The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The manager and staff team have worked diligently in order to meet all of the requirements made at the previous inspection and all previous recommendations had been met Good standards of care remain at the home, with many favourable comments received from residents and relatives about the care staff at the home in respect of their caring manner and approach. Those residents spoken with during the inspection said they were happy and enjoyed life at the home. The house is located within close proximity to local services and provides a homely environment for those who live there.

What has improved since the last inspection?

Those living at the home can feel secure in the knowledge that staff have been recruited following a robust recruitment and selection process, ensuring that criminal record bureau checks have been obtained before commencing employment. Residents can be assured of their safety should a fire occur in the home as staff have received sufficient fire instruction and are aware of their responsibilities in this area. Residents can also be assured that the use of the stair lift has been fully risk assessed and that their safety has been fully considered. Residents can be further assured of their safety as evidence is in place at the home of the testing of portable electrical appliances and also as the managers checklist has incorporated that emergency call bells are checked on a regular basis. The environment in which residents live has improved as a source of odour in a resident`s room had been eliminated. Residents can have some reassurance of the homes record keeping systems as the home have ensured that there are photographs in place of all residents and also as the home had sought and recorded the wishes and choices for residents in the event of their death. Further improvements to record keeping at the home were evident as in place was a consistent approach to the recording of visiting health professionals and/or appointments.

What the care home could do better:

In order to ensure the safe administration and the security of residents medication is required that records of medication given must be improved and controlled medication must be stored correctly.

