This inspection was carried out on 21st September 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home provides good care in warm comfortable surroundings. Residents appeared contented and those spoken with confirmed the standard of care was good. A visitor spoken with was very pleased with the care provided. The home is clean and well maintained with no odours noted. The records for providing suitable personal care and to ensure safety in the building were well maintained and regularly used.

What has improved since the last inspection?

Each of the requirements made at the last inspection have been addressed. The home has maintained its environmental standards and has renewed bedroom furniture in several bedrooms. Training is continuing and the manager is near completion of the Registered Managers Award. A quality assurance document is being used to improve the quality of care provided.

What the care home could do better:

Staff facilities could be improved to provide private lockable storage. The information obtained prior to employing care staff who work with vulnerable people could be improved.

Date of last inspection 17 January 2005 Brief Description of the Service: Ella McCambridge is a modern purpose built care home providing accomodation with personal care for up to 37 older people. The home is owned by Manor Care Home Group who have two other homes in the vicinity. The home is in a residential area of Walker, close to shops, pubs and other local amenities. The home was opened in 1999 and was purpose built over two storeys. There is level access from the car park and into the enclosed garden and there is a passenger lift between floors. One bedroom is registered to accept double occupancy and all but two bedrooms have en-suite toilet and wash hand basins. There are lounges and dining rooms on both floors and a separate lounge that is used by smokers. This was the home’s first annual inspection. It lasted over 5 hours and in that time I spoke with several residents and their visitors. Management and staff were interviewed, a selection of records maintained in the home were reviewed and there was a general tour of the building. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 6. Each resident is assessed to ensure the home can meet their needs before a place is offered. The home do not provide a rehabilitation service. EVIDENCE: Three care plans reviewed contained a comprehensive review done by one of the management team. The information collected was comprehensive and provided the home with enough information to limit the possibility of a place being offered to someone whose needs they could not meet. The home does not provide a rehabilitation service but does have beds used for respite. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 10. Good quality records are maintained regarding residents’ health and personal care needs. Healthcare is supported by professional services and appropriate equipment. Staff respect the privacy of residents. EVIDENCE: A plan of care is in place for each resident that contains information for staff as to how care should be provided. Management and the care team regularly review these plans and any change to the care required is recorded. When necessary appropriate professional assistance and any equipment such as special mattresses is obtained. Privacy is discussed on induction and promoted by management. Staff were observed knocking on doors before entering and attending to residents needs in a sensitive way. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18. Residents and visitors are comfortable about making a complaint and are confident their concerns are taken seriously. Vulnerable adults are protected by trained staff. EVIDENCE: Residents spoken with said they new who to complain to and that they would if necessary. A complaint log is held in the home to record any complaints received. Staff were fully aware of procedures to protect vulnerable adults and training is provided to all of the staff team. Staff have access to appropriate procedures and guidance on this topic. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 26. The home is safe, clean and well maintained. EVIDENCE: The home was found to be safe, clean and reasonably well maintained. New furniture has been provided in some bedrooms. There were no offensive odours noted in the home. During a tour of the building the following issues were identified; There is a staff room used to hang coats and take short breaks. This room contained lockers but these were not lockable. The tablecloths in this room were burned and dirty. The payphone on the ground floor was loose on the wall and had to be adjusted to get a dial tone. The air extractor in the smokers’ lounge was ineffective. It should be cleaned and maintained. There were no towels in the ground floor sluice and the light bulb flickered. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, 30. Residents are cared for by an appropriate number of trained staff. Appropriate employment practices are not always followed. EVIDENCE: Residents stated that staff are very caring. The staff rota showed that the staffing in the home generally meets the agreed standard. New staff are provided with an appropriate induction into care. 60 of care staff now have a National Vocational Qualification (NVQ) level 2 or above in care. Of the other care staff, 15 are working towards the qualification and 10 are waiting to start. The manager monitors all training to ensure it meets the desired standard. The file of the latest staff recruit was reviewed. This person came with a Criminal Records Bureau check from a previous employer. All staff should have a satisfactory check from the vulnerable adults register before starting work. This has now been applied for. All other employment issues were good. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 33, 36, 37, 38 The manager has many years experience in the care of older people, he runs the home in the best interests of the residents and has introduced a quality assessment procedure. The building is well maintained with a reasonable standard of health and safety. Staff meet regularly with management. EVIDENCE: The manager has many years experience of providing care for older people and is nearing the completion of the Registered Managers Award. Staff spoken with stated morale in the staff team is high and that management provide clear direction in the standard of service expected. There are regular meetings involving management, staff and residents where a good range of issues are discussed. The fire procedures are comprehensive and regular entries were found in the home’s log. Accidents are well recorded and a separate analysis of falls is produced. Appropriate maintenance certificates for lifting equipment, the passenger lift, gas, electric and fire appliances were seen. A training analysis ensures all staff have accredited training in; moving and handling, first aid, fire safety, food hygiene and infection control. Cleaning materials are safely stored and regular premises checks are conducted by management to ensure the home is safe.
