This inspection was carried out on 29th September 2009.

CQC found this care home to be providing an Adequate service.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 1 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

We found that people had care plans in place for all assessed needs. The vast majority of these are reviewed on a frequent basis. One person had detailed records in place regarding their wound care and the other person also had a specific care need in relation to eating and drinking, and their care plan contained all the required information for staff to follow. Records are in place of any health care professional visits.

What the care home could do better:

We found that care plans and risk assessments are not linked together and it appears they are written in isolation of each other and this was evident in that conflicting information was found in a number of care plans. Some care plans were task orientated and not based on peoples individual care needs and they were not always up to date with peoples current care needs. We discussed with the Registered Manager that care plans need to contain information about peoples care needs, peoples choices and what realistically the home can achieve. For example in one person`s care plan it mentions about continually observing this person, but as the home does not provide one to one care this can not be met. The staff need to look at the terminology used in care plans for example, words such as regular and adequate are not descriptive enough for staff to follow. When we visited one person in their room we found that equipment for their wound dressing changes was left on their table. We would suggest that this is put away away in a cupboard.

It is situated close to Cheltenham town centre, a short walk from Pittville Pump Rooms and the racecourse. The Care Home is equipped with shaft lifts to assist those unable to manage the stairs. In addition, a variety of aids and adaptations have been provided throughout the property to help people who use the service. With the exception of two spacious double rooms, all the bedrooms are for single occupation. Only two rooms have en suite facilities but assisted bathrooms and toilets have been installed Care Homes for Older People Page 2 of 8 Brief description of the care home throughout the property. The communal facilities consist of three well-appointed lounges and a dining room. A Day Centre has been developed within the property; this is in use 4 days a week but may also be used by people at weekends. The private gardens are easily accessible and may be enjoyed by people in warm weather; they are well protected from the busy main road by tall mature trees. Adequate parking is provided for staff and visitors. The provider supplies information about the home, including our most recent report in a file at the entrance of Ellerslie. Care Homes for Older People Page 3 of 8 What we found: The purpose of this inspection was to follow up the requirement issued at the last two key inspections in relation to care planning. We examined in detail the care records of two people who use the service and we visited both people in their rooms. We discussed our findings of this inspection with the Registered Manager. They say what the registered person had to do to meet the Care Standards Act 2000, Regulations 2001 and the National Minimum Standards. No. Standard Regulation Requirement Timescale for action 1 9 13 When any medication is 06/03/2009 administered to people who live in the home it must always be clearly and accurately recorded. (This particularly relates to records for prescribed treatments applied to the skin). This requirement was not followed up at this inspection. This is to help to make sure people receive their prescribed medication correctly and to help reduce risks of mistakes. 2 9 13 For any medication labelled 06/03/2009 for use when required, as directed or with a variable dose, make sure there is always clear written direction for staff on how to make decisions about administration for each person and medicine and taking into account the provisions of the Mental Capacity Act 2005. This requirement was not follow up at this inspection. No. Standard Regulation Requirement Timescale for action levels of medication in accordance with their needs and planned actions. 3 29 19 The home must make sure that all the required recruitment checks are undertaken as listed in this Regulation prior to the new worker starting work at the home. This requirement was not followed up at this inspection. This will help to make sure staff are provided with clear directions on how to meet peoples assessed needs. Recommendations These recommendations are taken from the best practice described in the National Minimum Standards and the registered person(s) should consider them as a way of improving their service.