May accommodate 21 service users aged 65 and over requiring personal care only May accommodate 4 service users aged 65 and over for Dementia care. 17th January 2006 Date of last inspection Brief Description of the Service: Edgecumbe Lodge is situated in an established residential area in the south of South Gloucestershire. It is approximately half a mile from the shopping areas of Staple Hill and Downend where doctors surgeries, post offices, banks, library, shops and other facilities exist. There are bus routes on roads at either end of Overnhill Road that take you into Bristol. The home is a large, detached Victorian house on four floors. Despite adaptation for its present use, many features of the original period remain and enhance the sensitively planned alterations. Full use is made of the basement/garden level area and of the top of the attic. Resident’s living here are Older People with some physical frailties. There is a passenger lift accessing all but the top floor, which has a stair lift to it. The stair lift is for the use of visitors and the occasional use of residents. It is the policy of the home not to accommodate those with limited mobility in this area of the home. There are well laid out, mature gardens which are well tended throughout the year. The exterior and interior of the property are in good repair, pleasantly decorated and comfortably furnished. The former garage/workshop is the manager’s office, with tea making facilities and a toilet, making the unit selfcontained. The purpose of the visit was to establish if the home is meeting the National Minimum Standards and the requirements of the Care Standards Act 2000 and to review the quality of the care provision for the individual’s living in the home. This inspection employed key elements of the national inspection methodology with the objectives of focusing on outcomes for the individual’s. This is evidenced through evaluation of core standards and verification through a surveying and case tracking approach that included talking with and the observation of individuals who live at the home and the views of the manager on duty. An opportunity was taken to view the home and a number of the records relating to the management of the home and plans of care for five residents were reviewed. Supervision, training and recruitment records of staff were also reviewed. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4, 5. The quality outcome in this area is good; this judgement has been made using available evidence including a visit to this service. Prospective residents and their relatives have the information they need in order to make an informed choice about where to live and admissions into the home are planned with individuals needs having been assessed. Improvements have been made to the terms and conditions of the placement and information about fees charged is clear. EVIDENCE: The home has a Statement of Purpose and Service Users Guide, which contain information required by the regulations. The Service Users Guide is given to prospective residents and/or their relatives when they visit the home or make enquiries to enable them to make an informed choice about moving into the home. Information seen within care files evidenced that no person is admitted into the home without having their needs assessed and have been assured that these will be met. Mrs Sardar was able to inform the inspector of those needs, which would not be able to be met at the home, and was able to demonstrate the homes capacity to meet the assessed needs of individuals admitted into Edgecumbe Lodge Rest Home for the Elderly DS0000062860.V304541.R01.S.doc Version 5.2 Page 9 the home. Individuals who had been referred to the home through the care management process had a full assessment of need recorded within their care plan and those individuals who are self funding and without a care management assessment Mr Sardar had completed a pre admission assessment of abilities and recorded how these would be met. Records also showed that individuals are able to move into the home on a trial basis before they make a decision to stay; unplanned admissions are avoided where possible. Through examination of staff records it was clear that staff individually and collectively have the skills and experience to deliver the services and care which the home offers to provide as outlined within the homes statement of purpose. Prior to the inspection a complaint was raised with The Commission in respect of fees charged for short term ‘nursing’ care provided at the home. As a direct result this concern contracts for a number of residents were reviewed during the inspection. These found that on the whole the information contained within these contracts was correct, however, information about actual fees charged was not clear and did not always correspond with records held at the home in respect of ‘third party’ contributions and fees paid by the local authority. In order that residents and/or their representatives are fully aware of the fees being charged at the home it is required that the homes terms and conditions of individuals placement at the home are reviewed and updated in order to fully reflect the current charges. The inspector returned to the home on 26th October and found that the registered provider had worked diligently in order to meet this requirement. All contracts within the home had been reviewed and contained all of the required information, including information about the fees. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8, 9, 10, 11. The quality outcome in this area is adequate; this judgment has been made been made using available evidence including a visit to this service. Care plans are in place for all residents. Medication records must be better maintained and controlled medication must be stored safely. EVIDENCE: Residents spoken with confirmed that staff provided them with privacy whilst assisting them with personal care. One resident stated, “Staff help me to have a bath’. Another resident stated that they were ‘satisfied’ with their care. Staff were noted knocking at the residents’ doors and waiting for an answer before going in to assist resident with personal care. One resident spoken with stated that “staff are good, they look after us well”. The care planning records of four residents were reviewed and evidence was in place to demonstrate that these are monitored and updated on a consistent basis. The plans ensure all areas of identified need is recorded with clear information to guide staff as to how these will be met. Systems of medication administration and recording were reviewed at this inspection. It was found that a controlled medication was not being stored as Edgecumbe Lodge Rest Home for the Elderly DS0000062860.V304541.R01.S.doc Version 5.2 Page 11 required in line with the Royal Pharmaceutical Guidelines, and that a residents record of a discontinued medication were not clear, furthermore, fourteen residents medication sheets had errors or omissions within them. This is not satisfactory and no reasonable explanation could be given. Therefore two requirements in respect of medication safety were given at this inspection, the first was that records of medication given to residents must be improved and also that controlled medication must be stored safely. At a return visit to the home on October 26th there was clear evidence in place to demonstrate that Mr Sardar had addressed these issues with staff members and additional support and training for staff had been provided, this is to be commended however the requirements remain and medication administration and practices will be reviewed at the next inspection. There was evidence of visits from the doctor, optician, chiropodist and other health professionals on the care files reviewed. At the last inspection it was found that there were different recording methods in place of the involvement of health professionals and a recommendation was made that consideration should be given to a consistent approach to the recording of visiting health professionals and/or appointments. It was found that the home have improved record keeping in this area, Waterlow assessments for residents had been fully completed and gave outcomes of what preventative measures should be implemented to prevent those risk skin pressure areas developing. In order to ensure that the rights and choices of residents were respected a recommendation was made at the last inspection the home should seek and record the wishes and choices for residents in the event of their death a review of records found that this had been undertaken and those records seen recorded residents views in this area. