It is situated in a rural area approximately five miles from Liskeard. Accommodation is provided on three floors and comprises 18 single rooms and one double. Four of these rooms are en suite. Access to first and second floor is aided by stair lifts. Communal space is provided on ground floor in a sitting room and a dining room. An additional lounge is available on the first floor. There is level access to the exterior with its extensive grounds. Car parking is available off road next to the home. This was an unannounced inspection, which took place over three hours on the 19th August 2010 by Compliance Inspector Gail Richardson. We worked in conjunction with the local Adult Care and Support Safeguarding Lead person. The purpose of the inspection was to gather information and ensure that peoples needs are properly met, in accordance with good care practices and the laws regulating care homes. For the purpose of this inspection the term We will be used when referring to the Care Quality Commission. We looked at policies and procedures relating to pre admissions and admissions to the home. We spoke with the registered manager and two staff members. What the care home does well: What they could do better:
We looked at the pre admission assessment process. We found that this is not always undertaken by the senior staff members. In one instance the information received was limited and did not contain sufficient detail to ensure that staff could formulate a detailed care plan to follow. The management of the home were seen to not always request any input from health professionals involved in peoples care. In one instance there were no assessment documents requested or received to ensure that an informed decision could be made about if the home could meet the persons needs. Some details within the pre admission document were incorrect, this included a current health condition and this may place people and staff at risk. The home has an admission policy which records a plan of action to ensure that, following any emergency admission an assessment will be undertaken within 48 hours. We found that in one case this assessment was not undertaken. Furthermore, no supporting information, assessments or input from health professionals were requested to ensure that the person was appropriately placed at the home. An overall review of pre admission and care plan identified a limited level of communication between the home and other care providers, health professionals and Social Services which meant that the people may be at risk of their needs not being met. We looked at care records for people admitted to the home. The care planning was basic, lacked detail and was not person centered. Not all areas of identified need had a plan of care and this may place the people at risk of not having their needs met. It is also needed so that staff can meet all areas of assessed ensuring peoples choices and preferences have been met. A previous requirement was made about the need for all areas of assessed need to have a care plan in place. This requirement had a timescale to meet the requirement of 23rd November 2009. This requirement has not been seen to have been met. It was also identified that prescribed medication for skin treatment was being administered but was not recorded as prescribed, received or administrated in the Medication Administration Records. The management of the home must ensure that all prescribed medications are recorded and provide a clear audit trail of prescription,

It is also required that as part of this assessment the input of associated health professionals is sought. This is required to ensure that the registered manager can ensure that the service can meet the persons needs prior to admission. 2 9 13 The management of the 29/09/2010 home must ensure that all prescribed medications are recorded and provide a clear audit trail of prescription, procurement and administration. This is required to ensure that there is a clear audit trail and reduce any risk of error in administration.
