Conditions of registration: 1. The total number of persons that may be accommodated at any one time must not exceed 22. 5th July 2005 Date of last inspection Brief Description of the Service: Fairholme House is a residential home registered to take 22 elderly residents. On the day of the inspection 1 resident was in hospital and the home had three vacancies. The home is situated in a village location on the outskirts of Banbury. The village has the facility of a post office, church and a public house. The market town of Banbury is able to offer a wide range of shops, restaurants, pubs and transport connections to other parts of the country. The home is privately owned with the owner being very much in evidence. The proprietor assists and supports the manager and staff to provide a quality service. The home is managed and staffed by competent trained staff. Twenty rooms are available for single occupation. The home is able to offer one room for a married couple or those wishing to share. The home has appropriate rooms to enable residents to socialise or have quiet time if they so wish. The home has sufficient toilet and bathing facilities to meet the needs of its residents. Residents are encouraged to be as independent as possible. The home provides entertainments and activities within the home for those wishing to partake. Residents are encouraged to maintain contacts with relatives and friends outside of the home. The local church is very active within the home and is able to provide services within the home, counselling, and befriending. This is an overview of what the inspector found during the inspection. This inspection is the second inspection of this home this year and was unannounced. The inspection took place on a midweek afternoon. The inspector toured the building, had informal conversations with the residents and staff, and read core documentation relating to the care of the residents. The inspector spent some time talking to the Manager and the Proprietor. On the day of the inspection the electrical system had failed in part and engineers arrived before the end of the inspection. There was no disruption or discomfort to the residents during this period. Staff files were inspected against the regulations. No serious concerns were found at this inspection. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): The home has robust assessment processes. Prospective residents are given comprehensive information about the service. Prospective residents visit the home prior to moving. EVIDENCE: As the above standards were inspected and found to be satisfactory at the last inspection and no changes have been made since the last inspection these standards were not inspected on this occasion. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11 The home is meeting the health and social care needs of its residents. EVIDENCE: The inspector spoke informally to three residents all of whom expressed satisfaction with the home and the level of care it provides. Six residents files were examined which provided evidence that resident’s individual health, personal and social care needs are being met. The home has appropriate policies and procedures to manage the storage, recording and distribution of medication. A random selection of resident’s medication was inspected and found to be satisfactory. The home has introduced a system of monitoring the medication room which is now satisfactory. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): The home provides an appropriate range of social activities and promotes involvement in daily life. EVIDENCE: As the above standards were inspected and found to be satisfactory at the last inspection and no changes have been made since the last inspection these standards were not inspected on this occasion. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17,18. The home has a formal complaints procedure which is known to residents and their family. EVIDENCE: The home has not had any recorded complaints since the last inspection. Due to the size and nature of the home any concerns are dealt with at source and informally. The inspector found this method of dealing with complaints acceptable. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26, The home is well maintained and fit for its purpose. EVIDENCE: A tour of the premises informed the inspector that the home is being maintained and that a rolling programme of redecoration and upgrade is ongoing. The inspector was shown a vacant room that had been newly decorated awaiting a new resident. The room was presented as very warm and welcoming. Residents are encouraged to personalise their own rooms and this is much in evidence. All rooms are comfortably furnished with good quality fixtures and fittings. The home is upgrading the laundry facilities. The kitchen will be fitted with new work surfaces, which will improve the overall look. On the day of the inspection the home was clean and smelt fresh. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30. The home provides staff in sufficient numbers and skills to meet the needs of its residents. EVIDENCE: The inspector examined a random selection of staff files, which evidenced that the home is recruiting in an equal opportunities framework. The home is taking the appropriate measures to ensure that staff are police checked and that references are taken up. Supervision is now being provided on a regular basis. Evidence was found that staff are having regular reviews and an annual performance review. The home continues to provide a high level of training opportunities for its staff with many having gained N.V.Q 2 or above.