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15. Quality outcome in this area is good. This judgement has been made using available evidence including a visit to this service The Home enables residents to maintain contact with family, friends and community. It also provides meaningful activities and choice in respect of meals and mealtimes. EVIDENCE: During the inspection staff were observed asking resident’s for their views and opinions and residents were encouraged to make choices on aspects that affect their life. Residents told the inspector about their preferred routines at the home and said that they are able to choose when to get up and when to retire. Staff spoken with confirmed that routines of daily living and activities are made available are flexible in order to meet individual’s needs. Residents spoken with gave a number of examples of choices they make in their lives such as religious observance, relationships with others, food and political preferences. The visitor’s book showed that there are a number of visitors to the home and those spoken with said they are always made welcome by staff at the home. And also said they are able to visit their relative at any time and spend time with them in the privacy of their room. Edgecumbe Lodge Rest Home for the Elderly DS0000062860.V304541.R01.S.doc Version 5.2 Page 13 Resident’s rooms contained many of their personal possessions such as small items of furniture, ornaments, pictures and photographs. The kitchen was found clean and tidy. Staff have attended basic food hygiene training. The lunch on the day looked nutritious and balanced and the residents spoken with stated that they enjoyed their meal. Some of the comments made by the residents in relation to the food at the home include “ I enjoy the food and the kind of food cooked here is what I am used to’. ‘Home cooked, well cooked and lovely’, ‘Very often lots of vegetables at mealtimes’, ‘Plenty of choice and different everyday’. The home has on display a copy of the activities available for residents to participate in, or not. These include bingo, quizzes and crafts. The residents told the inspector of their enjoyment of a recent trip to the Bristol flower show. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18. The quality outcome in this area is good; this judgement has been made using available evidence including a visit to this service. There are clear complaints and adult protection protocols. EVIDENCE: A number of residents were spoken with during this visit no complaints or concerns were raised. All spoken with said that they would speak with the manager or a member of staff if they had any problems. Prior to the inspection a letter of complaint was received from a relative of a resident who once lived at the home, the issues raised were reviewed as part of the inspection as the complainant did not feel satisfied that their complaint had been dealt with effectively by the management of the home. One area was unable to be fully evaluated; the other issue in respect of contracts was reviewed as part of the inspection process. There was evidence that staff have attended Protection of Vulnerable Adults from abuse training and the home has in place a Protection of Vulnerable Adults policy; there was evidence of the South Gloucestershire Council document on how to report incidents of suspected abuse. Mrs Sardar demonstrated knowledge of the procedure for reporting incidents of abuse if they occur. Staff are aware of the Whistle Blowing Policy and are able to report incidents of abuse without fear of reprisal. Evidence from the most recently employed staff files showed that a staff members are recruited following robust recruitment and selection and information seen in staff files showed that completed application forms were in Edgecumbe Lodge Rest Home for the Elderly DS0000062860.V304541.R01.S.doc Version 5.2 Page 15 place, references and Criminal Record Bureau enhanced checks had been sought. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20,21,22, 25, 26. The quality outcome in this area is good; this judgement has been made using available evidence including a visit to this service. The quality of furnishings and fittings in the home is good and overall a warm comfortable environment has been created ensuring individuals needs are met. EVIDENCE: No changes had occurred in relation to the home’s suitability for its stated purpose on the provision of care for the residents. The residents were found sitting in the communal areas and their private rooms and appeared relaxed in their homely environment. Toilets and bathrooms meet the needs of current residents and are fitted with grab rails. A stair Lift is installed to maximise residents’ mobility and independence. This equipment is regularly serviced. A requirement was made at the last inspection that a risk assessment must be completed re the use of the stair lift this had been completed and contained all of the required information. Edgecumbe Lodge Rest Home for the Elderly DS0000062860.V304541.R01.S.doc Version 5.2 Page 17 A recommendation was made at the last inspection that the home must eliminate source of odour in the room identified, the inspector returned to this room and saw that the carpet had been replaced and no odour was present. Other areas of the home had no offensive odours. Staff were seen shampooing carpets and washing floors on the day of the inspection. A recommendation was made at the last inspection that the managers check list that was in place to ensure the health and safety of those living and working at the home should incorporate that emergency call bells are checked. A review of this document found that that the checking of call bells in all areas of the home (including residents private rooms) had been included within the checks and was being undertaken on a regular basis. It should be further noted that staff were seen to respond promptly when the call bell was used in order to request assistance. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29, 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The residents enjoy a good, warm relationship with competent staff. The Home’s recruitment procedure offers protection to residents. There is adequate numbers of staff to meet the needs of the residents with staff members undertaking appropriate sufficient training in order to ensure they fully have the skills required to undertake their duties. EVIDENCE: A requirement was made at the last inspection that no staff are to commence employment until a satisfactory police check has been obtained. The recruitment and employment records of four staff were evaluated. All staff had evidence of criminal record bureau and protection of vulnerable adult checks; completed application forms were in place, verification if identification were in place as were references. Each staff member had on file a copy of their job description; these clearly outlined the role and responsibilities of individual’s in line with the aims and philosophy of the home. There is a core of well-established staff with varying abilities most of which are skilled and experienced to meet the needs of the residents in the home. Resident’s spoken with said that they felt well supported and safe, that staff listened to them. Those spoken with did not raise any concerns about the Edgecumbe Lodge Rest Home for the Elderly DS0000062860.V304541.R01.S.doc Version 5.2 Page 19 staffing levels at the home. Comments from residents were: ‘the staff are always very kind’, ‘nothing is too much trouble’, ‘ the staff here are lovely’. A requirement was made at the last inspection that staff must undertake a minimum of three days paid training per year, it was also recommended at the last inspection that the home undertake an audit of staff training in order to identify training needs. An audit had been completed and there was evidence in place to show that staff had undertaken sufficient annual training. Training had included medication competency, fire safety and a distance learning package in which 22 staff members participated in training about infection control. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 36, 37, 38. The quality outcome in this area is good; this judgement has been made using available evidence including a visit to this service. The home is well managed with good polices and procedures in place to ensure ongoing review and development of the service, however the home must ensure that all potential risks are assessed. EVIDENCE: Edgecumbe Lodge is a well run home. The manager is experienced in working with older people. Mrs Sardar was able to demonstrate a clear understanding of her role and responsibilities and how this would influence the service delivered at the home to ensure that services were delivered as outlined within the individual’s care plan. Mrs Sardar was aware of her role and responsibilities as the registered provider. Edgecumbe Lodge Rest Home for the Elderly DS0000062860.V304541.R01.S.doc Version 5.2 Page 21 A number of records were reviewed as part of this inspection process, these included care documentation, staffing records and health and safety records. A requirement was made at the last inspection that the home must ensure that photographs of all residents must be in place. These were all in place and the requirement had been met. In order to demonstrate the safety of equipment in use at the home a requirement was made that evidence must be provided of portable electrical appliance testing. At this inspection records of individually checked items and verification of their safety was seen. The fire logbook was viewed, the logbook demonstrated that the appropriate weekly and monthly checks are undertaken and that the equipment and detection systems are serviced appropriately by a contractor. Records of fire training were satisfactory. The home has in place a fire risk assessment in place this covers all potential areas of risk in relation to fire safety have been identified and the likelihood of events occurring eliminated or reduced. The home has in place a number of comprehensive risk assessments and those seen in place covered potential hazards, who could be affected, how risks are controlled and outlines any further actions required. Assessments in place included general manual handling, slips, trips and fall, food contamination and use of equipment. A requirement was made at this inspection that a risk assessment must be completed on the use of a bed aid in order to demonstrate its safe use. A review of staff records found that staff members are receiving regular supervision from the manager this promote communication and continuity of care for residents.
