Conditions of registration: 1. The service may admit four persons 60 years of age and over. Date of last inspection 9th January 2006 Brief Description of the Service: Eastfield Care Home provides Nursing and Residential care to older people, the home is registered for 59 beds, 34 beds offer nursing and residential care this includes 4 beds to accommodate people aged 60 years, also separate unit with 25 beds offering residential EMI care. The home is situated in the village of Askern and is within walking distance of shops, churches and other community facilities. All the homes bedrooms are single and three new bedrooms have ensuite facility. Eastfield Care Home DS0000065778.V285979.R01.S.doc Version 5.1 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. One Inspector from the Commission for Social Care Inspection carried out this unannounced inspection at Eastfield Care Home, on the 8th March 2006, commencing at 9:45 and finished at 12:45. This was the home second Inspection since Ashbourne (Eaton) Ltd has become the proprietor of Eastfield Care Home, however the home as been taken over again by Southern Cross (but is still registered as Ashbourne Eaton) As this was the second sale of the service in a short time the Inspector was interested in how these changes affected the residents and relatives. Most of the key standards were assessed on the first Inspection, but it may be the case that some standards are covered twice within the inspection year 2005/2006, which is considered good practice, and consistent with a professional approach to regulation. During the Inspection we looked at a number of standards, a chosen number of documents, sampling of records, tour of the premises and direct and indirect observation of staff interaction with residents, this Inspection also included individual and group discussions with staff, residents, and comments from any visitors on the day. Any issues or concerns that were raised were discussed with the Manager during and at the end of the Inspection. What the service does well:
In respect of the service and care delivery the home continues to provide a good quality provision, as they have a very stable staff group and the manager as been in post a number of years, who ensures the home is well managed and provides leadership, guidance and direction to staff. Residents spoken to confirm they were happy with care they received, and the staff at the home, other comments from residents about the changes in ownership, this does not appear to have had any impact on life within the home. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Resident’s health is monitored and staff can access all NHS health care facilities. Medication practice by staff ensures that Medicines in the custody of the home are handled according to the requirements of the Medicines Act; this ensures residents are protected by the homes policy and procedures. EVIDENCE: This standard was assessed at the last Inspection and issues were raised all have been addressed including the purchase of medicines fridge for both units. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All these standards were assessed on the last inspection and met, however a good practice recommendation was to look at developing the activities role with an increase in hours, this was not addressed and will be carried forward, as this would be beneficial to most of the residents in the home especially in the EMI unit. EVIDENCE: The complaints procedure was discussed with the registered manager, which remains the same, and records checked, show that the home have had no complaints since the last Inspection. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 21 24 26 The company need to have a refurbishment plan for the home to upgrade the fabric of the premises and renew some fixture and fittings. EVIDENCE: Tour of communal area on both units within the home raised a number of issues regarding environmental standards. Nursing unit main entrance to the home is showing signs of wear and tear, carpet very worn and ready for a refurbishment. Corridor carpets are showing signs of long use, and furniture in the lounge looks worn in appearance, chairs are stained on the arms and look well worn. EMI unit was found the same with a number of carpets looking very worn and furniture showing signs of long useage. Eastfield Care Home DS0000065778.V285979.R01.S.doc Version 5.1 Page 13 The EMI unit was discussed with the manager as a lot of the client group have deteriorated, the company need to re-assess some environmental areas, for example steep open staircase, ramp from the dining room to the lounge and it was noted that one residents was having difficulty using this, these issues may pose a risk to residents who have dementia problems. Bathrooms seen although most bathrooms had appropriate aids that meet resident’s needs the fixture and fitting require upgrading. Bedrooms on both unit were randomly selected and found that a number of these are looking very worn with carpet being old and stained, two were identified to the manager as being badly frayed and would have been safety risk to residents. Some of the bedding was thin, curtains worn and furniture mixed and matched. A number of bedrooms need upgrading with redecoration and refurnishing. Laundry service was being moved on the last Inspection, however this work as stopped re issue with ventilation and fire exits, therefore the laundry continues to remain in the old unit until these issues are addressed. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 30 The registered provider must provide appropriate training especially for new staff to ensure that they are equipped to meet the needs of resident group within the home. EVIDENCE: Training was discussed with the manager as in the past the home has always ensured that any new staff receives training and development programme that met the NTO specification, but since Ashbourne have taken over they have changed the way they deliver staff training, and this raises issues. Evidence was seen that new staff had an induction to the home that covered all mandatory training, but no evidence was found that staff receives induction and foundation training that meets NTO specification that equips staff to meet the needs of resident group within the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 36 38 The homes policies and practices promote and safeguard the health, safety and welfare of residents within the home. EVIDENCE: Supervision for both the manager and staff was discussed with the manager, an requirement issued on the last Inspection with regard to formal supervision not being regularly maintained, this was discussion with the manager, who stated that staff are now receiving supervision on a regular basis with records available, this was confirmed by staff members seen. Although the manager as contact with her line manager, and usually discusses development and performance issues, no evidence was seen that she receives formal supervision. NO STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards.
