before removing plates. Staff encouraged service users to finish their meal when it was required, in the appropriate manner ensuring service users needs were met. Service users told the inspector that the food was very good and there was always a choice. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. The home has robust systems in place to protect service users. EVIDENCE: The home had a comprehensive complaints procedure, which states timescales to be followed and contact details of the home, head office and CSCI. The home had received 2 concerns since the last inspection these were well documented and fully investigated to ensure service users and staff were protected. Service users spoken to were aware of the complaints procedure and were satisfied that if they raised a concern or complaint it would be dealt with appropriately. The home had a good adult protection policy and all staff had received training. All staff had POVA and CRB checks prior to appointment in order to protect service users. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26. Quality in this outcome area is adequate This judgement has been made using available evidence including a visit to this service. The home was maintained to a high standard of cleanliness however some areas of the environmental required attention. EVIDENCE: A full tour of the building was carried out, the home had commenced a renovation programme and the lounge and dining room on one unit had been re-decorated and all new furniture provided. The bedrooms on the unit where service users with dementia are accommodated required attention. The inspector received confirmation from the company’s estates department that the bedrooms were due to be re-carpeted week commencing 26th March and then decoration to start from 2nd April 2007. Eastfield Care Home DS0000065778.V297834.R01.S.doc Version 5.2 Page 15 The standard of cleanliness observed during the tour was excellent and many comments from service users were that the home was always very clean. Toiletries and ointment were found in unlocked cupboards in most bathrooms on both units which could pose a risk to service users. The laundry facilities were sited in the room where the three gas boilers for the home were located, which did not provide a good work area for the laundry assistants. The acting manger agreed to follow this up with the health and safety executive to determine if this was a safe working environment. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff were suitably qualified. Robust recruitment procedures protected services users. EVIDENCE: The home provided staffing numbers adequate to meet the needs of the service users, a staff rota was seen showing staffing numbers. The home had 75 of care staff trained to NVQ level 2 or above ensuring service users were in safe hands at all times. A selection of personnel files were looked at they all contained all the required information and checks to ensure the service users are protected. Staff training was very good with all staff receiving mandatory training yearly and fire safety every six months. All staff training was up to date ensuring they were competent to do their jobs and could meet service users needs. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 & 38. Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. Management and administration of the home promotes the well being of service users. EVIDENCE: The home had appointed a new manager since the last inspection and he was in the process of being registered with Commission for Social Care Inspection. Effective quality monitoring systems were in place so the home was run in the interests of the service users. The homes administrators were in the process of changing bank accounts, which would provide individual umbrella accounts for each service user, interest would be given on each individual amount. Good records were kept in the home of all transactions and receipts to safeguard service users. Safe working practices are maintained within the home with good health and safety policies and procedures, all maintenance records were seen and up to date. All environmental risk assessments i.e. for use of equipment within the home were in place they were completed and clearly documented to ensure staff, visitors and service users were protected.
