The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The inspector was pleased to note that staff are being given a range of training opportunities and that staff interviewed expressed the view that they felt supported to do the job. The number and range of training opportunities available is to be commended.

Date of last inspection 9th November 2005 Brief Description of the Service: Fairholme House is a residential home registered to take 22 elderly residents. On the day of the inspection 2 residents were in hospital and the home had one vacancy. The home is situated in a village location on the outskirts of Banbury. The village has the facility of a post office, church and a public house . The market town of Banbury is able to offer a wide range of shops, restaurants, pubs and transport connections to other parts of the country. The home is privately owned with the owner being very much in evidence. The proprietor assists and supports the manager and staff to provide a quality service. The home is managed and staffed by competant trained staff. Twenty rooms are available for single occupation. The home is able to offer one room for a married couple or those wishing to share. The home has appropriate rooms to enable residents to socialise or have quiet time if they so wish. The home has sufficient toilet and bathing facilities to meet needs of its residents. Residents are encouraged to be as independant as possible. The home provides entertainments and activities within the home for those wishing to partake. Residents are encouraged to maintain contacts with relatives and friends outside of the home. The local church is very active within the home and is able to provide services within the home, counselling, and befriending. Residents wishing to attend other denominations are assisted to do so where possible or practical. This was an announced inspection, which took place over a morning. The inspector had previously sought the views of residents and relatives and professional organisations who have had contact with the home. The inspector met with both the proprietor and the manager. Two staff members were interviewed. Two residents were spoken to following comments made on the feedback form. Confirmation was given that the home is providing a good standard of care. Evidence was found at inspection that staff are receiving regular training and support. All records inspected were found to be satisfactory. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5,6. Information is provided to prospective residents in a clear and concise manner. EVIDENCE: The inspector was shown the homes brochure, which outlines what the home can and cannot provide. It was written in plain English and was easily understood. Each resident has a written contract/ terms and conditions. The residents contract is signed by the resident or his/her advocate. The manager assesses all prospective residents prior to admission. Admission is dependant on a mutual agreement that the home can meet the resident’s needs. Prospective service users are encouraged to visit the home prior to admission. Friends and relatives are encouraged to maintain contact with residents and feedback from relative’s indicated that they are made welcome when visiting and can visit at all reasonable times. An individual care plan is drawn up stating what the residents care needs are and stating how the home will met them. The home does not offer an intermediate care service. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10,11. Resident’s health care needs are being met. EVIDENCE: Evidence was found at the inspection by reading residents files and in talking to residents that individual health, personal and social care needs are being met. The home has appropriate policies and procedures to manage the storage, recording and distribution of medication. A random selection of resident’s medication was inspected and found to be satisfactory. The inspector spoke to two residents and received feedback from several others was assured that residents are treated with respect and that their privacy is being respected. Information is now being gathered to inform the home of the wishes of residents following their death. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15. The home provides a range of activities suitable to the needs of its residents. EVIDENCE: Residents spoken to at the time of the inspection stated that they were happy with the activities provided and the opportunities to continue to be active in the community and with friends. There is a pleasant garden, which has plenty of shaded area’s which is very well used by residents in good weather. The home has the benefit of two conservatories, which are used for a variety of activities. The inspector was present whilst the lunchtime meal was being prepared. Ingredients used were fresh and a choice was available. Residents were being consulted as to their choice of meal. Feedback to the inspector both from residents and relatives with regard to the food was positive. A selection of past menu’s inspected showed a good range of fresh foods being used with personal choice in evidence. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17,18. The home has a clear complaints procedure, residents were confident that any concern/complaint would be dealt with. EVIDENCE: The size and management structure of the home ensures that concerns\complaints if any are dealt with at source and without delay. The home has a formal complaints procedure. No complaints have been logged for some time. Residents spoken to stated that that if they had a complaint they would go to the proprietor or Manager and felt confident that it would be resolved. Evidence was found through reading case files and speaking to residents that their legal rights are being protected and that they are protected from abuse. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25,26. The home provides a safe environment for its residents. The home presents a warm homely environment. The home was found to bne clean on the day of the inspection. EVIDENCE: The inspector was shown around the home and with residents permission was shown their bedrooms. All bedrooms were individually styled with resident’s personal possessions much in evidence. The home has sufficient toilet and bathroom facilities. The home has sufficient communal area’s which can provide for entertainment or for peace and quiet. The home has appropriate equipment to assist staff in the safe moving and handling of residents. On the day of the inspection the home was clean and free from odour. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,20,30. The home is staffed in sufficient numbers of suitably qualified staff. EVIDENCE: The home employs sufficient permanent care staff to meet the needs of its residents. Evidence was found at inspection that the home had not followed procedure in obtaining a CRB check for one member of staff. The manager showed a lack of understanding of the process and needs to ensure that she is aware of the requirements of the Act in respect of recruitment. The inspector was impressed by the range and number of training sessions staff had attended. Staff had clear training plans and the home are to be commended for this initiative. Staff spoken to had been supplied with job descriptions and were clear of the expectation of the role. Staff who were spoken to were happy to work in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35,36,37,38. The home is run in a professional manner and has the required documentation to assist the manager and staff to be sure of the what is required of them. EVIDENCE: The registered manager is experienced and trained to manage the care provision offered by the home. The registered manager is supported to do this by a staff group of sufficient numbers and experience in the care of the elderly. There is much evidence of the proprietor’s involvement in the home to support the Manager and the ethos, leadership and management stems from this and the desire to provide a good quality service. The home has sufficient policies and procedures to assist in the protection of potentially vulnerable adults. The home is managed in a professional manner and the inspector was informed that it was financially sound. Accounts are available for inspection if required. Accounts were not required as part of this inspection. The health safety and welfare of residents are being promoted and protected.
